Conditions of registration: Date of last inspection 9th June 2005 Brief Description of the Service: Fosters provides accommodation and care for up to thirty-seven service users over the age of sixty-five years who have care needs associated with old age. The home is one of two local authority homes within the Wokingham District Council Unitary authority. Fosters is a purpose built home built in the 1970s. The home has been organised to provide separate dining, kitchen and lounge areas for 4 distinct groups. The home has recently registered an area to provide care for people diagnosed with a dementia. This is an overview of what the inspector found during the inspection. The inspection took place between 9.45am and 2.05pm. Included in the inspection was a tour of the home, discussion with the manager and staff, conversation with 10 residents and examination of some records. The home has no vacancies. It was agreed that within the report the term residents would be used for service users. Care plan received demonstrating action is taken to carry out requirements and recommendations. Serious consideration should be given to providing domestic staff to support care staff. A fire risk assessment needs to be prepared and available. Any action assessed as necessary needs to be completed. Recruitment records should include a recent photograph of the member of staff. One member of staff needs to have a second reference and a POVA check. The care planning system needs further review to ensure that each resident has full details of care required plus risk assessments. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 11 Care plans are improved, showing more detail of care expected. Risk assessments are known to be important but are not always completed. Residents and relatives are treated with sensitivity and respect when seriously ill. EVIDENCE: Staff have made great efforts to improve the quality of care plans and of daily recording. While statements are made about the way of lifting and handling plus bathing there are no risk assessments routinely used. It is important there is a clear understanding of the way staff should give care to residents. The manager is going to research different methods of recording to see if this can be improved. Risk assessments are now in place for all residents that use bedsides. This assessment has been made by staff and will be discussed and agreed with relatives. A relative confirmed that care given to her mother was thoughtful and considerate. That she and her sister had been consulted and included in all discussions. The care staff were gentle and considerate.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Residents are confident that their complaints will be listened to and acted on. Residents are protected from abuse. EVIDENCE: The complaints record seen demonstrated that complaints are taken seriously and acted upon. There has been one POVA (Protection of Vulnerable Adults) referral by the home to the local authority. There were no staff involved. Most of the staff have completed POVA training. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The house is well maintained and being improved. EVIDENCE: The house was purpose built as a residential home some years ago. The home has recently had some work to improve it and more is planned. The corridors on the first floor had just been decorated and during the visit carpets were being laid. Further decoration is planned downstairs. The entrance hall is now looking very worn. The manager is aware of the condition of the home and works very hard to improve the building. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30. Adequate staff meet residents care needs with limited time for activities and personal time. The recruitment practice is safe except 2 references are required for all staff. Staff training in mandatory areas is satisfactory. EVIDENCE: The care staff provided work hard to meet the needs of increasingly frail residents. Care staff also provide domestic cover for residents rooms and communal areas. This means there is very little time for activities and personal time with residents. The manager has recognised this problem and requested domestic staff to give care staff more time. By observation during the visit and through experience from other homes the Inspector would strongly support this proposal. In one wing 2 members of care staff were providing care and support for 11 residents plus cleaning all rooms and some preparing and serving meals. The needs of residents admitted to homes are generally greater and the manager aims to provide a home for life. Recruitment records are responsibly managed including an application form, interview record, references and CRB check. The exception is a member of staff that transferred from another dept and under Council policy only one reference was needed. Also the existing CRB was used. Minimum standards ask for two references for all staff and CRB require a POVA check therefore a new CRB check was required. Also all recruitment records need an up to date photograph of the member of staff. Training records are held in individual units plus being transferred to central computer records for management overview. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 and 38 The home is run in the best interests of the residents. Health and safety of residents and staff is promoted, the exception is the need for the fire risk assessment. EVIDENCE: The quality assurance procedures include monthly regulation 26 visits, preparing an annual development plan, regular staff meetings and a recent residents questionnaire. The results have been collated and sent to CSCI. Residents were very positive about the home. The action sheet following this is to be displayed. The residents finance records are now completed with 2 signatures, involving the resident, whenever possible. There is a responsible approach to health and safety. The lift and hoisting equipment was serviced. Fire records were seen and included weekly checks

The Registered Provider(s) must comply with the given timescales. It is required that all recruitment records include a current photograph of the member of staff. It is required that a second reference and POVA check is obtained for one member of staff.
