Conditions of registration: Date of last inspection 2nd November 2005 Brief Description of the Service: Hannah Levy House Trust has charitable status and has been set up to provide care, including personal care, to those members of the Jewish Faith who need such assistance. The home has a Board of Trustees that has regular contact with the home and there is a registered manager, Miss Julie Wareham in dayto-day charge of the home. The Responsible Individual is Mrs Jessica Blooman OBE, one of the trustees, who has regular contact with the home. Hannah Levy House Trust is situated half a mile from the centre of Westbourne, with its local shops, library and other amenities. The home is on a bus route providing easy access to Bournemouth town centre and the Travel Interchange. There is plenty of on site parking as well as parking on the roads surrounding the home. Hannah Levy House Trust is registered to accommodate a maximum of 35 older people and provides both permanent and respite care as well as day care. All rooms are for single occupancy although some are large enough to be used as doubles should this be requested by residents. Bedrooms are situated on the ground, first and second floors. There is a dining room, large lounge and also a large conservatory all of which are well furnished and equipped. Aids and equipment are available for residents who may have disabilities, to help promote independence. There are passenger and stair lifts to aid access to all floors of the home as well as assisted baths, hoists etc. The home has a small synagogue where services are held on Shabbat and Holy Days. All festivals are also celebrated within the home. The home has good links with the local congregation and has its own minibus to enable residents to make trips out to either local synagogues, visit members of the Jewish community, clubs etc. The home is centrally heated throughout and twenty-four of the thirty-five bedrooms are provided with ensuite facilities. There is an accessible rear garden with garden furniture and a patio area. Hannah Levy House Trust provides twenty-four hour personal care, all meals, laundry and domestic services. Residents are encouraged to participate in a wide range of activities,

The home’s brochure states that its aims and objectives are as follows: The aim of the management of Hannah Levy House is to enable Jewish residents of Bristol and Bournemouth and their environs, to enjoy the optimum possible quality of life having regard to the individual’s age and state of health. The objectives through which this aim may be achieved are: To provide a strictly kosher environment in which facilities are available to meet the emotional, religious, spiritual and cultural needs of all residents. To assess the care needs of each individual resident and to provide a high standard of care and support to meet those needs through the employment of well trained staff. To create a safe, secure and pleasant home for residents and a good atmosphere in which to work for staff. To involve relatives and friends, where possible, in all aspects of the residents care, keeping them informed and encouraging them to maintain family life. To facilitate the interaction between residents and the local Jewish community by participation in communal activities and maintaining an open visiting policy. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 2 hours on the 24th February 2006 and was one of two statutory inspections carried out each year. The main purpose of the inspection was to check that the service users living in the home were safe and properly cared for and two requirements made at the previous inspection had been met. The previous inspection took place during November 2005; since that inspection no complaints about the home have been received or investigated. A tour of the premises took place and the inspector spoke with four service users, three staff and together with the registered manager, Miss J Wareham, considered other evidence relating to the National Minimum Standards, as described in this report. At the last inspection, most of the National Minimum Standards examined were met, so this inspection was brief in duration and focussed on assessment of only the most essential Standards. The majority of Standards assessed and found met during the previous inspection were not reassessed during this inspection; this report should therefore be read in conjunction with the report of the previous inspection of 2nd November 2005. What the service does well:
All residents at Hannah Levy House Trust are supplied with terms and conditions of residence at the point of admission to the home. These are clearly laid out and easy to read. A thorough admissions procedure is in place. This ensures that only those who was needs can be met by the home are offered places. All prospective residents are assessed prior to admission. The outcome of this assessment is confirmed in writing so that prospective residents are fully assured their care needs can be met. Satisfactory arrangements are in place for managing medication and records indicate that residents receive their medicines as prescribed. Hannah Levy House Trust provides a kosher diet under strict supervision and all prospective residents are made aware of this prior to admission. The chef says he tries to provide residents with whatever they want. Residents commented, I think that, on the whole, we have very good food here. I think we are very fortunate and I have no complaints about the food. Residents have their legal rights protected and are assisted in exercising these rights, for example by voting in local and national elections. The home has an

Miss Wareham is ensuring that all staff have received training in Adult Protection issues, to ensure a proper response to any suspicion or allegation of abuse. The home provides sufficient communal bathrooms and WCs to meet the needs of residents. In addition, 24 of the 35 bedrooms in the home are provided with ensuite facilities. Training is provided to make sure that staff are equipped with the knowledge and skills necessary to carry out their work and meet the needs of residents. Residents are assured that their financial interests are protected and properly managed. The home seeks to provide a safe environment to ensure the protection of residents. Measures are in place to promote the health and safety of residents. For example, equipment is regularly serviced and maintained. All substances that could be potentially hazardous to health are handled and stored safely. Regular maintenance of the fire warning system, emergency lighting and fire fighting equipment is taking place. Staff take part in regular fire training and drills so that they are fully aware of what to do in the event of fire. What has improved since the last inspection? What they could do better:

It is also recommended that copies of Patient Information Leaflets be obtained from the pharmacist to give further information about each of the medicines in use. Medicines requiring storage at low temperatures (usually 2-8°C) must be kept in a fridge. The actual temperature of the medicines fridge at Hannah Levy House Trust is checked on a weekly basis. Guidance from the Royal Pharmaceutical Society states that in order to effectively monitor the fridge temperature, a maximum/minimum thermometer should be used on a daily basis. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 3 (Standards 1, 2 and 5 were found met at the previous inspection) Service users are issued with a contract describing Terms and Conditions of occupancy at the point of admission to Hannah Levy House Trust. A thorough admissions procedure allows prospective residents to make informed decisions about admission to Hannah Levy House Trust and ensures that only those whose needs can be met by the home are offered places there. EVIDENCE: All residents at Hannah Levy House Trust are issued with terms and conditions of residence. These are clearly laid out and easy to read. The terms and conditions are signed by the resident or their representative and the registered provider. The resident and/or their representative retain a copy and a copy is held on file. At the last inspection the process of updating the terms and conditions was in progress and this has now been satisfactorily completed. Hannah Levy House Trust has a written admissions policy. Individual care records are kept for each resident and care needs are fully assessed by the

However, at the last inspection, it was noted that the outcome of such assessments was not confirmed in writing, so prospective residents were not fully assured that their care needs would be met. Documentary evidence was seen during this inspection to demonstrate that this requirement has now been met. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 (Standards 7, 8, 10 and 11 were found met at the previous inspection) There are satisfactory arrangements in place for managing medication and ensuring that the medication needs of service users are met. Recommendations were made regarding the use of the medicines fridge and obtaining Patient Information Leaflets, to ensure staff have the information they need about the medicines in use. EVIDENCE: Medication is stored securely within the home. There is a separate cupboard available for Controlled Drugs if necessary. Only senior staff administer medication and are responsible for ordering and recording all medicines received and for any unwanted medication sent back to the pharmacy. A monitored dosage system is in use. A sample of Medicine Administration Record (MAR) charts was checked. These demonstrated that staff sign to record that medicines have been given as prescribed or record the reason for non-administration. All staff that administer medicines have received training on medicines, how they are used and how to recognise and deal with problems in use.

This will record the maximum, minimum and actual temperatures daily when medicines are stored in the fridge. After taking the readings the thermometer must be reset for the next recording period. Care staff should have a clear understanding of the action to be taken if the temperature is outside the normal range. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 (Standards 12, 13 14 and 15 were found met at the previous inspection) Kosher food is provided at times and locations convenient to residents. EVIDENCE: Hannah Levy House Trust provides a kosher diet under strict supervision. All prospective residents are made aware of this prior to admission. The chef has considerable experience in kosher catering. Lunch on the day of inspection was a choice of fruit juice, followed by quiche or fish, with chipped potatoes. This was followed by a choice of baked rice pudding, fruit and cream/ice cream or fresh fruit. The evening meal was to be chicken soup or fresh fruit, followed by roast chicken or fish, roast potatoes, cabbage and parsnips, then stewed apricots or fresh fruit. Alternatives are always available to suit individual taste and preference. The chef says his policy is, as far as possible, to provide residents with whatever they want, as long as it is kosher. All meals are usually taken in the dining room or in the residents own bedroom. The lunchtime meal in the dining room took place over two sittings Meal times were relaxed with those residents requiring extra assistance receiving it in an unhurried and caring atmosphere. Mealtimes can be flexible to fit in with care needs, appointments etc. The menu shows that residents

Residents commented, I think that, on the whole, we have very good food here. The food is much better than many other places I have been in. JUDGEMENT – we looked at outcomes for the following standard(s): 17 and 18 (Standard 16 was met at the previous inspection) Residents have their legal rights protected and are assisted in exercising their rights. The home has an Adult Protection policy in place to ensure residents are protected from possible abuse. EVIDENCE: Ms Wareham confirmed that all service users are on the electoral roll. During local and national elections, opportunities are made available to residents to either vote in person, by proxy or by post. At the last election a number of residents were taken to the local polling station in the home’s minibus. Service users have access to information within the home about various independent advice agencies and advocacy etc. Hannah Levy House Trust Trust has comprehensive policies and procedures in place to protect residents from the risk of abuse. These also include related policies about ‘Whistleblowing’, Gifts and Gratuities for staff, Management of residents finances and other relevant matters. A copy of the Department of Health No Secrets guidance regarding Adult Protection is available in the home. At the last inspection it was noted that not all staff had received Adult Protection training, to ensure a proper response to any suspicion or allegation of abuse. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21 (Standards 19, 20, 22, 24 and 26 were met at the previous inspection) There are sufficient communal bathrooms and toilets available to meet the needs of residents. EVIDENCE: Assisted baths are situated on each of the ground, first and second floors. Each of the bathrooms also has a WC. The ground floor bathroom contains a Hydro bath with Jacuzzi facility. A shower room is also available on the ground floor. There are sufficient additional WCs around the home, including those which are easily accessible from the communal lounge and dining areas. In addition, twenty-four of the thirty-five bedrooms in the home are provided with ensuite facilities. The home has an annual development plan with an ongoing (rolling programme) of internal redecoration and replacement of furnishings and fittings, as and when required. Recent work includes the redecoration of several bedrooms, a new reclining chair for the lounge, carpet renewal in some

JUDGEMENT – we looked at outcomes for the following standard(s): 30 (Standards 27, 28 and 29 were met at the previous inspection) Training is provided to make sure that staff are equipped with the knowledge and skills necessary to meet the assessed needs of residents. EVIDENCE: At the last inspection, it was noted that some staff had undertaken training in moving and handling, basic food hygiene, Adult Protection, medication awareness, dementia awareness, continence, health and safety and first aid. Infection control training was being planned. It was recommended that a training audit be carried out to ensure that all staff had received the training they needed, with particular reference to moving and handling, Adult Protection, first aid and infection control. Miss Wareham evidenced that an audit has been completed. Training courses in first aid, health and safety, Adult Protection, basic food hygiene and moving and handling have been booked to take place shortly. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 and 38 (Standards 31, 32, 33 and 36 were met at the previous inspection) Residents are assured of sound management of their financial interests. The home seeks to provide a safe environment to ensure the protection of residents. EVIDENCE: Some residents at Hannah Levy House Trust manage their own financial affairs whilst others are assisted by a relative or representative. The home also looks after monies for some residents. A separate account is maintained for each resident and records kept of all incoming and outgoing payments. Each resident has a key worker, a member of care staff with special responsibilities towards a small number of residents. If necessary, the key worker will assist residents with purchasing items of clothing etc. All receipts for such purchases are retained. Balance checks are carried out by two members of the management team to ensure accurate records are maintained.

A detailed health and safety policy is in place and staff receive training in health and safety matters. A full-time maintenance person is employed to deal with minor repairs. Records confirm that equipment, such as hoists, the passenger lift and portable electrical appliances etc are regularly serviced to ensure resident safety. All substances that could be potentially hazardous to health are handled and stored safely. Radiator surfaces are guarded in bedrooms and communal areas to prevent direct contact with very hot surfaces. The fire records show appropriate checks and maintenance being carried out on the fire warning system, emergency lighting and fire fighting equipment. Staff take part in regular fire training and drills so that they are fully aware of what to do in the event of fire. Risk assessments are in place for all parts of the building, including the car park. Risk assessments are also carried out for any contractors working in the home. Miss Wareham carries out regular checks on water temperatures, window restrictors, wheelchairs, thermostatic mixing valves and showerheads to help ensure resident safety.
