The total number of service users in the Home must not exceed 44. 4. Date of last inspection Brief Description of the Service: Gabriel Court is a large property located in Kettering in a quiet residential street and provides personal care, and not nursing care, for up to forty-four older people. There is a relatively new extension to the rear of the original building that provides additional ground floor bedroom accommodation. The original building has lifts to the upper floor, and along with the extension Gabriel Court provides a comfortable, homely environment conducive to the care of this large group of service users. Additionally, there is an attractive paved garden to the rear of the building that the service users enjoy during fine weather. As Gabriel Court is located close to the centre of Kettering, there is ready access to community resources. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This process considers the home’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provision that need further development. The primary method of inspection used was ‘case tracking’ which involved selecting two service users and tracking the care they receive through meeting with the service users, a review of their records, discussions with the care staff and observation of care practices. The Inspector spent an hour preparing for the inspection and visited the home for four and three quarter hours during mid morning and lunchtime period from 11.00am to observe practices by staff and to meet with service users. Some Service Users have diagnosed Dementia conditions or confusional states and thereby communication for some is difficult. Establishing Service Users choices and informed decisions is dependant to some extent upon the consistency of staff, service users relationships with staff, and the quality of communication. Feedback obtained from Service Users in this report was in part through observations of their relationships with staff, and also through interpretations of their general levels of happiness with their routines. At the time of the inspection the inspector had received thirteen comment cards from Service Users’ relatives, comments were generally positive, although 2 raised queries about staffing. The homes self-assessment form was not yet due and therefore not available to the Inspector. What the service does well:
A sensitive understanding of Service Users holistic needs is being promoted by the Registered persons, and relevant training and guidance to enhance staff knowledge understanding and practice in this area is being provided. Staff are alert to the individuality and needs of Service Users and were seen to be caring in their approach and relationships. Service Users told the Inspector that staff were very caring and often exceptional. Night care was felt to be very good with staff checking on Service Users throughout the night, and responding promptly to their needs and wishes. The home was found to be very clean.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Prospective Service Users needs are properly assessed prior to their moving into the home. EVIDENCE: A person who had been recently admitted to the home had been properly assessed and a provisional care plan created. Service Users who spoke to the Inspector about their admission reported that they had been very happy with the arrangements, had been given sufficient information, and had been warmly welcomed and made to feel at home by the staff. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,10 Service Users needs are well met. EVIDENCE: Service Users reported that they were happy with their care and that staff were aware of privacy and dignity issues. Records showed that Service Users care needs were regularly reviewed and details updated as required. Service Users are properly involved with the development of their care plans. Records are maintained of community health services input, and appropriate healthcare support is accessed as required. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 The food provision at the home is wholesome nutritious and appetizing. EVIDENCE: Service Users were very happy with the food provision and some identified that their choices were sought and taken more into account lately. Some Service Users were concerned that as staff were busy over the lunchtime period the food was sometimes rather cool when it was served in the “new” extension. They were aware that staff were happy to re-heat the food if necessary. The Registered Owner plans to provide a heated trolley for food transfer from the main building. (See Recommendations) The Inspector saw the midday meal which was appetizing nutritious and well presented. Alternatives were offered. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Service Users are confidant that they are listened to and that any concerns that they raise will be responded to. EVIDENCE: Service Users informed the Inspector that they feel their opinions are taken into account through residents meeting and individual discussion with the Registered Manager and/or with the Registered Owner. They were very positive in their opinions that efforts were being taken to seek and respond to their views, and they were confident that if they had any concerns or complaints that these would be taken seriously. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 Service Users have a pleasant and well-maintained environment. EVIDENCE: The home is comfortable, homely and well maintained. Service Users are very happy with the level of cleanliness and commented on the emphasis that has been put on the “deep” cleaning of communal carpets recently. Advice was given to review the adequacy of lighting in a few areas of the home, and to implement the use of disposable towels in communal bathroom and toilet areas in the interests of guarding against cross-infection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30 Staffing is in the process of being reviewed by the Registered persons as some shortfalls have been identified. EVIDENCE: The Registered persons were already aware at the time of the inspection of potential shortfalls in the staffing at some times, and are in the process of employing more staff to cover peak periods. The new Registered Manager is reviewing staff training needs in conjunction with the Registered Owner and appropriate training is being accessed to resolve any shortfalls. Staff have responded positively to these new training opportunities and are enthusiastic about the increased professionalism that they are working towards. The Inspector observed that staff need guidance sometimes on the way in which they organize their duties to ensure effective cover at all times. Recruitment processes are undertaken professionally and staff records are in the process of being reorganized to enable better access to information. There were no adverse comments about staff from Service Users except the observation that they are sometimes very busy. Service Users felt that their relationships with staff were very positive.

Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,33 The recently appointed Registered Manager is effectively addressing issues to ensure that all the National Minimum Standards are fully met and is properly supported in this by the Registered Owner. EVIDENCE: The Inspector noted positive progress since the last inspection, which will enhance Service Users quality of life. The benefits of these changes are already being noticed by Service Users. Service Users find the Registered Manager and the Registered Owner approachable and responsive. Previous Requirements and Recommendations have been met and the homes own Quality Audit processes have identified other necessary improvements which are being acted upon.
