CQC found this care home to be providing an Good service.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Care Plans are well written and regularly monitored and reviewed, in accordance with individuals` needs. Good quality evidence was seen to confirm that care planning is truly `person centred` and advice and guidance is sought and actioned, as appropriate, by relevant health professionals. The staff and management are striving to maintain a high standard of hygiene and cleanliness throughout the home, to ensure people live in a good quality environment. More time has been allocated to cleaning duties for staff and a full time cleaner has been employed, which is also helping to maintain good standards of hygiene and cleanliness.

What the care home could do better:

The manager (and staff) should give consideration to the amount of staff in the office at any one time and ensure, where possible, that numbers are kept to a minimum, so that staff are more available at all times for the service users in the main areas of the home.

It stands on the main road in the Norfolk village of Catfield. The large main house has been extended and provides 23 single rooms and 1 double room. The rooms in the extension are particularly good with their own shower facilities and French doors to the outside. The accommodation is bright and attractive and has extensive communal space. In addition, a new building on the grounds provides 8 self-contained flats, 6 Care Homes for Adults (18-65 years) Page 2 of 8 Brief description of the care home accommodating 2 service users and 2 for 1 person making 14 in all. These service users are assisted to shop, make their own meals and to be more independent. There is also a large garden. The basic fees are currently in the region of #315 per week and increase according to the level of support each individual requires. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: This random, unannounced, inspection included a tour of the premises and grounds, looking at cleaning records and care planning information, meeting and speaking with some of the people living at Hamilton House and having discussions with the manager and a member of staff. The tour of the premises and grounds showed all areas seen to be clean and mostly tidy, with generally no unpleasant smells noted. From records seen and a discussion with the manager, it was confirmed that in order to help eliminate unpleasant odours, staff assist the people who live at Hamilton House with regular cleaning checks of their rooms and a full time cleaner has been appointed, so that a good quality cleaning regime is carried out over seven days a week. The cleaner also regularly cleans each room thoroughly on a rotational basis. Additionally, it was confirmed through discussions with the manager, observations and requisition order sheets seen, that many people have had their bedroom carpets replaced with good quality/pleasant vinyl flooring, new beds have been ordered for some people (to be higher from the ground and not material divan bases) to help make thorough cleaning easier and more effective. Also, numerous waterproof (good quality, breathable) mattresses, duvet and pillow protectors have been ordered and purchased, together with an assortment of bed linen. Requisition orders looked at for the past few months stated that items were being requested due to current items being old/worn, mostly due to excessive washing, which confirmed that the manager and staff continue to ensure people consistently have good quality bedding and live in a comfortable environment, which is clean and hygienic. Care plan information that was looked at showed how the staff and management have previously, and continue to, work closely with other healthcare professionals, in order to improve individuals quality of life. Evidence was also seen of advice being sought and taken to assist people with these improvements. Observations, notes seen and a discussion with the manager confirmed that all hairdressing is now carried out by a qualified visiting hairdresser, in a separate room, which provides the people living at Hamilton House with more privacy and dignity, as required. All personal care tasks such as shaving were confirmed to be carried out in individuals personal rooms, thereby also ensuring peroples privacy and dignity is maintained. Observations, at the time of arrival for this unannounced inspection, were that five staff members were all in the office at the same time. However, it was also acknowledged and observed that they were each collecting money/keys etc for separate outings with service users. It was discussed with the manager that she gives consideration to this area and to ensure not all staff congregate in the office at the same time. With regard to a previous requirement, the notice on the conservatory door, regarding smoking, was seen to have been removed and the whole area much improved in respect Care Homes for Adults (18-65 years) Page 4 of 8 of cleanliness and tidiness. Over all, the tour of the premises, numerous and detailed records seen and discussions with the manager and a staff member, supported our judgement that Hamilton House continues to be a well run home. Previous requirements have been complied with and any concerns received by the Commission have been acknowledged and addressed/put right by the manager and staff, where applicable and as much as practicable.
