Conditions of registration: 1. The home may from time to time admit persons from the ages of sixty years to sixty five years of age 2. 17 Intermediate Care beds aged 50 years and over Date of last inspection N/A Brief Description of the Service: Grove House is part of the Emmanuel Care Centre located in a residential area in Birkenhead, Wirral. The home is registered to provide transitional care for up to 46 persons and intermediate care for up to 17 persons. All service users are accommodated in single bedrooms, each having en-suite facilities. A number of lounges and seating areas are provided to enable service users to choose where they spend their day. Service users accommodated for intermediate care have a greater input from Occupational and Physiotherapists to provide them with the skills to enable them to return home following hospitalisation and are accommodated at the home for a period of up to six weeks. The ground floor provides for 17 persons requiring Intermediate Care. The first floor provides for 23 persons who have dementia and are awaiting a place in a care home of their choosing, and the second floor provides for 23 elderly persons who are awaiting a place in a care home of their choosing. Service users accommodated for transitional care are usually accommodated for up to eight weeks. This is an overview of what the inspector found during the inspection. This is the first inspection of the home since it was registered with CSCI. The inspection was conducted over a period of seven hours and all standards were assessed. A high number of service users, relatives and staff were spoken to, to obtain a broad view of the service provided. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4, 5, 6 The homes Statement of Purpose and Service User Guide are good and provide prospective service users and their families with sufficient information to enable them to make an informed decision regarding admission to the home. EVIDENCE: A statement of purpose and a service user guide were submitted to CSCI as part of the registration process. These documents contain full information regarding the care provided at the home and the facilities available. All service users accommodated are funded by the Local Authority who issues the contracts. Assessments are undertaken on service users prior to their admission to ensure that the home can meet their identified needs. These assessments are made by the Social Workers, the Long Term Care team and one of the senior members of staff from the home. The service users and their families are also involved with the assessment and their views are taken into consideration when a place is being sought. The assessments for those persons who have dementia are not sufficiently detailed in relation to the service users mental

The information given by Social Workers for those persons accommodated for intermediate care should also include the expected outcome from the intense intervention given by the home. The manager stated that she would discuss this with the relevant personnel. The daily reports prepared by the staff are detailed and give full information regarding the care given to each individual. Service users and their family are encouraged to visit the home prior to admission to view the rooms available and to meet with staff. Some of the facilities within the home have been provided by the PCT. This includes the equipment within the physiotherapy room. The home is still awaiting the provision of a cooker and a domestic size washing machine as these items are necessary for the staff to give training to service users and to enable risk assessments to be undertaken prior to the service user returning home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10, 11 The manager is continuing to review the documentation within the home to ensure that the service users individual needs are identified and met and so benefit the service users. EVIDENCE: Individual care plans are prepared for each service user. The manager regularly reviews the care plan format and content and is looking to make improvements in the current format as part of the care plan audit. Risk assessments are undertaken on all service users and risk management strategies are put in place to remove or reduce all risks. None of the service users accommodated at the home have developed pressures sores following their admission, however, a number of service users have been admitted with this problem. The advice of the Tissue Viability Nurse is sought to ensure that appropriate treatment is provided at the earliest opportunity. The medications are generally dealt with as required, although staff should ensure that hand written entries on the Medication Administration Record

Problems have been encountered regarding the ordering of medications due to the different admission dates of the service users. The manager is currently liaising with the GP’s and the pharmacist to overcome this difficulty. The home has a policy and procedure for the administration of medications and this was seen to have been followed by the staff. All service users are provided with a single bedroom, each having en-suite facilities. Personal care is given to service users in their bedroom or in the bathroom as appropriate to ensure their privacy and dignity. Service users may meet with their visitors in the privacy of their bedroom or in one of the communal areas as they wish. The home has a policy and a procedure to be followed in the event of the death of a service user. Information is held in the home of ministers who can be contacted whenever necessary. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 Dietary needs of service users are well catered for with a balanced and varied selection of food available that meets service users tastes and choices. EVIDENCE: The routines within the home are flexible to suit the service users preferences. All preferences are detailed within the care plans. Those service users accommodated for rehabilitation have a more structured day to work around the routines of the occupational and physiotherapy professionals. Visiting is available at any time and relatives and friends are encouraged to visit. The home provides short-term care for the service users and so it is not feasible for new links within the community to be developed, however, maintaining existing links is encouraged. Some family members take service users out during the day. Choices are offered within the home and these are based on individual preferences and abilities. All preferences are recorded in the plan of care. The home offers a four-week rotating menu that offers a selection of meals at each mealtime. Service users are encouraged to take their meal in the attractive dining rooms to promote social interaction, but can be served their meal in the privacy of their bedroom if they wish. Special diets can be provided on the advice or recommendation of the GP or dietician. The meal served on the day of the inspection looked and smelled appetising. Meals are prepared in the main kitchen. The home employs dedicated catering staff and is currently advertising for a kitchen assistant. The kitchen was clean and well organised. Some additional kitchen equipment had been ordered and its delivery is awaited. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17,18 Appropriate checks are made on staff prior to them commencing work at the home to ensure the protection of service users. EVIDENCE: The home has a detailed complaints procedure and information regarding this is documented in the Service User Guide and is also displayed on the notice board within the home. The home does not deal with any legal issues on behalf of service users as they are accommodated at the home for short-term care only. The home has an Adult Protection procedure, including a whistle blowing policy. Some training has been given to staff on adult protection issues although it is recommended that a more comprehensive training opportunity is made available to staff on the different types of abuse and the action to be taken in the event of it being suspected. All staff are fully vetted prior to working at the home which includes Criminal Record Bureau and Protection of Vulnerable Adults checks. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25, 26 The quality of the building and facilities within the home are high creating a safe and comfortable environment for the service users. EVIDENCE: Grove House is a purpose built care home located within the Emmanuel Care Home Centre. It is located close to shops, parks and is accessible by public transport. Car parking areas are provided. The home is on three floors with all areas being accessible by a passenger lift. All service users are provided with a single bedroom, each having en-suite facilities. Bedrooms are decorated and furnished to a high standard. A selection of communal areas is provided and these too, are decorated and furnished to a high standard. The home provides sufficient sitting and dining space for the number of persons to be accommodated. Additional toilets and baths are provided together with an additional shower room. All areas of the home were found to be clean and there were no offensive odours. The occupational therapists have been involved in the assessment of the premises to ensure that it provides the appropriate facilities to meet the individual needs of service users. Each service user accommodated for intermediate care is individually assessed to ensure that the appropriate aids and adaptations are provided to assist them in their rehabilitation. The home is central heated and all rooms have windows that can be opened to provide natural light and ventilation. Thermostatic valves have been fitted to all hot water outlets to ensure that the service users are not put at risk of scalding. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30 The standard of recruitment and vetting of staff is good to ensure the protection of the service users. EVIDENCE: The home employs qualified nurses and care staff to provide for the service users care needs. There is always a minimum of three qualified nurses on duty, with four during the morning period up to 2pm. Sufficient care staff are employed and deployed to meet the assessed needs of the service users. The home is promoting NVQ training amongst the staff and this training continues. Staff spoken to during the inspection confirmed that they were given training relevant to the work that they did and to meet the assessed needs of the service users. The home has a robust recruitment procedure, based on equal opportunities, with all prospective staff being required to complete an application form before attending for interview. Two references are taken and all staff have CRB and POVA checks made on them prior to commencing work at the home. Qualifications are also checked. A selection of staff files were inspected and were found to contain all relevant documentation as required. New staff complete an induction training programme. staff files of all training given.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 34, 35, 36, 37, 38 EVIDENCE: The manager is a qualified nurse and an experienced manager. She has achieved an NVQ level 4 in management and could evidence details of ongoing training. She has an open door policy and is available to service users and families each day. Staff meetings are held on a regular basis and minutes of these meetings are held. A quality monitoring system is currently being established. Questionnaires have been prepared and it is hoped to issue these over the next few months. The home does not deal with any of the service users financial matters. These are dealt with by family members or advocates.

Evidence of this training is held on staff files. A staff supervision programme is currently being established and all staff will be required to have an annual appraisal. Risk assessments have been carried out on the premises and these will be reviewed on a regular basis. Safety certificates inspected were found to be well maintained and up to date. Equipment for the detection of fire is tested regularly and these tests are duly recorded. Health and safety issues are the responsibility of the Manager, the handyman and the Registered Person. Evidence that health and safety issues are addressed is held in the home. Monthly reports are prepared by the Registered Person and copies of these are held in the home.
