Conditions of registration: 1. None Date of last inspection 8th March 2006 Brief Description of the Service: Gallagher Care Residential Home is located on the outskirts of Shepshed, with easy links to Coalville and Loughborough and within easy reach of local facilities. The home is registered for nine adults with a learning disability. The property comprises of seven single and one shared bedrooms located on the ground, first and second floors. On the ground floor there are two lounges, and a dining area. Bathrooms/shower rooms and toilets are sited on the ground and second floor. The house has a large rear garden, which is well maintained by a service user. The range of fees currently charged is £1306.29-£2737.86 every four weeks. Extra charges for services such as hairdresser and chiropodist are made on an individual basis. This information was given on the day of the inspection. This is an overview of what the inspector found during the inspection. The primary method of inspection used was ‘case tracking’ which involved selecting three residents and tracking the care they receive through review of their records, discussions with them and with the care staff and observations of care practices. A plan was made prior to the visit in which available information from the previous inspection report and service history was summarised. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ needs are assessed in order to inform the care planning process. EVIDENCE: All three files tracked contained community care assessment documents and review notes. The registered manager confirmed that such assessments are always acquired prior to admission. Residents’ needs are subsequently assessed as part of the care planning process. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents have comprehensive care plans that focus on the individual and useful risk assessments. These enable them to make choices, take risks and for their needs to be met. EVIDENCE: All files tracked contained comprehensive care plans. All but one had been recently reviewed and the assistant manager stated that all care plans are currently being reviewed. Residents confirmed that they are enabled to make choices. One stated that he often prefers to opt out of activities and is able to do so if he wishes. Risk assessments are being developed for residents at present. An example was shown to the inspector and was a very useful and comprehensive document. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ rights, activities within the local community, contact with families and the quality of the food all contribute to a positive lifestyle. EVIDENCE: Residents who spoke to the inspector stated that there is a good range of activities on offer. One resident enjoys shopping and another went to a local air show the weekend before the inspection. A group of residents are about to embark on a holiday to Mabelthorpe. Residents are also enabled to attend local day centres and colleges. A resident spoke to the inspector about voluntary work he carries out at a local charity shop. Residents are enabled to use local facilities such as shops, pubs and parks and are encouraged to be as independent as possible. Residents are enabled to keep in touch with families; files contained a list of significant contacts. Residents who spoke with the inspector stated that staff treat them with respect. Household tasks are shared, one resident maintains the garden and a group were preparing dinner when the inspector arrived at the home. All residents who spoke with the

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ personal and healthcare needs are met and they are protected by medication policies. EVIDENCE: Files contained information regarding residents’ personal and healthcare needs. Residents who spoke with the inspector stated that staff meet their needs. Medication provision has improved at the home. An alternative pharmacist has been sourced who uses a monitored dosage system. This in turn means that medication records have improved. All those seen by the inspector had been completed correctly. Medication is stored appropriately and managers have reiterated with staff the importance of safe practice when giving medication. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are protected from abuse and their concerns and complaints are listened to and acted upon. EVIDENCE: The home has a robust complaints procedure and uses the Multi-Agency Vulnerable Adult Protection document No Secrets. Staff have had training on adult protection. When asked, residents stated that they knew who to speak with if unhappy about the service. Records of complaints indicated that appropriate action is taken in a timely fashion. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The property is homely, comfortable and safe. EVIDENCE: Two residents showed the inspector round the home. The property was clean, tidy safe and in good decorative repair throughout. The shaving point mentioned in the last report has been disconnected and the boiled room is now locked. Residents’ rooms are personalised with décor, pictures and belongings. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ needs are met and they are protected by the recruitment and retention of a well-trained, experienced staff team. EVIDENCE: Staff files contained records of a comprehensive induction programme and staff training. Staff are adequately supervised although the process of recording formal sessions is not yet up and running. Risk assessments for lone working need to be completed. Staff are offered a comprehensive training package including NVQ awards where appropriate and the deputy manager is currently completing the NVQ assessors award. Staff files contained appropriate documents except references which none had. All resident who spoke with the inspector stated that they like the staff and that they feel their needs are met. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39,42 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well managed. EVIDENCE: The assistant manager is currently working to take on the role of registered manager. Residents who spoke with the inspector were aware of the roles of senior staff and appeared to be confident to approach them if necessary. Records regarding health and safety were up to date and accurate. Fire tests and drills have been carried out at appropriate intervals. The home has recently received a fire inspection at which they were found to be complying with relevant legislation.
