Date of last inspection 6th July 2005 Brief Description of the Service: Gracelands is operated by Rosie Hancock and registered as a care home to provide accommodation and personal care to six older people. Its Statement of Purpose purports that enabling residents to live independently with the care and support offered at the home as its aim. Gracelands is located on the Fishponds Road close to shops, amenities and bus routes. It a domestic dwelling converted to provide accommodation and personal care to six older people. There is level access to the front and rear of the property, with a stair lift to access the first floor. Accommodation is arranged over two floors with single occupancy bedrooms on the ground and first floor and shared space on the ground floor. This is an overview of what the inspector found during the inspection. This inspection took place in November 2005 with a second inspector. It is the second inspection for this year and both were conducted on an unannounced basis. Interaction between staff and residents was indirectly observed. Records, policies and procedures were examined to confirm the standards of care at the home. Three residents and one visitor agreed to give feedback on their experience of the provision of care at the home. What the service does well: What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7&9 A Person Centred approach to meeting the need must be developed to underpin key principles of rights, independence choice and inclusion. Reactive strategies must be formulated for people that may exhibit aggressive and violent behaviours. Safe systems for administration, recording and storage of medicines exist at the home. EVIDENCE: Personal information health care documentation and care plans are kept within case records. A photograph, background information and personal histories are incorporated into the personal information format. Medical profiles and other documentation evidences that residents are referred for specialist support. Care plans describe the physical, medical, community and personal needs. Emotional and social care needs are also included within the format. The assistance required by the staff to meet the individual needs is clearly described. While the care plans are specific and guide staff to meet the

Action plans must include the individual’s likes, dislikes and preferred routines. Care plans are monitored monthly by the staff and where necessary altered. Reviews take place six monthly with the resident, the staff and family participating. For residents that may become aggressive, the actions to diffuse the situation must be described. For example, moving away if the person displays violent behaviour. Reports of significant events detail outcomes of visits, daily activities and the provision of care. The records are factual and objective. Medications that staff administer are through a monitored dosage system. The records of administration indicate that staff sign the records immediately after administration. A record of medications no longer required is kept and countersigned by the pharmacist to indicate receipt of the medication for disposal. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 14 Opportunities exist for residents to make choices about leisure activities, daily routines, meals and relationships. EVIDENCE: Three residents were consulted on the standards of care at the home. Feedback on their social and leisure activities was sought. One resident participates in community-based activities while the other two residents stay at home during the day. Watching the television, reading, socialising with each other and going to the shops were the daytime activities described by two residents. Regarding organised in-house activities, the residents stated that because of their impairments they are no longer able to pursue hobbies. In terms of opportunities to exercise choice, residents reported that the food was very good and varied. It was understood that during their admission to the home, their food preferences are sought and therefore alternatives are not required. Additional comments were made about the arrangements of meals. It was felt that because breakfast in served in bedrooms, there is plenty of time in the morning for a leisurely rise, indicating that routines are based on residents needs.

One visitor at the home corroborated residents’ comments that visiting can take place at any time. The visitor made additional complimentary comments about the standards of care and felt the environment was more of a “family home”. Comments made by residents indicated that the staff respects their rights. Their mail is handed to them unopened; members of staff knock and wait for an invitation to enter their bedrooms and personal care in conducted in private. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Residents’ views are sought and felt confident that their views would be taken seriously and acted upon. EVIDENCE: There were no complaints received by residents for investigation at the home since the last inspection. Residents reported that they had no complaints and would approach members of staff or the service provider with complaints. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22 & 24 Gracelands is suited for the purpose of providing accommodation to people over 65. It has a homely atmosphere and is well maintained. Communal areas provide residents with sufficient shared space to socialise as a group. The number of toilets, washing and bathing facilities provided meet the needs of the residents. Equipment and aids are provided to assist less mobile residents with moving around the home. The premises are kept clean and free from unpleasant odours. EVIDENCE: Gracelands is located on the Fishponds Road close to shops, amenities and bus routes. The property is within a residential environment and maintains an appearance of a domestic dwelling. It is arranged over two floors with bedrooms on both floors and shared space on the ground floor. The premises were clean, decorated to a good standard and free from unpleasant odours during this inspection.

The lounge has seating for six residents and in the dining room there is sufficient dining space for residents to eat their meals together. Within the dining room there is additional seating for five residents. There is a full bathroom on the ground floor and another full bathroom on the first floor, which is assisted, with a separate toilet adjacent to the bathroom. Currently the ratio for bathrooms is three people sharing which is above the National Minimum Standard. There is a toilet on each bedroom floor and two bedrooms are en-suite, making the ratio of two people sharing a toilet. There is level access and one step to enter the home. The stair lift provides access to the first floor and assists residents with moving around the home independently. The residents currently accommodated move around the home with Zimmer frames and walking sticks. Bedrooms are single and two are en-suite and contain a combination of the home’s furniture and personal belongings. As bedrooms are lockable and staff knock before entering bedroom, personal space is private. It was confirmed that keys to bedrooms and the front door are provided. One resident commented that the accommodation was the primary reason for moving to the home permanently. From observation of the premises, the property is well maintained and comfortable. The laundry is sited away from the laundry. There is a domestic washing machine and hand washing facilities. Gracelands DS0000026506.V263775.R01.S.doc Version 5.0 Page 15 Staffing

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 Application forms and references must form part of the robust recruitment process of the home, to ensure the staff are suitable to work with vulnerable adults. EVIDENCE: The standard application form requests personal information, employment and educational background. A statement regarding physical fitness is also sought through the application form. Full employment history must be required with the reasons for leaving, the names of two referees must be provided and one must be the most recent/current employer. The application form must also require the person to disclose any criminal background to establish integrity. Within the application form, the job description is included which defines the purpose and responsibilities of the role. The expectation that staff participate in training must be specified in the job descriptions. Standard request for reference formats should elicit information about the length of employment, capacity known and would re-employment be possible if requested. Additional details about the reasons for leaving their employment, any disciplinary processes and any other information that establishes their suitability to work with vulnerable adults. Employees must be provided with GSCC Code of Conduct and evidence of receipt must be kept in their personal files. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 Insurance cover and fire checks and practices indicate that residents’ safety and interests are safeguarded. EVIDENCE: Insurance certificate for employer’ liability is up to date and displayed at the home. The records that relate to fire safety policies, procedures, checks and practices were examined. It is evident from the records that checks and practices are conducted at the stipulated frequencies. For residents placed by the Local Authority, individual schedules that denote the sources, which contribute to the fees, are in place. The arrangement for handing personal allowances to residents was discussed with the service provider. Currently residents receive direct payments for their benefits and the service provider returns their entitlement each week. A record of personal allowances handed to residents is currently maintained. It is signed by the resident to indicate receipt of their personal allowances.
