Date of last inspection 7th December 2005 Brief Description of the Service: Freedom Care Limited at 70 Conway Drive is in the village of Shepshed. The home is registered for a maximum of two people with learning disabilities. , The home is a converted detached bungalow, to the ground floor of the property there is a lounge, dining room, kitchen main bath and shower room, and home office. Resident’s bedrooms are situated on the first floor. There is a small garden at the rear of the home, which residents can use. This is an overview of what the inspector found during the inspection. The focus of the inspections undertaken by the Commission for Social Care Inspection is upon outcomes for service users and their views of the service provided. The primary method of Inspection used was ‘case tracking’ which involves selecting clients and tracking the care they received through looking at their records, talking with them where possible, and looking at their accommodation. This inspection took place over one weekday, commencing at 1.00pm and took three hours to complete, and was assisted by the Team leader of the home. An opportunity was taken to look around the home, view records, policies and care plans and to talk to both residents. The current fees charged weekly fall between £1800 and £2495 per week. What the service does well:
A pre-admission assessment process is in place in the home. Ongoing reviews are held with social services and health professionals. The care plans are based on a person centred approach and incorporated residents aspirations and goals and are individually detailed. Care plans are supported by risk assessments, which detail how risks are to be managed, and detail crisis intervention, including access to external agencies. Positive choice is offered, and each resident’s care plan. Both residents have planned education built in to their daily life; leisure is varied with residents deciding on a daily basis, what they want to do. Families are encouraged to keep in contact, and visiting times not restricted. Residents’ emotional well being is dealt with appropriately in the plan. Menus are made up in advance, staff assist with the nutritional balance and ensure a healthy diet is offered. The monitoring of residents weights and healthcare in general is regularly maintained, with additional input and advice sought from health professionals’. Personal support is offered on a flexible basis. Medication is administered appropriately and risk assessments are in place for “as required” medications. The complaints policy information was available for staff to view and refer to, staff spoken with on the day showed an awareness of the complaints procedure, and were also aware of the whistleblowing and the Protection of Vulnerable Adults policy. The home provides relaxed and comfortable accommodation for the two residents. Bedrooms are personalised and individually decorated. The staff in the home when spoken with demonstrated a good awareness of Health and Safety issues. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. Quality is this outcome area is good. This judgement has been made using available evidence including a visit to this service. The assessment process is well managed with assessments of need being carried out to ensure service users needs are met. EVIDENCE: An assessment process is in place in the home, but there have been no new assessments recently, because the same residents are living in the home. The assessment process is ongoing: care plans are regularly updated and reviewed, and staff have a excellent awareness of the changing needs of residents. Ongoing reviews are held with social services and health professionals; evidence of these was evident on files viewed on the day. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 & 9. Quality is this outcome area is good. This judgement has been made using available evidence including a visit to this service. Individual plans of care are in place ensuring that service users are supported to make decisions concerning their care and treatment in the home. EVIDENCE: The Inspector viewed a sample of care plans; all were detailed and regularly reviewed, though not signed by the resident or their relatives. The care plans are based on a person centred approach and incorporated residents aspirations and goals and are regularly reviewed. Care plans are supported by risk assessments, which detail how risks are to be managed, and detail crisis intervention, including access to external agencies. Positive choice is offered, and each resident’s plan has been changed to reflect the coming holiday period from college. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 & 17. Quality is this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents have choices of various activities both within the home and out of the home, providing a good lifestyle. EVIDENCE: On the day of the inspection the staff and residents had returned from a visit to the local gym, these take place twice weekly and residents are accompanied by staff. Residents are fully involved in the choices made to enable a varied lifestyle. Both residents have planned education built in to their daily life, and at these times life is more structured, however as the college is presently on a summer break, days are more relaxed, one resident stated, “ I can go to bed and get up when I want”. Leisure is varied with residents deciding on a daily basis, what they want to do. Families are encouraged to keep in contact, and visiting times not restricted.

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20. Quality is this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ health and personal care needs are met on an individual basis. EVIDENCE: Personal support is offered on a flexible basis, care plans reflect what abilities residents have; these are regularly reviewed and updated. The monitoring of residents weights and healthcare in general is regularly maintained, with additional input and advice sought from health professionals. Medication is administered appropriately, the staff when spoken with, showed a good awareness of administration techniques. Medication is stored securely, and medication administration records (mar charts) are up to date and signed appropriately. Risk assessments are in place for “as required” medications, staff indicating verbal authority was also obtained prior to giving these medicines. EVIDENCE: The complaints policy information was available for staff to view and refer to, staff spoken with on the day showed an awareness of the complaints procedure, and were also aware of the whistleblowing and the Protection of Vulnerable Adults policy. Residents were aware of a number of contacts other than staff in the home they could make any concerns to. No current records of complaints was available to the inspector. Quality is this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents live in a homely, comfortable and clean environment. EVIDENCE: The home provides relaxed and comfortable accommodation for the two residents. Bedrooms are personalised and individually decorated. The staff in the home when spoken with demonstrated a good awareness of Health and Safety issues. Laundry facilities are domestic in nature and suitable for the residents’ needs. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35. Quality is this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Residents’ benefit from good staffing levels and training. EVIDENCE: Staff stated they are given training to assist them with their work, though staff records were unavailable to confirm this. Staff talked about their jobs and demonstrated knowledge of the resident’s individual care plans, likes and dislikes. The team leader indicated 3 out of the 6 permanent staff have completed National Vocational Qualification level 2 or above, this represents 50 of the staff group, which is the minimum advised within the Care Standards Act 2000. Visual confirmation of certificates was unavailable at this time due to staff files being unavailable. There was enough staff at the home to support the residents, during waking hours no less than a one to one ratio with residents. Staff said they felt well supported by the manager. It was not possible on this occasion to confirm the recruitment practices of the home were secure and the appropriate clearances and references obtained on

This judgement has been made using available evidence including a visit to this service. Improvements to the availability of information for inspection purposes is required to safeguard service users’ interests. EVIDENCE: Staff feel that the manager is supportive. There are a number of policies and procedures in place to enable staff to care appropriately for residents. Safety tests are done periodically to ensure the home is safe for residents. Staff stated they use pictures to help aid communication with residents. Staff communicate well with residents, and treat them with respect. The owner has developed surveys to use with other organisations to gain their views of the service, however these were unavailable for inspection purposes, as was any other quality assurance information.
