Conditions of registration: None Date of last inspection 11th October 2004 Brief Description of the Service: Gerald Street is a Local Authority owned residential care home, which provides permanent accommodation for up to thirty-five older people with personal care needs including a residential short break (respite care) service for up to three people. The Home does not provide nursing care. A day resource centre for older people is also operated on the same site. All accommodation is at ground floor level and is divided between four units, each with its own self-contained facilities including lounges, toilets and bathrooms and access to a central dining area. The property is situated on a housing estate in the Whiteleas area of South Shields and is within walking distance of a range of local amenities, including a small selection of shops and a Church. The area is well served by public transport therefore easily accessible. There is ample care parking to the front of the building. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over seven hours and was carried out as part of the annual inspection programme. A tour of the building took place and discussions were held with care staff, the Registered Manager, assistant managers’, six of the twenty-eight residents and four relatives. Three resident care plans, staff training records and activities programme were inspected. What the service does well: What has improved since the last inspection?
This is a Home that has little to improve particularly in relation to staff relationships with the residents and visitors and as a team; the home is well run, the standard of accommodation is very good and the meals provided are highly praised. Staff training records have improved. One of the assistant managers has spent sometime making sure the paperwork is up to date and shows the right information. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2 and 4 The Home has enough information in the statement of purpose and service user guide to help prospective residents decide if they would like to live there. The Registered Provider needs to review the categories of registration to ensure the home is correctly registered and can meet the needs of prospective residents or those currently living there. Staff have limited training in dementia care therefore might not be fully aware of the needs of those residents with dementia. EVIDENCE: The Home has an up to date statement of purpose and service user guide. Some discussion took place about residents who might need dementia care services, which the Home is not registered to provide. One resident is being assessed for possible transfer to a service that might better meet their needs. A small number of other residents have got degrees of dementia, which staff said are not unmanageable. Some staff have worked in dementia care

The Local Authority have produced a terms and conditions document, entitled ‘Resident’s Agreement for Provision of Care’, which has been reviewed by the Commission for Social Care Inspection and agreed as meeting the National Minimum Standards. A good range of information leaflets are displayed in the home about services in the local area and activities in the Home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 Staff are keen to improve the care planning system but need further training and guidance when the new documentation is provided. This should help staff to assess the needs of residents in a more holistic way, involving the residents to ensure their needs are fully met. Appropriate involvement of health care professionals is evident therefore residents can be assured their health needs are met in a timely manner. Communication between staff is well organised therefore the residents and their relatives can be confident that information is passed on between staff. EVIDENCE: Staff have attempted to improve the care plans in the home. Some elements are not within the current care plan, for example, pressure area and nutritional assessments. A working group in the Local Authority has developed a new care plan, which they have forwarded to the Commission for Social Care Inspection for comment. During the inspection healthcare professionals were seen visiting the home, for example, the district nurse and phone calls were made to the GP service.

The Registered Manager ensures the Commission for Social Care Inspection is informed of untoward incidents and accidents involving residents by submitting regulation 37 notifications for each event. Residents and relatives stated they were very happy with the care provided at the home, one saying, ‘the Queen couldn’t have better’. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 care staffing levels are sufficient for provision of care therefore the expectation that they will organise activities means they are only able to provide a limited range of social events. Staff ensure they are available to support relatives therefore build relationships so that they can deal with issues raised. EVIDENCE: All the residents and relatives spoken to stated they were very satisfied with the care and services received at the home. One lady had a poem published in the local gazette, which praised the kindness of the carer’s at the Home. Staff interact with the residents in a friendly, respectful manner conveying a homely atmosphere. Relatives and visitors are free to come and go as arranged with the resident in the home. They are made welcome by staff and have the opportunity to sit in a number of quiet areas as well as the bedroom of the person they are visiting. Currently staff organise social activities supported by a young person employed at the Home as part of the youth opportunities scheme. The main activity residents say they enjoy is bingo. Several photos are displayed of

The cooks support social events with buffets, which are held in high regard for the variety and standard of food provided. The Registered Manager stated the Local Authority has not yet provided information to residents about their right to access their personal records, which has been made a requirement on previous inspection reports. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The complaints procedure is clear therefore residents, visitors and staff are able to follow the guidance easily. The management style in the Home is open and inclusive therefore any concerns are dealt with quickly and reduce the likelihood of a complaint being made. EVIDENCE: No complaints have been recorded in the complaints register and staff are not aware of any complaints being made to the Home. None have been made directly to the Commission for Social Care Inspection. The complaints procedure is provided in the service user guide and displayed in the Home. Relatives have a high regard for the management team and staff in the Home and said they feel they could discuss anything with them. The management team office is at the entrance foyer to the Home and they operate an open door policy therefore if any issues are raised they can be dealt with speedily. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 30 Staff morale is good in this Home therefore positive working practices are in place to improve resident’s lives. The improvement in staff training records means that individual training needs can be quickly identified therefore training can be organised to ensure the workforce is appropriately trained to meet the needs of the residents. EVIDENCE: Staff training needs are discussed during supervision sessions. The training records are well documented with details of when training has been completed and when refresher training is required. The manager stated that all care staff, except one new staff member have completed NVQ Level 2 in Care training, including two domestic staff. Two staff are undertaking NVQ level 3 training and three are doing the Registered Managers Award course. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 There are good management arrangements in the Home. Staff can approach the management team for advice knowing they will be supported therefore residents live in a relaxed and well-run environment. EVIDENCE: The Registered Manager has nearly completed the Registered Manager Award. The assistant managers are encouraged to take part in the inspection process and were present throughout the day. They are also undertaking the Registered Manager Award and become involved in implementing administrative procedures in the Home, for example, supervision with staff. Staff freely came to the office to ask for information and advice and the management team were seen going about the Home, speaking to residents, staff and visitors.
