Conditions of registration: N/A Date of last inspection 12th October 2004 Brief Description of the Service: Friary Fields is a care home providing personal care and accommodation for 25 older people. An application is currently being considered to include caring for people with dementia. The home provides short and long term care and will accept emergency admissions. The home is owned by Friary Fields Limited which is run as a family business The home is located in a residential area close to Newark town centre where there are shops, pubs, post office, theatre, churches and other amenities. The home was opened in 1982 and consists of an extended domestic dwelling. 21 of the home’s bedrooms are single, and none of the bedrooms have ensuite facilities. Bedrooms are located on 3 floors, some rooms are served by a passenger lift and others a stair lift. The gardens have been temporarily out of use due to building work and are to be landscaped in he near future. There is car parking available for 8 cars. Since the last inspection the home has built a six bedded extension which is hoped to be registered shortly. This is an overview of what the inspector found during the inspection. This inspection was the first of two unannounced inspections to be carried out between April 2005 and March 2006. The inspection lasted for 5 hours and the main method of inspection used was called case tracking which involved selecting 3 residents and tracking the care they receive through the checking of their records and discussing this with them. Other residents were spoken with and additional records were seen. A discussion was had with the manager, care staff on duty and care practices were observed. Other residents and relatives were spoken with as well as a visiting social worker. The premises were also inspected in light of an application received to vary the homes current registration. The home should make sure that up to date tissue viability assessments are used when required. There were some Health & safety concerns, particularly with regard to the risk of residents falling down the stairs, and a hazardous exit route at the rear of the property, if evacuating along the fire escape – particularly at night. Windows on the 1st and 2nd floor have not been fully restricted to prevent any accident.

When recruiting staff from overseas the same recruitment checks should be carried out as for staff from this country. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. EVIDENCE: The home require a community care assessment to be provided prior to admission to establish whether the needs of any potential resident can be met. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 The documentation allows for residents needs to be set out in the care plan but needs to be kept up to date. EVIDENCE: Care plans include a daily living assessment and these detail key areas of care and how these are to be met. Residents said that staff know the support they want and help them when needed. A record is made of all medical appointments in the daily notes. There are forms for monitoring the well being of residents where concerns are identified, however one resident who is confined to bed did not have a recent tissue viability assessment on file or details of support required in the care plan. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 Residents have opportunities to take part in activities and spend time with friends and family. EVIDENCE: Residents have the choice of 2 lounges, and were seen involved in differing activities. Residents said they enjoyed playing various games and liked doing word searches. A group of residents were seen playing a board game during the inspection. A member of staff was seen taking requests from residents when going to the shops, including buying a pair of slippers and purchasing a lottery ticket. Visitors are welcome at any time and can see residents in private. One resident described going out with her friend and another said that her family visits her. Staff said that residents are able to make choices over their daily routines. Residents were seen choosing how they were going to spend their time and discussing the colour of some new slippers. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 and 22 Residents have been disrupted by the current building work. Some work is required in order to promote the safety of residents. EVIDENCE: There have been a number of temporary arrangements made due to the recent building work. Residents said that they had got used to the changes but were looking forward to being able to use the main lounge again. A variation request to care for people with dementia is currently being considered by The Commission for Social Care Inspection. The suitability of the accommodation was considered during this inspection. As an older building it has considerable character and many interesting features, but these also have some associated risks. Rooms by a staircase (10, 11 and 12) are unsuitable for use with people with dementia. The banisters at the top of this staircase pose a potential hazard and this needs to be addressed. Access to the 2nd floor can only be achieved by the use of the lift and residents need to be accompanied by staff when using this. Only residents who have been assessed as being able to call for assistance should be placed in these rooms. Windows on the first and second floor have not been adequately restricted.

JUDGEMENT – we looked at outcomes for standard(s) 29 & 30 The home has the correct recruitment procedures, however these are not being followed in all cases. EVIDENCE: Correct recruitment procedures are followed for staff who are employed from this country, however the home has recently employed some staff from overseas. These staff have been recruited through an employment agency who collected the required references and criminal records check in order to secure a work permit. The home must follow their full recruitment procedures for all staff irrespective if they have already provided similar information to another body. Records in staff files showed that there was a good range of basic staff training available in the home.
