The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides a safe and comfortable environment for service users and visitors. An excellent range of activities are offered to service users. Information regarding the activities provided and the planned entertainment are on display. The home provides a good variety of choice for service users. The service users are consulted regarding daily menus and the food provided was very well presented and tasty. The care records were very well organised and kept up to date. The care plans reflect service users needs and record any progress or deterioration. The staff were observed going about their daily tasks in a friendly professional manner. Time was spent with service users in and amongst the daily routines.

What has improved since the last inspection?

The care plans have been reviewed since the last inspection. A new care planning system has been implemented. The new care plans provide more detailed information regarding individual care needs. Confirmation is now recorded of the CRB checks completed for the staff team.

To admit named SU under 65 as stated in variation application 19.10.05 16th August 2005 Date of last inspection Brief Description of the Service: Gledhow Christian Care Home is a purpose built property. The home is located in a residential area close to local amenities and public transport routes. There are car parking facilities. There are gardens surrounding the home that are accessible to the residents. The accommodation is on two floors with a passenger lift connecting the two. There are fifty en suite bedrooms. There are several lounge and dining rooms located throughout the home. There are sufficient communal bathrooms and toilets. The inspection was unannounced and took place over the morning and early afternoon of the 23rd February 2006. This was the second inspection for this year. The first inspection was announced and took place in August 2005 and was undertaken by inspector Gill Sangster. Copies of reports for this and previous inspections are available either from the home or on the CSCI website. This was the first ever inspection undertaken at the home by Michael Smithson. The visit focused on any outstanding issues from the last inspection, a spot check of the service user records and staff recruitment files. Time was spent familiarising with the environment and chatting to service users. The inspector also joined service users for lunch. The home offered a warm welcome and made all the information required available. The feedback from service users was very positive and all spoken to felt the home offered a very good standard of care. They were very complimentary about the manager and the staff. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3. Good written information is available to make sure that people can make an informed choice about moving into the home. All service users are assessed before moving in to make sure that their needs can be met. Service users are offered trial visits so that they see if the home is suitable before moving in. EVIDENCE: An up to date statement of purpose and service users guide are available. The documents provide detailed information regarding the facilities offered and the levels of care provided. A separate brochure and introductory pack are also produced for new admissions to the home. All service users are assessed prior to admission. The home has a detailed preadmission assessment format. Two service user records were checked during the inspection. Both had pre-admission assessments completed by the home and separate assessments completed by the placing social worker. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 10. Care plans are very informative and kept up to date. Good arrangements are in place for service users to access health care services as required and good monitoring records are kept. Service users are treated with respect and their privacy is maintained. EVIDENCE: The care records for 2 service users were checked during the inspection. Both contained detailed and up to date information about the service users. A new care planning system has recently been introduced and it includes the recommendation made at the last inspection. The care plans now contain specific details of any care needs. The care plans follow a standard format that is used to identify and special needs. The care plans are regularly updated and formal reviews are held. Where possible the service users and family are involved. Detailed daily reports are kept which also relate to issues identified in the care plans. These include pressure care, nutritional and manual handling assessments. General risk assessments are kept where risk has been identified. The care plans include information regarding treating service users with dignity and respect. During conversations with service users it was evident that this it put into practice by the staff team. They felt that any personal care tasks required are undertaken in a professional manner by the staff team. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 and 15. An excellent range of activities and entertainment is offered. Service users are encouraged by staff to exercise choice and control over their lives. The service users enjoy the food. EVIDENCE: A very good range of activities are provided at the home. These include daily activities and special events such as visiting entertainers and reminiscence therapy. The service users felt there were plenty of activities to help to keep them occupied. They also confirmed they have a choice whether they participate or not. Many service users do like to spend time in their bedrooms. Many have their own entertainment systems including televisions, radios and books. A good selection of newspapers are also available. The inspector joined the service users for lunch. The meal was well presented and was tasty. Service users were offered a choice of menu, which is good practice. Staff were on hand to provide assistance where required. The feedback from service users was very positive. They felt the meals were of a good quality and offered good variety. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 22, 23 and 25. The home provides a comfortable environment that is decorated and furnished to a good standard. Bedrooms are personalised and a good range of mobility aids are provided. EVIDENCE: A full building inspection was not undertaken during this visit. However the inspector did walk around areas of the building to familiarise himself with the layout. Time was also spent in the some service user bedrooms and the communal areas. All the areas seen were maintained to a very good standard of decoration and hygiene. Good quality fixtures and fittings are provided in all areas. A number of communal areas are provided throughout the home. These include lounge/dinning rooms, sun lounges and a private chapel area. Small kitchens are also provided on each of the 4 units. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29. Staffing levels are sufficient to meet the service user needs. Recruitment procedures make sure that staff are suitable before they start work in the home. EVIDENCE: A spot check of the records for 2 staff recently employed was checked. One was a transfer within the organisation. All the recruitment records for the previous employment were available. The second member of staff was a qualified nurse. All the required information was available except for a copy of the nursing qualification. The manager agreed to rectify the error and provide CSCI with a copy of the information when obtained. Confirmation was provided by the manager. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 and 35. The financial interests of service users are protected. Service users and relatives feel they can contribute to the decision making process. EVIDENCE: During conversations with service users it was evident that they felt able to contribute to the decision making process. The manager has set up a number of different channels for service users and relatives to contribute to the day to day organisation of the home. These include regular meetings, individual service user reviews and satisfaction questionnaires. However the managers “open door” policy appears to be the most successful and widely used by both service users and relatives. Personal allowances are held on behalf of a number of service users. Secure storage is provided for the safe keeping of the money deposited. Records are kept of income, expenditure and the balance held.
