CSCI found this care home to be providing an Adequate service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 2 statutory requirements (actions the home must comply with) as a result of this inspection.

The home is owned by Surrey Rest Homes Ltd and provides accommodation and care Care Homes for Older People Page 2 of 9 for up to 24 older people, 3 of whom may have dementia and two of whom may have a sensory impairment. The accommodation is arranged over three floors, with a passenger lift and a stair lift available to access the first floor. The second floor is reached by stairs. There are 22 single occupancy rooms, 19 of which have en-suite facilities, and one double room. Those rooms without en-suite facilities are located close to toilets and/or bathrooms. The communal areas include a large lounge leading onto a conservatory, a dining room and another conservatory at the far end of the building. There is a large, enclosed, garden with a patio to the rear of the property and parking for several cars to the front. Care Homes for Older People Page 3 of 9 What we found: The quality rating for this service is 1 star. This means the people who use this service experience adequate quality outcomes. The reason for this visit was to undertake a specialist pharmacist inspection following concerns regarding the handling of medication in the home which have been notified to The Commission for Social Care Inspection. We looked at the medication records, procedures, storage, talked to care staff and the acting manager and watched some medicines being given to people. This home has had some problems in the safe handling of medication. Action is being taken to correct these and systems are being put in place by the acting manager so that any future problems can be quickly identified and dealt with. It extremely important that these plans are put in place and that all staff work to the new systems, so that peoples health and welfare are protected. Some people hold and administer some of their own medicines. Peoples ability to safely hold and administer their medication and any risks to other people living in the home had been assessed. Other people have their medicines given to them by designated trained staff. We watched a senior carer giving medicines to people who live in the home. Medicines were given to them safely and in a way that took into account their specific needs. From the records and supplies of medicines in the home we could see that people get the medicines that they are prescribed. However, whilst the records showed that one person had received their antibiotics at the correct frequency one extra dose had gone from the supplies which could not be accounted for. Another person had received medicines wrongly labelled by the pharmacy and so whilst they were receiving the correct dose the home was recording that they were giving a different strength of the medicine. No detailed procedures for the safe handling of medicines had been available in the home for staff to work to. The new acting manager had brought a copy of medication handling procedures from another home in the group with the intention of introducing them in Glebe House. It is extremely important that staff have access to clear procedures so that they can act appropriately and consistently for the benefit of people who live in the home. Medication was stored both securely and at the correct temperature for the protection of the service users. The medication cupboards and trolley were clean and orderly with the keys being held by the staff. Controlled drugs were stored securely, but not in a cupboard that met the requirements of the Misuse of Drugs (Safe Custody) Regulations 1973. A recent change in the law means that all care homes must now keep all Controlled Drugs in a Controlled Drugs cupboard. The manager was informed of this change. Additional records were kept of the usage of Controlled Drugs so as to readily detect any loss. They may not have been looked at during this inspection, as a random inspection is short and focussed. The registered person must take the necessary action to comply with these requirements within the timescales set. All areas of the home must be free from offensive odours. Unmet from 10/05/07. 05/10/2007 2 26 16(2)(k) 3 26 23(2)(b)(d) The laundry floor is 28/12/2007 unsuitable and arrangements need to be reviewed to ensure an easily washable and impermeable floor covering is in place. 4 29 19 Schedule 2 All persons working in the 28/11/2007 care home must have adequate pre employment checks as set out in Schedule 2 of the Care Homes Regulations 2001 (as amended).
