This unannouced inspection was carried out following concerns raised by the local social services. It lasted approximately two hours and was facilitated by the registered manager and provider. We mainly looked at residents care plans, risk assessements and the general record keeping in the home. Four residents care plans were sampled at random and it was noted they generally included information necessary to deliver the residents care but did not cover all the residents needs. For example two of the residents suffer from diabetes, these were not covered in their care plans. This was discussed in depth with the registered manager and also with the provider. Residents care plans must include all aspects of their health, personal and social care needs to ensure that all their needs are being met. Although care staff and professionals write the care plans, this could be improved, as there was little evidence that the residents and their family contributed or were actively involve in the process. The care plan should sets out in detail the action which needs to be taken by care staff to ensure that all aspects of the health, personal and social care needs of the resident are met. The care plans must also meet relevant clinical guidelines produced by the relevant professional bodies concerned with the care of older people, and includes a full risk assessment, with particular attention to prevention of falls. Some care plans, which were sampled, had not been reviewed and updated on a monthly basis. The home must ensure that residents plans are reviewed at least once a month, updated to reflect changing needs and current objectives for health and personal care, and actioned. It was identified during the inspection that the daily records were sometimes brief and did not give enough information on the changes in residents needs. Daily records are a good source of evidence to show that care is being provided, as detailed in the care plan. When well written they help to ensure a consistent approach and good quality of care for residents. Changes in people individual health care needs must be made aware to all staff and recorded to ensure they receive support and care as needed.
