This inspection was carried out on 22nd February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Haighfield is a good care home where staff work hard to provide the type of care that the residents need or expect. Residents are given a choice in all aspects of their daily lives, and are encouraged to remain active and independent. Help is at hand from staff where it is needed, and access to health care services is arranged, as it becomes necessary. Staff are attentive and cheerful. Communication with families, friends, and other professional agencies is good.

The carpets on the corridor, near the lounge area are stained and in need of replacing.

The Home is registered for a maximum of 45 service users to include:up to 45 service users in the category of OP (Older People) The service should employ a suitably qualified and experienced Manager who is registered with the Commission for Social Care Inspection. 11th February 2005 Date of last inspection Brief Description of the Service: Haighfield Care Home offers nursing and personal care and support for up to 45 service users over the age of 65 years. Haighfield is situated on the main road between Wigan and Standish. Local amenities are a short drive away form the home. The home is a four storey, purpose built home, with a purpose built extension. The home offers 35 single rooms and five shared rooms, some rooms have en suite facilities, however all rooms have a hand basin and bathrooms and toilets are in close proximity to service users accommodation and communal areas. The home offers two small patio areas at the rear of the home but outside garden space is limited. The external areas are well maintained and the grounds are well presented. This inspection was carried out over a 4 -hour period on one day, and was unannounced. In the absence of the manager, the nurse in charge and the senior carer assisted with the inspection. The first part of the morning was spent talking with staff and residents and watching how staff carried out the morning’s duties. The rest of the time was spent looking at records the home keeps on residents (care plans ) and other records the home needs to keep to run its business. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Standard 6 does not apply at Haighfield as the home does not provide intermediate care. The admission procedure ensures that all people receive a proper assessment prior to moving in to the home in order to ensure that their care needs have been fully identified and that the home is capable of meeting those identified needs. EVIDENCE: The admission procedure is sufficiently detailed to guide staff on the actions to be taken to ensure that all new residents needs are properly assessed and planned for. All prospective residents are assessed prior to admission, at a place that is best for the resident, either at their home or in hospital. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 The standard of care planning and review was good, providing staff with the information they need to meet the resident’s care needs. EVIDENCE: Individual care plans were made available for inspection. Three care plans were looked at and indicated that all aspects of resident’s health, personal and social care needs were planned for. The care plans were clear and easy to read and had been updated monthly or more frequently if required. There was no evidence to demonstrate that residents or their family had been involved in the drawing up and in the maintaining of the care plan, however one relative spoken with was aware that care plans were kept. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The routines of daily living are flexible enough so that the different needs, expectations and preferences of residents are met. Residents are assisted to maintain contact with friends and family. Residents are kept informed and are able to maintain a good level of independence, and to exercise choice and control over their lives. Meals are good, and served in quantities suitable for residents, and offer a generous amount of choice and variety. EVIDENCE: It was clear from observations, from records, and from conversations with residents that they are offered real choices about how they live their lives. Residents spoke about the activities planned for example, entertainers that were booked to come to the home; they enjoyed playing bingo and dominoes and arts and craft sessions. The home enjoys a good relationship with the local school and some pupils visit the home. Residents are able to part in religious services carried out in the home by visiting clergy and on the day on the inspection, the vicar was offering communion to residents. Residents were seen to have access to a range of reading materials, including books and newspapers. Residents said they had been able to bring in some of their own processions, sufficient to make their bedrooms feel more personal. Residents were complementary about the meals that were provided. They said the food was good, that it was nicely cooked and presented. It was noted that some residents were on a pureed diet; the food was blended in separate portions to allow residents to savour different tastes and textures. Staff were available to assist any resident who required help, this was seen to be done in a discreet and sensitive manner. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 24 The standard of the environment within the home is good, providing residents with a comfortable and homely place to live. EVIDENCE: From a tour of the building, it was seen that the premises were being maintained to a good standard both inside and out. It was noted that there was a problem with one of the windows in the lounge area; this was attended to during the inspection. The carpet in the corridor is marked and stained, this requires attention. Several bedrooms were inspected and were seen to be warm and comfortable. Residents had brought in personal items, small pieces of furniture, photographs and mementoes. JUDGEMENT – we looked at outcomes for the following standard(s): 38 Procedures and practices operating within the home promoted and safeguard the health, safety and welfare of the people living and working in the home. EVIDENCE: Health and safety issues were good with regular maintenance checks of equipment being undertaken. All accidents and incidents were being correctly recorded and reported. Certificates for the gas, lift, fire equipment, electrics and portable appliances were available for inspection and were found to current and up to date.
