Conditions of registration: 1. 2. 3. The maximum number of service users to be accommodated is sixteen (16). Service users must be older people aged sixty-five (65) or over on admission. Service users with a dementia type illness only to be accommodated. Date of last inspection 29th July 2002 Brief Description of the Service: Greenbank is located close to Bexhill-on-Sea old town and about one mile from the seafront. There is access to local shops, public transport, churches and other community services. Greenbank is a long established family owned home registered to accommodate up to 16 older people with a Dementia type illness. The property was originally a two storey Victorian house, which has since been extended to accommodate residents on the ground floor of the building. There are fifteen bedrooms, one of which may be used as a shared occupancy room but is currently used as single occupancy. All bedrooms have en-suite facilities and there are toilets and bathroom/shower rooms located throughout the premises. There are two large lounge areas, one of which is utilised as a quiet lounge. The home has a good sized dining area, which overlooks the homes secure fishpond. There is a large well maintained garden to the rear of the property with has easy access for residents. Potential new residents can obtain information relating to the home via Care Managers, Placing Authorities by word of mouth and by contacting the home direct. The range of fees charged (at the time of this report) are £400 - £450 per week, with extra charged for hairdressing, chiropody, newspapers and toiletries. This is an overview of what the inspector found during the inspection. This unannounced inspection was conducted over a period of four hours and the registered provider was present along with the deputy manager. The home had provided the CSCI with a completed AQAA (Annual quality assurance assessment) prior to the inspection. A tour of the building was carried out and a variety of records and documents were examined. The inspector held discussions with management, members of staff, residents and visiting relatives. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. Residents can be assured that the home can meet their needs prior to admission. Residents are provided with an individual written contract of terms and conditions with the home. EVIDENCE: Care plans examined evidence that in addition to a copy of a care management assessment from the placing authority, the home conducts their own pre-admission assessment for information about medical, social and personal care needs. These pre-admission assessments are carried out by the home’s registered manager or the deputy. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ health, personal and social care needs are met and are recorded in their individual care plans. Residents are protected by the home’s policy and procedure for dealing with medication. EVIDENCE: A sample of care plans that were examined were comprehensive in covering all aspects of residents’ health, personal and social care needs with evidence of input from resident/relative/representative. The registered provider confirmed that these plans are now reviewed on a monthly basis. Records viewed evidenced referral to other healthcare professionals when required. Residents spoken with were complimentary about the care and support received from carers. The procedures for monitoring and recording of medication are reviewed periodically by the local pharmacist. Medication is stored appropriately and only administered by competent trained staff members. Records viewed were legible and complete with a secondary check system. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The lifestyle experienced by residents matches their preferences and satisfies their social, religious and recreational needs. Residents are supported in maintaining friendships and family contact and they enjoy a wholesome and varied diet according to their choosing. EVIDENCE: Activities are arranged within the home and within the local community, residents are encouraged to participate but are able to decline the opportunities if they wish. Examples of activities that take place in the home are word association, scrabble, mild mobility exercises to music and reminiscence sessions on a 1 – 1 basis. A professional external activity coordinator visits every Wednesday. The home has two lounges where one has a television and the other provides a quieter environment to meet residents’ preferences.
Green Bank EMI Care Home DS0000021120.V340304.R01.S.doc Version 5.2 Page 13 A “Care at Home” agency comes into the home and takes residents out for meals, shopping trips, walks into town etc. There is an open policy for relatives to visit and residents can entertain guests in communal areas or in their own rooms. Many of the relatives take residents out for the day. Two sets of relatives were visiting at the time of this visit and spoke positively about the care their relative received and the facilities available. A visiting priest visits and arrangements are in place for residents to receive non-denominational or Roman Catholic communion if that is their wish. The home has a qualified cook and menus are devised on a weekly basis and able residents are involved in making choices. Menus viewed were varied and healthy and all meals are home cooked with an alternative and vegetarian options. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents and relatives know their complaints and concerns will be listened to, acted upon and responded to. Residents are protected from abuse and neglect. EVIDENCE: The home has an established complaints procedure in place and the provider states there have been no complaints since the last inspection. The home keeps a written record of complaints along with the responses to the complaint. Relatives spoken with were familiar with procedures and state that they would have no difficulties in raising issues with staff and management of the home. The deputy manager has a “Train the Trainer” qualification and ensures staff members receive refresher training around complaints and the protection of vulnerable adults. All of the staff team have received training around adult abuse issues and those spoken with were familiar with reporting procedures. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,23,24,25,26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents live in a safe, clean and hygienic environment. They have comfortable bedrooms and there are sufficient toilets, bathrooms and washing facilities to meet their needs. Residents have access to safe and comfortable indoor and outdoor communal facilities. EVIDENCE: The home has recently redecorated six rooms and replaced carpeting on the lower floor. A number of bedrooms are fitted with pressure pads to alert night staff if residents are on the move.

Bedrooms are comfortable and most have the personal possessions of occupants within them reflecting individual interests and lifestyles. There are adequate toilets and washing facilities that are accessible to all residents and hand washing equipment has been provided as required by the previous inspection. Specialist equipment is provided to maximise the independence of those residents who require it. Hazardous areas are kept secure to prevent injury to residents who may not recognise the dangers. The home has an infection control policy in place and a waste management contract. Records indicate that all staff have recently received training around infection control. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are cared for by sufficient numbers of competent staff to meet their needs. Residents are protected by the home’s policies and procedures in recruiting staff. EVIDENCE: The home has robust recruitment procedures in place including obtaining two references and appropriate CRB and POVA checks. Where appropriate the home has made necessary checks on foreign workers in line with Home Office guidelines as evidenced by documents on file. There were sufficient numbers of staff on duty to meet the needs of residents at the time of this visit. The manager states that the staff complement daily is a cook, cleaning staff, laundry staff, support staff, the manager and deputy manager. The manager states that some NVQs are near completion and this will achieve the target of 50 qualified staff in the home. Staff members spoken with feel they have the training opportunities to ensure they have the necessary skills and knowledge to competently fulfil their roles. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The home is well run and residents benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of residents and their health, safety and welfare is promoted and protected. EVIDENCE: Green Bank EMI Care Home DS0000021120.V340304.R01.S.doc Version 5.2 Page 20 The registered provider is now conducting monthly monitoring visits as part of quality assurance and has addressed concerns raised at the previous inspection. New carpet has been fitted in areas of concern and rugs creating hazards removed. The home has implemented a maintenance development plan for improvements and so far six rooms have been redecorated, carpet renewed, and access to hazardous areas secured. Health and safety records show that gas inspection has been conducted and portable appliance testing carried out. Wheelchairs have appropriate footplates fitted. Records viewed evidence that safety checks are conducted on food hygiene, fire equipment and water. Policies and procedures were reviewed in June 06 and the manager states they are to be reviewed in July 07 and that they are now compatible with the staff handbook. The home conducts annual quality assurance surveys and the registered provider states that a notice board is about to be fitted and will display the findings along with other information about the home. There is evidence of two monthly supervision meetings with staff members and staff spoken with confirmed this takes place. Staff members spoken with state that the management team is open and approachable and observations demonstrated a good working relationship.
