This inspection was carried out on 16th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This is a care home where residents are well looked after. The staff team work well together and show a good understanding of the needs of the people living at the home. Those residents spoken to said they liked living at the home and felt they were being well cared for by the staff. One resident said, " I am very lucky. I get on very well with the staff who are very caring and patient when attending to my needs. I have no complaints and cannot find any faults with the home". Another resident said they had lived at the home for a number of years and couldn`t be happier. The resident said, " From the day I moved into the home I have always found the staff to be hard working, friendly and very caring. Nothing is too much trouble for them". The home is well maintained and was clean and tidy. Staffing levels were sufficient for the number of residents living at the home. Staff members seen said they were happy with their workload and were well supported by the manager of the home.

What has improved since the last inspection?

Procedures for the administration of medication have been reviewed and updated to ensure these are safe and residents are protected. The homes recruitment procedures have been reviewed to ensure records show clearly when references and police clearances have been applied for and received.

What the care home could do better:

Some furnishings in the lounge and dining room areas are showing signs of age and if replaced would improve the overall appearance of the home.

The places designated for (TI) may also be utilised by persons within the category OP providing the total number of persons accommodated in the home does not exceed 40. 5th September 2005 Date of last inspection Brief Description of the Service: Guys Court is a purpose built home located in a residential area of Fleetwood. The home is one of a number owned by European Care (UK) Ltd and is registered under the Care Standards Act 2000 to care for forty older persons requiring nursing or residential care. Five of these beds are registered to care for residents with a terminal illness and a qualified nurse is employed at the home who has the appropriate training. The home has three floors comprising of bedrooms, toilet and bathing facilities. Resident accommodation comprises of thirty-two single rooms and four double rooms. Ten bedrooms have en-suite facilities. The home has a passenger lift to facilitate easy access between the three floors. Lounge and dining areas are located on the ground floor. Regular social activities are organised within the home. This inspection was unannounced and started at 9.30am and took place over 3 hours. The Inspector spoke to three staff members, three residents and the manager. Staff and care records were also examined. A full tour of the premises was undertaken with the manager. What the service does well: What has improved since the last inspection? Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Residents feel respected by the staff team and their right to privacy is upheld. EVIDENCE: Residents spoken to said they liked living at the home and were well treated by staff members. One resident said, “This is a wonderful home. The staff are great and I am well treated”. Another resident said they enjoyed the company of other residents but also liked to spend time on their own reading. The resident said, “ I come down to the garden room every day after breakfast but like to return to my own room after lunch. The staff are very good and know when I want to be my on my own”. Three residents said the staff team respected their privacy and they could spend time on their own if that was their wish. Staff members spoken to had a good understanding of the needs of residents and felt they were able to respect the rights of residents at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 The homes procedures enable residents to exercise choice and control over their lives. EVIDENCE: Residents spoken to said routines within the home were flexible and they were able to make their own decisions about how to live their lives. One resident said, “I have lived at the home for a number of years and I am very happy and settled. The staff are very caring and give me all the support I need. Another resident said the staff were very respectful and allowed them to make my own decisions”. Most residents at the home handle their own financial affairs or these are handled by their relatives/representatives. Records being kept by the home in respect of residents unable to manage their own finances were being well maintained. A tour of the premises confirmed residents had been allowed to bring their own personal possessions with them on admission to the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 22, 23, 24, 25 and 26 The home has a planned maintenance and renewal programme for the redecoration and refurbishment of the home to ensure residents live in a comfortable, homely and safe environment. EVIDENCE: Lounge and dining areas have been decorated and furnished for the comfort of residents. However, the Inspector did observe some of the furnishings are old and if replaced would improve the appearance of the home. One resident sitting in the garden room said, “ This used to be the best room in the home. It was always so pleasant and comfortable but it’s starting to look a bit neglected now”. All residents spoken to said they were happy with their rooms and were provided with the choice of spending time on their own or in the lounge area’s. Bedrooms have been furnished to ensure the comfort of residents. En-suite facilities are available in 10 rooms. Toilet and bathing facilities are located on all floors and are easily accessible for residents. The home has a passenger lift to provide access between the three floors. Radiators throughout the home are guarded or have guaranteed low temperature surfaces to protect residents from the risk of burning. Hot water temperatures throughout the home were checked and found to deliver water at a safe temperature in line with health and safety guidelines. It was observed during the visit the home was clean and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): 27 The deployment of staff throughout the day is sufficient to meet the needs of residents EVIDENCE: Staffing levels were sufficient for the number of residents living at the home. Residents said they were happy with the care they receive from the home and were well treated by the staff. One resident said, “ This really is a wonderful home. There is always plenty of staff on duty and they are always there when you need them”. Observation of care practices throughout the day confirmed residents are treated with respect and dignity. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31 and 35 The home is well managed and run in the best interests of residents. EVIDENCE: The manager of the home is a Registered General Nurse and has achieved a relevant management qualification. She is well supported by the owners of the home who visit regularly. Records seen confirmed the manager has access to training to ensure her knowledge and skills are updated. Inspection of records for resident’s finances were well maintained and up to date.
