This inspection was carried out on 24th October 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The manager, her deputy and a core of the staff team have worked in the home for several years. This provides the basis for a home that from entry appeared calm, warm and friendly. Residents appeared contented and those spoken with confirmed the standard of care was good. Two visitors spoken with were very pleased with the care provided. The home itself was clean and well maintained with no odours noted. The records for providing suitable personal care and to ensure safety in the building were well maintained and regularly used. Regular meetings are held for residents and staff with a range of issues discussed.

What has improved since the last inspection?

Since the last inspection, good practice in food storage has been implemented in the kitchen and kitchen staff are working towards a certificate in catering. The manager is near completion of the Registered Managers Award. The home continues to be well maintained.

What the care home could do better:

Employment practices were identified at the last inspection as requiring improvement. Again at this inspection poor practices in employing staff were identified. As this could lead to residents being at risk, an immediate requirement was made to ensure their safety.

The accommodation consists of 2 units, both on the ground floor with the upper floor used for storage and staff facilities. One unit consists of 20 bedrooms with ensuite toilet facilities for frail older people, the other is of 20 bedrooms with ensuite toilet facilities for older people with a dementia. The home does not provide nursing care. Garden Lodge is situated in a residential area of Walker, a suburb to the east of Newcastle upon Tyne. The home is close to local shops and public transport links. This inspection took just over 4 hours and in that time I spoke with several residents and their visitors. I obtained information from the home’s management, reviewed a selection of records maintained in the home and toured the general areas of the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Pre-admission assessments are conducted by management to limit the possibility of unsuitable placements being accepted. Intermediate care is not provided in this home. EVIDENCE: Four residents files were checked and each contained a pre admission assessment. The relative of a recent admission confirmed the manager met with her relative in hospital before the placement was confirmed. The home does not provide a rehabilitation service but respite beds are available. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 8 Good quality records are maintained regarding residents’ health and personal care needs. EVIDENCE: Four care plans reviewed, contained detailed information regarding activities of daily living. Arrangements to meet individual healthcare requirements were detailed and the plans were regularly evaluated. The care provided in the home is supported by the community nursing service, with a community liaison nurse visiting the home weekly to discuss any health issues. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Residents and visitors are comfortable about making a complaint and are confident their concerns are taken seriously. Although staff receive training in abuse awareness some employment practices do not support this standard. EVIDENCE: Residents and visitors that were asked were confident that management take complaints seriously. A log of complaints is maintained in the home and these were investigated and well documented. Staff have received training in abuse awareness and this is supported by appropriate policies and procedures. The home’s employment procedures are clear but in practice they do not protect residents from potential abuse (see standard 29). The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 The home is safe, clean and well maintained. EVIDENCE: The home is safe with all residents’ facilities on the ground floor and with level access to the outside. A good standard of hygiene was noted throughout the home with no offensive odours found. The laundry is fitted with washing equipment that meets disinfection standards and clear information is displayed for staff regarding washing programmes that are to be used. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The procedures for recruitment of staff are not robust and do not offer protection to the residents of the home. EVIDENCE: Staff are employed without having their references checked and without getting a valid check done by the Criminal Records Bureau. The home also has no system in place to audit staff files. It is important for the home to maintain a good standard in recruitment to ensure their residents are safe. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35,37 and 38 The manager has many years experience in the care of older people and runs the home in the best interests of the residents. The financial procedures are accountable. The building is well maintained with a reasonable standard of health and safety. EVIDENCE: The manager has many years experience of providing care for older people and is nearing the completion of the Registered Managers Award. There are regular meetings involving staff and residents where a good range of issues are discussed. A receipt is obtained for all purchases made for residents by staff. The use of the staff accident book does not follow the procedures laid down to meet data protection. Other records are maintained to a good standard and the home is safe. YES STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. Any person working in the home currently without these having been obtained should be appropriately supervised to ensure vulnerable residents are safe.
