CSCI found this care home to be providing an Good service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

Staff were observed practicing good hygiene and infection control - such as wearing gloves and washing their hands as required. Staff were observed to be caring and responsive to people`s individual needs. Staff showed a good understanding of how to raise concerns.

What the care home could do better:

People who live at the home need to have a range of suitable activities on offer to provide stimulation and to meet their needs and preferences. The recording of peoples medication needs to be accurate to ensure that they receive their medication as prescribed. Staffing levels need to be adequate to meet people`s assessed needs. Arrangements need to be in place to cover for staff absence, including absence at short notice.

Date of last inspection Brief description of the care home Five Gables Residential Care and Nursing Home can accommodate up to 43 older people, some of who have dementia and/or physical disabilities. The home is situated in the rural village of Ringstead. Residents are accommodated in one of three units depending on their needs. The Villa, at the rear of the property, caters for mainly nursing residents, and has a small dementia unit within it. The Lodge, at the front of Care Homes for Older People Page 2 of 9 the property, accommodates those needing residential care only. The Villa, dementia unit, and most areas of the Lodge are accessible to people with limited mobility. The grounds include a car park, mature gardens, and patio areas. The fees at the time of the last key inspection ranged from 380 pounds to 400 pounds per week. Further information about the home is available from the Registered Manager. Care Homes for Older People Page 3 of 9 What we found: The inspection was conducted in the nursing and dementia unit following anonymous concerns raised with the Commission for Social Care Inspection. The inspection focused on the areas of concern raised which were:- i) Staffing levels and use of cover staff. ii) Administration of medication. iii) Activities available for people who live at the home. iv) Infection control. v) Use of hoists and equipment for helping people with mobility problems. There were a number of requirements made of the home at the last key inspection. These were not assessed on this visit but will be followed up at the next key inspection, or before if further information or concerns are received which suggests that the requirements have not been met. The registered manager was on annual leave at the time of the inspection. Staff were observed wearing gloves and aprons when attending to peoples personal and medical care needs and washing their hands after assisting individuals. Staff files contained evidence that staff received training in infection control. Staff confirmed that nurses, and not carers, administer medication in the nursing care unit. Medication administration records for four people were viewed. Several doses of medication had not been signed for although blister packs were observed and all showed the correct numbers of tablets. Four staff spoken to confirmed that there were adequate numbers of hoists to cater for peoples needs. The service records for these hoists were not available on the day of inspection. Staff explained that sometimes they are able to do activities with the people such as listening to music, talking to people or going outside. However this was not always possible. One person on the dementia unit commented I quite like it here. I dont do much during the day At the time of inspection there was a total of one nurse and five carers working at Five Gables. The nurse in charge explained that this was the usual allocation of staff. The nurse in charge explained that staff would work together to assist the people who require the assistance of two people to, for example transfer from a wheelchair to a chair. Staff rotas were observed along with a staff allocation book which showed how many staff were on each shift in each area of the home. The rotas for the previous four weeks showed that staff members had been taking annual leave and there had also been periods of staff sickness absence. There had been occasions where only three carers and one nurse had been working. The nurse in charge explained that there had been occasions when staff had phoned in Care Homes for Older People Page 4 of 9 sick at short notice and the home had been unable to cover their shifts. One staff member said we can be short staffed when people phone in sick but we manage Another commented We have been short staffed, we could do with 2 carers on the dementia unit Staff were seen communicating with people in a caring and sensitive manner. The nurse in charge was attentive to peoples requests for assistance and offered advice and direction to the carer that she was working with to ensure that peoples needs were met. Staff spoken to commented I am happy to work here, it is very calm Other staff comments were this is a good home peoples needs and choices are respected. Written policies on safeguarding adults,whistle blowing and complaints were observed and staff spoken to demonstrated an understanding of how to raise concerns. Yes R No £ Outstanding statutory requirements These requirements were set at the last inspection. They may not have been looked at during this inspection, as a random inspection is short and focussed. The registered person must take the necessary action to comply with these requirements within the timescales set. No. Standard Regulation Requirement Timescale for action 1 7 15(1) Care plans relating to 24/05/2008 challenging behaviour were either not in place, or lacking in detail. These must be reviewed and improved to ensure that residents needs are identified and met. The carpet tiles in communal 24/07/2008 areas in the Lodge must be replaced as they are worn, stained, and potentially a tripping hazard. This will improve the environment and increase resident safety. The garden and other outside 25/04/2008 areas must be risk assessed and action taken where necessary, to improve safety. This will help to reduce the risk of accidents.
