This inspection was carried out on 26th May 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Half Acre has rooms to view that are available for prospective service users and their families to see. In these rooms there is a copy of the statement of purpose and the service user guide as well as a welcome gift set of toiletries. Half Acre provides a safe, clean, comfortable and homely environment for the people living there in respect of bedrooms for service users. A good rapport was noted between the service users and the staff team.

What has improved since the last inspection?

That an assessment by either a social worker or the registered manager prior to the service user being admitted to Half Acre, had been undertaken. There is an activities organiser at the home, who provides a range of activities for service users to take part in if they want to. The number of staff at the home has increased. An induction training system has been introduced. Staff members are now able to access NVQ training and this is ongoing. Records are generally better maintained.

What the care home could do better:

The last inspection was an unannounced inspection and took place on 15th September 2004, at which 30 requirements were made. An additional visit was made to follow up outstanding requirements on 10th December 2004 where 16 out of the 30 requirements remained unmet. At this announced inspection 5 of these requirements remain unmet. It is the inspector`s opinion that the staff members at the Home have done all that they can in respect of the above requirements. The outstanding items have cost implications that are the responsibility of the organisation. The Commission is concerned that these areas have not progressed further despite agreement that the work is needed and this is to the detriment of service users at Half Acre in terms of health, safety and comfort. The following areas must be addressed. Provide adequate and usable bathing facilities for service users to meet their needs, preferences and comfort. Increase the communal space available to service users in the combined lounge/dining room on the ground floor by building the planned conservatory. This will then provide service users with a safer day-to-day living environment where people are able to move about safely, without tripping over other people and equipment that allows privacy when conversations take place and allows service users to watch the television without constantly being disturbed. Ensure compliance with health and safety legislation in terms of fire safety requirements, gas safety and carry out a Legionella risk assessment for the present bathing and showering arrangements. Look at the arrangements in the dining area and the menus to ensure that there is a wide and varied selection of food available to service users and a choice. The organisation must make every effort to obtain the registered managers certificate to evidence that she has completed the Registered Managers Award.

The service should employ a suitably qualified and experienced Manager who is registered with the Commission for Social Care Inspection. Date of last inspection 15th September 2004 Brief Description of the Service: Half Acre is a care home providing personal care only and accommodation for up to 32 older people. Highfield Care is part of the NHP PLC GROUP. Half Acre is located approximately 1 mile from the centre of Radcliffe and 3 miles from Bury. It is on a bus route and has a number of shops and a pub nearby.The premises are purpose built on three floors with a passenger lift.Accommodation is provided in single rooms with en-suite facilities, with three former double rooms available to share should service users require this. At this announced inspection the inspector looked round the building, watched the social interaction between staff members and service users and practice issues. Talked to the regional manager, the registered manager, staff members and service users. Looked at documents and records of four service users, health and safety documents and relevant the policies and procedures. A second visit was made to Half Acre on 26th May 2005 to speak to more service users and to see if any of the work that had been asked for before had been done. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 2 3 4 5; not applicable in this case. 6 is Appropriate assessments had been undertaken before service users’ moved into Half Acre by suitably qualified people. This is an improvement for service users moving into the home and ensures that their needs can be met. EVIDENCE: There is a statement of purpose and the service user guide available to service users and their relatives and social worker that gives information about Half Acre. Prospective service users and or there families are encouraged to visit the Home. At the last inspection serious concerns were raised about the ability of the staff team at Half Acre to care for a service user with a nursing assessment, which was contravening Half Acre’s registration certificate. No issues of this nature were found during this inspection HIGHFIELD CARE HOMES Ltd F56 F06 S8404 Highfield Care Homes Ltd (Half Acre) V217625 100505 Stage 4.doc Version 1.30 Page 9 A sample of care records for the above service users confirmed that an assessment by either a social worker or the registered manager prior to being admitted to Half Acre had been undertaken. Of the three new service users admitted recently, two had come from other Homes’ were the service user and their family had not been happy with the service offered. Review notes confirmed that the families are happy with the service at Half Acre with comments “ Half Acre is 100 better than her last home.” There are quite a few male service users at Half Acre. Prospective male service users saw this as an important consideration. There are two types of contract in place one for service users who are funded by a local authority and another for who are privately funded. No issues of concerns were raised at this inspection in relation to service users individual needs not being met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 8 9 10 Care plans and risk assessments examined were seen to be in good order, these provide staff members with information to enable them to meet service users needs. Improvements have also been made in respect of medication in staff training and security arrangements that help to protect the service users by ensuring safer practices. EVIDENCE: At the last inspection serious concerns were raised about the lack of care plans for a number of service users with a high level of need and the lack of regular reviewing of care and health needs. This has improved since the last inspection. Individual care plans are in place for service users. Care plans include areas of health, personal and social care needs. These are generally well written and offer clear instruction to staff members. References to psychological needs are also made. Care plans are kept under monthly review. Service users families are encouraged to become involved in care planning and are involved in review processes. Evidence of changing needs was reflected in the plans. Although improvement was noted, concerns were still raised about the care and risk assessments where a restriction was in place on the grounds of HIGHFIELD CARE HOMES Ltd F56 F06 S8404 Highfield Care Homes Ltd (Half Acre) V217625 100505 Stage 4.doc Version 1.30 Page 11 maintaining the health and safety of a service user. The service users family have now agreed the plan. The plan does however need to be developed further to demonstrate that all options have been considered and that restriction is the only option and that the service users human rights have been considered. A requirement has been made in respect of this issue. A range of health care risk assessments are in place including, pressure areas, moving and handling, nutrition, weight, continence and general risk assessments. District and Macmillan nurses provide input where appropriate. Service users have access to GP’s and domiciliary opticians, dentist and chiropody services. Equipment such as bed rails, pressure-relieving mattresses, air flow cushions, bed rails etc is provided as necessary. The medication system was checked and appeared to be in good order. Since the last inspection all the staff members administering medication have received accredited training and have a certificate to confirm this. Also the medication trolley can now be securely held outside the treatment room by means of a locking device. The inspector will request that a CSCI pharmacist visit Half Acre to audit the medication systems. The inspector observed service users being treated in a courteous manner throughout this inspection. Care was taken to ensure that service users privacy and dignity whilst involved in personal care tasks. Staff members do not open service users mail. A note is left at reception to let service users and their relatives know that there is mail in the office for them. Staff members and service users refer to each other on first name terms. It was noted that all staff members knocked on service users doors before entering. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 13 15 Half Acre provides service users with a range of activities for service users to join in, if they so choose. Service users experience of mealtimes is impaired due to the poor layout of the dining room and the quality and choice of food. EVIDENCE: Half Acre has recently employed a new activities organiser. Half Acre offers a range of social activities for service users. There are weekly planned activities, which from the previous week included, manicures, word search, card-making, name that tune with a flute player, “Frank Sinatra” entertainer, armchair basket ball, memory pairs etc. On the day of the inspection the hairdresser was visiting and preparation was underway to celebrate a service users birthday. Some service users have recently joined others at Half Acre’s sister home Rose Court for a tea dance. Staff members at Half Acre are also planning involvement in the forthcoming local carnival on a float. Some service users are going to watch part of the procession. Service users were looking forward to the event. A member of the local church visits every Sunday and there is a regular Catholic mass and a priest visits. Visitors were made welcome and staff members were observed assisting service users back to their own rooms, on request, so that visits could take HIGHFIELD CARE HOMES Ltd F56 F06 S8404 Highfield Care Homes Ltd (Half Acre) V217625 100505 Stage 4.doc Version 1.30 Page 13 place in private. Visitors can call at anytime but after 9pm a telephone call is requested. A placing officer’s comment card stated that they “ could hear noise and the chatting of residents from the lounge – an experience often noticeable by its absence.” The inspector received eight, service user comment cards. All were generally of a positive nature, though some service users stated that they were not always happy with the food offered. The dining areas are very compact (see Standard 20) and staff members had difficulty manoeuvring wheelchair users in and out of places available. There are only 22 dining places downstairs for a potential 29 service users. Some service users said that they were not always happy with the food they receive and that the dining area is “cramped.” This affects the experience of service users at this important social event. The staffing situation of the kitchen has improved since the last inspection, with a stable and consistent staff team that operates without the need for domestics and care staff to be taken away from their duties. The replacement of the tile floor has also been replaced with a non-slip floorcovering. Menu’s were examined and it was agreed with the manager that planning needs to be reviewed to ensure that a varied, wholesome and a balanced diet is provided and that choice is available to service users. A record of food provided to service users must be maintained. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Complaints from service users are taken seriously by the Home and were documented. The registered manager was approachable if service users had any concerns. Service users were confident that she would deal with them to their satisfaction. EVIDENCE: A complaints procedure, with relevant details of the Commission, is displayed in the foyer area. The Home has a complaints file. Complaints are taken seriously and a log is maintained. Disciplinary action would be taken if necessary. The Commission has received no complaints since the last inspection. Service users at this inspection raised no issues in respect of complaints. There is a comprehensive policy in place for the prevention of abuse. Adult protection forms part of the induction training for new staff members. A statement in respect of whistle blowing is included in the staff handbook and is available in the staff room. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 20 21 22 23 24 25 Half Acre provides a clean and homely environment for the people living there in respect of bedrooms. However, there are serious concerns about the adequacy of bathing and shower facilities, communal space and a number of health and safety matters, which have an impact this has on the health, safety and comfort of service users. EVIDENCE: Half Acre provides the people that live there with single bedrooms that are decorated and furnished to a good standard. The bedrooms are comfortable and in many cases highly personalised with residents own possessions. There is an enclosed patio area to the rear of the Home, which affords access to a bowling green. The service users enjoy watching bowling matches in fine weather. The Home is kept clean to a very good standard. Service users have the specialist equipment they need. HIGHFIELD CARE HOMES Ltd F56 F06 S8404 Highfield Care Homes Ltd (Half Acre) V217625 100505 Stage 4.doc Version 1.30 Page 16 There are however serious concerns about several matters in terms of the health, safety and comfort of the people who live in the home. There is currently only one shower available to up to 32 residents because of problems with the water supply in terms of heat and poor water pressure to the Parker baths. There are also concerns about Legionella in respect of water supplies at these points. The one bathroom on the ground floor is unsuitable for service users needs. This has been the case since April 2004. Since April 2003 concerns about the lack space available to service users in the combined lounge/dining area on the ground floor have been raised. This area is overcrowded and cannot accommodate all service users necessary furniture, visitors and activities taking place. e.g. staff moving service users, ability to watch television and talking to people in private and raises issues over safety and comfort of service users. At previous inspections the organisation agreed that this was the case and the planning department, in February 2004, passed the plans for the new conservatory to be built. This work is yet to be undertaken. The inspector is concerned that if the work does not take place soon that the better weather may be missed and the project will be put back for another year. Consideration during alteration, needs to be given to the adequacy of present storage arrangements for wheelchairs, stand aids, etc that occupy corridor areas. There are outstanding fire requirements made at the Fire Officer’s visit on 14th April 2005. The inspector was informed that this was being addressed. The CSCI needs to be informed when this situation has been addressed. The gas safety certificate was out of date and overdue from February 2004. Two new clinical waste bins are needed for the bathrooms to prevent malodour. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 28 29 and 30 The staffing arrangements have improved since the last inspection enabling the home to meet the needs of service users. Induction training for staff members has improved as well as opportunities to access NVQ training. Once this is completed this will ensure the health and safety of service users. EVIDENCE: The staffing situation has improved since the last inspection. There are currently no staffing vacancies at Half Acre. Agency staff are used on only a very occasionally to cover last minute sickness. The staff team is described as stable and working well together. NVQ Level 2 training has improved since the last inspection but there is still work to do. Five members of the day staff hold NVQ Level 2, approximately 35 . All staff members are CRB checked and recruitment files have been brought into line with the requirements though more work is needed to ensure that a full employment history has been given and the reason for any gaps has been evidenced. An induction format has been introduced which is in line with TOPSS. Every effort must be made to ensure that the present improvements are maintained to ensure the establishment of sound occupational standards. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 32 33 and 38 The registered manager is aware of her role to promote and safeguard the health, safety and welfare of the service users. However the organisation has been slow to respond to both the registered manager’s requests and to previous inspection reports even though their requirements have been agreed. This is to the detriment of service users and the running of the home. EVIDENCE: The registered manager has recently returned to Half Acre following maternity leave. The registered manager has completed the Registered Manager’s Award, however she does not have a certificate because the work has not been externally verified due to an unpaid bill on the part of the organisation. This situation has prevailed since June 2004, and this situation is unacceptable. Conversations with service users and staff members suggest that the return of the registered manager has brought with it consistency, which appeared to be lost at the last inspection. The registered manager must conduct a review of the quality of care provided by Half Acre and supply the Commission with a copy of the report. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard 8 Regulation 13 Requirement In situations were restrictions are in place for a service user details that all other options and that the service users rights have been considered, must be documented. That clearer records in respect of menu planning are maintained to ensure a varied, wholsome and nutritious and a record of choice offered to service users is also kept.
