Conditions of registration: Date of last inspection 28th June 2005 Brief Description of the Service: Forget-me-not Residential Home is a care home providing personal care and accommodation for 16 service users aged 65 years and over. Mr and Mrs R. W. Pell privately own the home, which was opened in 1995. The two storey home is situated in a quiet road just off a main thoroughfare on the outskirts of Slough All the homes bedrooms are single, and 7 have en-suite facilities. There is a passenger lift. This is an overview of what the inspector found during the inspection. This unannounced inspection took place during the day on Thursday 15th December 2005. The Inspector looked at records and spoke with two service users, one member of staff, manager and proprietor of the home. Seven standards were assessed of which four were met, two almost met and one not met. Two requirements and one recommendation were made during this inspection. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Number 3. New service users are admitted only on the basis of a full assessment. EVIDENCE: Full assessments are undertaken before service users are admitted to the home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Numbers 7 and 9. The service user’s health, personal and social care needs are set out in an individual pan of care. Service users, where appropriate, are responsible for their own medication. Staff follow the procedures and policies for dealing with medicines except when recording the administration. EVIDENCE: All service users have individual care plans. They are regularly reviewed. Two service users self medicate some of their medicines. Staff follow the procedures for dealing with medicines except when recording the administration of medication. Several gaps were found in the MAR sheets. The pharmacist last visited the home to look at the receipt, recording, storage, handling, administration and disposal of medicines in May 2005. It would be good practice if these visits took place more frequently as it provides protection to service users. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Numbers 16 and 18. Service users are confident that their complaints will be listened to, taken seriously and acted upon. The registered person must operate a thorough recruitment procedure to ensure service users are protected from abuse. EVIDENCE: Service users are given a copy of the complaints procedures on admission to the home. Service users said staff are very approachable and they would talk to any member of staff if they had a problem. The manager said she encourages staff to talk to service users and address any issues. If they think a service user has a problem/concern they will speak to that person in private. Service users’ complaints are recorded. The majority of staff have undertaken appropriate ‘protection of vulnerable adult’ training. Staff would speak to the manager or proprietor if they consider a service user has been abused. A member of staff, who does not have direct contact with service users, has recently been employed without a CRB disclosure check. Even if the job description does not include direct personal contact this practice could place service users at risk. EVIDENCE: Records showed; a) a member of staff has been employed without a CRB disclosure check being requested; and b) it has been agreed for a new care worker to start work before the CRB disclosure application has been returned or a POVA first check being requested. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): Number 38. The health, safety and welfare of service users are promoted and protected. EVIDENCE: Records seen by the inspector showed that the manager ensures as far as reasonably practicable the health, safety and welfare of service users. Staff undertake regular fire awareness training. The manager and proprietor recently attended a fire safety course for managers. Service users’ risk assessments are regularly reviewed. The Environmental Health Officer visited the home to assess the standard of food hygiene and considered the home operates safe working practices.
