Conditions of registration: N/A Date of last inspection 5 January 2005 Brief Description of the Service: Gingercroft is a large detached house that has been extended to accommodate 14 elderly people. The home is situated in the Village of Gnosall, there are shops, public houses, church and Health Centre. Service users accommodation is located on the ground and first floor of the home offering both single and shared bedrooms, the first floor being accessed via a passenger shaft lift, stair chair lift or staircase. Communal areas consist of a very pleasant dining room and two lounges. Bathrooms and toilets are sited on both floors and offer assisted facilities. Externally there are gardens to the side and rear of the home with limited off the road parking. Work is to start on a new patio area for residents with seating and umbrellas. The General Practitioners who are based at the Village Health Centre opposite the home own Gingercroft. Gingercroft E09 E51 S4946 Gingercroft V193705 070605 Stage 4.doc Version 1.30 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This inspection was carried out over half a day on Tuesday 7th June 2005. Prior to the inspection the inspecting officer received responses to five comments cards three from residents and two from relatives. Comments made by both residents and relatives were of a positive nature and provided written evidence that residents who had responded were satisfied with the manner in which services are provided to them. The Care Manager provided written information regarding staffing, staff training, menu and dietary provision that was observed by the inspector to be in place at the home. On arrival at Gingercroft individuals had completed breakfast and were in the lounge areas or their bedrooms. Inspector was greeted by residents and staff in a friendly manner and invited into lounge where pleasantries were exchanged. Individuals were deciding on their daily routines reading daily newspapers and doing crosswords. Resident’s accommodation is located on the ground and first floor observations of the inspector were that the home was clean and maintained to a high standard. Resident’s bedrooms were personalised as individuals wished with familiar items they had brought to Gingercroft with them. Gingercroft has in place a statement of purpose and service users guide to inform residents and their relatives of the services and facilities the home offers to provide. All service users have a full assessment of their needs carried out prior to admission to the home to ensure that staff and the home can meet the assessed needs. Residents/relatives are informed in writing of the outcome of assessments. Arrangements are in place for meeting the health and personal care needs of residents and details are recorded in care records. There is a regular programme of social opportunities available for those residents who wish to be involved. Residents are provided with a choice of well-balanced and nutritious food with the cook consulting individuals on a daily basis with regard to personal preferences. Residents confirmed this when talking to the inspector.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5. Standard 6 is not applicable to the home. The homes statement of purpose and service users guide provides prospective residents with details of the services the home has to offer enabling an informed decision about admission to be made. EVIDENCE: The homes Statement of Purpose and Service users Guide is given to residents and their relatives clearly describing the services and facilities Gingercroft is able to offer. All residents have a contract of their terms and conditions of residence at the home that is explained/discussed with the resident and their relative. Prior to admission a full assessment of individual needs is carried out to ensure that Gingercroft and its staff are able to meet the assessed needs of the prospective resident. Assessments were seen by Inspector. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. The health, personal and social care needs of individuals is well met with evidence of good multi disciplinary working practices taking place. The systems for the administration of medication are good with clear detailed arrangements being in place to ensure resident’s medication needs are met. EVIDENCE: All residents at Gingercroft had an individual plan of care that is divided into modules relating to specific areas. Risk assessments are carried out and were included in care records in the form of a risk assessment file. All contacts the resident has regarding health and personal care are recorded in care records. Detailed records are in place for the administration of medication all of which is securely stored. Policies are in place to inform staff of systems and procedures. Gingercroft staff access all specialist services at the health centre specialist staff i.e. continence advisor, diabetes nurse, give talks to staff at the home.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. The systems for resident’s consultation in the home are good with a variety of evidence that indicates that resident’s views are sought and acted upon. There was a relaxed and friendly atmosphere with residents moving freely around the home. Contact with the local community is good. The cook at the home provides excellent food with a choice at all meals. EVIDENCE: Residents during conversations with them confirmed that they enjoy the lifestyle at Gingercroft. Small homely environment where residents are involved as they wish in quizzes, bingo, sing-along, dominoes, residents said they were not so keen on outings prefer to remain in the home and do something. Like sitting in the garden in warm weather, have family nights cheese and wine. Visitors come in from the village and there is good contact with relatives. Observations were that residents are encouraged to make choices about their daily routines. The homes menu offered a balanced nutritious diet with a choice at all meals, home baking and fresh produce purchased locally in the village.

The home has a satisfactory complaints system with some evidence that service users feel their views are listened to. EVIDENCE: The home has in place a complaints procedure that has been discussed with residents and their relatives. Residents the inspector talked to said they were aware of the complaints procedure and that they would talk to sue the manager if they had any concerns. Relative’s questionnaire and feedback evidenced knowledge of the complaints procedure. Resident’s legal rights are protected by the involvement of relatives, advocates, and solicitors as appropriate. Residents are protected from abuse by staff awareness and training, and the policies and procedures in place at the home. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,23,26. The standard of the environment within this home is good providing residents with an attractive and homely place to live. EVIDENCE: The home generally is well maintained and suitable for its stated purpose. There is a programme of routine maintenance and renewal of soft furnishings and equipment. Externally there are attractive gardens a new patio area is being created in the shade of the willow tree; tables, chairs, and umbrellas are to be provided. There is a lawn and limited parking to the side of the home. Resident’s bedrooms are comfortable and have been personalised as they wished. A married couple at the home said they were delighted with their accommodation, which is spacious and has sofa, chairs, and many personal items. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 Staff morale was observed to be high with the staff team working well together with residents to improve their quality of life. EVIDENCE: At the time of this visit to Gingercroft observations of staff identified that staff on duty were as the rota and were sufficient in number to meet the observed needs of individuals in their care. Robust recruitment procedures are in place to employ staff Criminal Records Bureau checks are carried out along with POVA checks prior to permanent employment being offered. There is a staff-training programme in place with records being maintained of training carried out. The home promotes NVQ training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35, 38. The manager is well supported by the proprietors in providing clear leadership throughout the home with all staff demonstrating an awareness of their roles and responsibilities. EVIDENCE: The Care Manager is experienced and competent to care for older people and is currently completing her Registered Managers Award. The registered proprietors and care manager ensures so far as is reasonably practicable the health safety and welfare of service users and staff. Resident’s financial interests are safeguarded by the involvement of individuals themselves and their relatives/solicitors the home does not deal with individual finances. Safe working practices are in place and staff had received training in moving and handling, fire safety, first aid, and food hygiene and infection control.
