Conditions of registration: None Date of last inspection 8 March 2005 Brief Description of the Service: Franklyn Lodge belongs to Residential Care Services Ltd, a private company, based in Brent specialising in providing personal care and accommodation for younger adults with learning disabilities in care homes. The home is located in a residential area of Sudbury. There are some local shops about five minutes walk from the home. Public transport is available on the main road, where District Road starts. There is a driveway with parking for one to two cars. Additional parking is available on District Road, which does not have parking restrictions. The home itself is a detached bungalow, with most accommodation on the ground floor. The attic is used for storage. There are three single bedroms (one with en-suite toilet and wash hand basin), a through lounge/dining area, a bathroom with a bath and shower and a kitchen. There is a paved area in front of the home with wheelchair access and a maintained garden at the back and a patio, which can be reached from the lounge/dining area. This is an overview of what the inspector found during the inspection. The unannounced inspection took place on a early midweek morning. The focus of the inspection was to look at the requirements from the previous inspection of 8th March 2005 and to ensure that the core standards had been assessed over the year. The Inspector looked around the building and a number of records and policies were inspected. The Inspector is grateful for the contribution made to the inspection by the Registered Manager and the resident at the home. What the service does well: What has improved since the last inspection? Though evident in the care home, the resident would have benefited from support around his mobility whilst on the group holiday and had experienced frustration in having limited access on an occasion. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Prospective service users and their relatives have the opportunity to visit the home prior to admission. EVIDENCE: There is only one resident currently at the home and he has been settled at the home for seven years. There are two vacancies at present and a number of potential residents have been visiting the home with a relative or their community worker but have yet to make a formal application. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 9 Residents are encouraged and supported to make choices. Residents’ assessed needs include appropriate risk assessments. Processes are in place to ensure that service users comprehensive needs are identified and met. EVIDENCE: The current resident has no allocated worker in the community, but is ‘open to the team for review’ for Ealing Community Team for People with Learning Disabilities. No external review has taken place this year but there is an established pattern of internal reviews taking place regularly every six months. The resident’s last review took place on 12/4/05. The review process is comprehensive and covers issues arising and resolved since the last review, general health, including details of specialist appointments and GP follow up, medication, mood and behaviour, independent living skills, education and day care leisure, personal care and hygiene, finance etc. The resident’s views are captured during the review, and the resident when spoken with confirmed that his say was important. Residents have the opportunity to be represented by trustees, family, friends, advocates etc at reviews.

For instance, there was a risk assessment on file reflecting thinking about furniture needed in the resident’s room balanced against the risks of tripping or falling. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, ,13, 15, 16, 17 Arrangements are in place for identifying and providing residents with their preferred activities. Residents are supported in maintaining links with significant others and their previous communities. There is understanding of residents needs and interaction with residents is in a positive and respectful manner. EVIDENCE: Residents at Franklyn Lodge have access to Franklyn Lodge Day Services, 5 days a week from 10am – 3pm. Transport collects the residents. The resident spoken to was very satisfied with the structure offered and the respect given to his lifestyle. The residents are supported in maintaining important relationships particularly contact where residents originated. Continuity is maintained and regular outings take place over the weekends through these maintained relationships. The key worker goes to a shopping centre with the resident to buy clothes. The resident described the food as being ‘lovely’ at the home and mealtimes felt relaxed and flexible to support the resident’s schedule. A balanced healthy breakfast was being served during the course of the inspection. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 Residents health and welfare needs are generally identified and met, though one resident had a recent poor experience (whilst on a group holiday) of their needs not being met. EVIDENCE: The resident felt that his health needs were well taken care of. There was evidence on file to support this view. Regular chiropody, optician and dental appointments were taking place and regular out-patient reviews were documented. Medication was observed to be supervised. The MAR sheets were accurate and an explanation of the potential side effects of medication were detailed. The resident described a poor experience he had had whilst on holiday with other residents from the Franklyn House group of homes and to which the general manager had been made aware. Because of mobility problems the resident had not been able to access an historic site in which he was most interested. He had felt stuck in his wheelchair and isolated and most disappointed he could not take part fully in the visit. With hindsight, he felt staff on the holiday could have facilitated this. JUDGEMENT – we looked at outcomes for standard(s) 22, 23 The home has policies and procedures in place in regard to the protection of vulnerable adults and ensures that residents are safeguarded from abuse. EVIDENCE: The resident felt that he was happy to express a view and knew that he was listened to and his views were acted upon. The complaints procedure was up to date and displayed in the hallway. The current resident originates from the London Borough of Ealing. There was an Ealing Protection of Vulnerable Adults policy on file. The manager had been on POVA training last year. There were no allegations recorded of abuse. The one resident has a trustee who handles the resident’s finances. The trustee is party to reviews. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 30 The home is generally well maintained and the surroundings are comfortable. The home is clean and free from offensive odours. EVIDENCE: The premises are kept clean, hygienic and free from offensive odours throughout the home. Laundry facilities, washing machine, dryer and butler sink are sited outside in a secure facility. Hand washing guidance is provided by sinks and disposable towels are in operation. The home is homely, warm and comfortable and well maintained. There were some areas that needed attention: - the exposed external wiring at the side of the house needs to be rectified. - the laundry basket and lid needed to be removed from the front garden. - the window sill at the back of the kitchen was rotting and needed replacing. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 34, 35 This is a small home with a stable trained staff group. EVIDENCE: Recruitment procedures are all administered centrally via the organisations central office. Throughout the organisation, the manager assured the Inspector of 50 of the care staff have achieved an NVQ level 2 qualification. Training is also organised centrally. The manager herself was currently undertaking a part-time course to obtain a Social Work qualification (Diploma of Social Work). Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 39, 42 The manager is experienced and has known the current resident for a number of years. The health, safety and welfare of residents are generally safeguarded. EVIDENCE: Action is progressed within agreed timescales to implement requirements identified from the previous unannounced inspection. All the previous requirements and recommendations were met. The resident spoken to felt that he was fully listened to and his views were taken on board in the running of the home and that he was accorded respect and dignity. Two requirements arising from the previous inspection: - Risk assessments are in place for accessing hot water.

A number of up to date risk assessments were on file for the resident. For instance, for falls and one for furniture layout. The proprietor’s monthly report is monthly and fridge and freezer temperatures are monitored and recorded. The gas safety record was inspected and was up to date (17/3/05). There was substantial safe storage space in the back garden.
