Brief Description of the Service: Hanover Drive is a care home, which is registered to provide personal care and accommodation for three people with a learning disability. The service is one of six homes in the area, which were owned by the Shaftsbury Society until June 2007 when they merged with John Grooms. The owners are now Grooms – Shaftesbury. 50 Hanover Drive is situated in a residential area of Brackley in Northamptonshire. The home is within walking distance of Brackley Town Centre where community resources include shops, pubs, restaurants, gymnasium and a swimming pool. Two of the residents have bedrooms upstairs, and one on the ground floor. The dining area, kitchen and lounge are also on the ground floor. There is a wellmaintained garden at the rear of the property. The Registered Manager, who is also Registered Manager for Holly Close and Remus Gate, has an office base on the first floor at Hanover Drive. Fees are dependent on the level of support required and currently range from £1,331.50 to £1,621.05 per week. Fees include accommodation, food and personal care and support. This is an overview of what the inspector found during the inspection. Standards identified as ‘key’ standards and highlighted through the report were inspected. The key standards are those considered by the Commission to have a particular impact on outcomes for residents. Inspection of the standards was achieved through review of existing evidence, pre-inspection planning, an unannounced inspection visit to the home and drawing together all of the evidence gathered. The pre-inspection planning was carried out over the period of a day and involved reviewing the service history, which details all contact with the home including notifications of events reported by the home, telephone calls and any complaints received. The information reviewed has been taken from the date of the current registration, which was June 2007. The unannounced inspection visit covered the morning of a weekday. The inspection was carried out by ‘case tracking’, which involves selecting samples of residents’ records and tracking their care and experiences. Following the inspection and prior to completion of the report Observations of the homes routines and care provided were made and the inspector spoke with residents about the support that they received. Staff were also spoken with to ascertain their views on the service provided. In addition information received following the inspection in surveys completed by two residents, a relative and two staff members has been taken into account. The management of a sample of residents’ medication was checked. And a sample of staff files reviewed to check the adequacy of the recruitment procedures in safeguarding residents’. Communal areas viewed and two residents invited the inspector to see their rooms Verbal feedback was given to the Registered Manager throughout the inspection. What the service does well:
There is an experienced staff team who appear committed to meeting the needs of residents. Residents were obviously comfortable and relaxed in the presence of staff and had developed good relationships with them. Routines within the home are flexible and based around residents’ individual choices and commitments and Hanover Drive is treated as the residents’ home by staff. Staff work with residents to provide them with support to meet their

Residents were encouraged to take responsibility and have some independence in their daily routines. Two residents spoken with were happy with their daily lives and the level of support that they receive from staff. One resident said that staff are also supportive in helping them attend the local church. Residents are clear about their right to raise any concerns and are encouraged to do so. This helps to ensure that any issues can be dealt with and that residents are safeguarded. What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The admission process provides assurances that the needs of people admitted to the home can be met. EVIDENCE: There is a statement of purpose and service user guide, which provides information about the service provided for prospective and existing residents’ and their families. A pictorial version of the service user guide is also available for residents’ if required. A sample check of the information within the service user guide identified the need to review and update some of the information. Advice was given about the need to include clear information about fees and additional charges to ensure people understand what the fee covers and what additional costs they are likely to have. It is now a requirement that this information is available to help prospective residents make decisions about their care. At the time of the inspection there were no vacancies in the home, however it was established that if there were to be a vacancy there is a thorough admission process. The admission process involves an assessment of needs

Comments from a resident received in the surveys confirm that they were able to visit before moving in but indicated that they had not been very involved in the decision making process. This judgement has been made using available evidence including a visit to this service. Residents are encouraged and supported in expressing preferences and making choices. EVIDENCE: A resident’s care plan reviewed contained clear information about their preferences and the support needed to meet their needs. A document titled “my introduction” provides new staff with sufficient information to provide the resident with consistent support. Surveys received from two staff members confirms that they are always given up to date information about residents needs. There was evidence within the care plans that residents’ had been involved in their development and that their views had been sought and wishes identified. Hanover Drive DS0000070264.V351842.R01.S.doc Version 5.2 Page 11 Observations and discussion with staff during the inspection confirmed that staff support residents in taking responsibility and achieving some independence in their daily routines. There is also flexibility in daily routines, for example times for getting up and going to bed are based around individual commitments and choices. There were records in place identifying situations where residents rights may be infringed, which had been signed by them. The infringed rights were in the main, the right for staff to enter the resident’s personal room, in the event of a fire and for fire prevention and maintenance checks to take place. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16, 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are supported to lead a lifestyle, which in the main meets their needs and preferences. However review of the arrangements for transport and holidays needs to take place to ensure that residents’ opportunities are not unnecessarily limited. EVIDENCE: On the day of the inspection two residents were going to the Links day centre, which is run by Grooms-Shaftesbury for residents from several care homes. The third resident was going to the day centre in the afternoon. Two residents’ spoken with said that they liked the day centre and were able to take part in a range of different activities there. A resident said that they have a day off from day centre each week and this is when they like to go to the shops. Records showed that a routine for the day

Daily care records show visits to places such as a farmers market, shops in Brackley and Buckingham, a pub/restaurant and church. Another resident said they liked to go to church every Sunday and staff support was provided to enable them to attend. Records showed that residents’ are supported to maintain contact with their families, and one resident was keen to discuss the arrangements that had been made for Christmas. An entry in a resident’s records indicated that a visit to a parent had to be rearranged due to transport not being available. A relative commented within the survey: “ I think they need more transport so that residents can go out and about more frequently”. Staff advised that there are six small homes in Brackley and there used to be two cars shared by three houses. This has recently reduced to one car for the six houses and is allocated on a first come first served basis with priority given for medical appointments which may on occasions limit leisure activities. Information received in the Annual Quality Assurance self assessment submitted by staff at Hanover Drive indicates that efforts are being made to resolve the transport problem and to arrange further social activities for residents. Discussion with a resident and a member of staff identified that residents had all been away on holiday this year supported by staff. Residents pay for the cost of their holidays and in the past have shared the cost of staff accommodation. This practice has quite rightly been stopped by the organisation, however at present there are no alternative arrangements in place to enable residents’ to go away which limits residents’ opportunities. The Registered Manager confirmed that a resolution to the problem was currently being sought. Advice was given to consider accessing support for the resident from an advocate. Residents’ said that they have choice with their meals and that the menus are planned with them on a weekly basis. Individual food preferences and nutritional needs are documented within residents’ care plans. Residents are encouraged to be involved in shopping for food and with the preparation and cooking. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents receive good care and support with health care services being accessed as appropriate. EVIDENCE: Comments in the surveys received include one from a relative, which states, “I can not fault the care that my -------- receives”. Individual care plans are very clear about the support residents need from staff in relation to their personal care. The plans and observations during the inspection indicate that staff encourage residents to be as independent as possible. Care records demonstrate that residents are supported to access health care services such as dentist, optician and chiropody. Appointments and advice are sought from health care professionals as required such as General Practitioner, Consultant psychiatrist and the Community Learning Disability Nurse helping to meet physical and emotional needs. Hanover Drive DS0000070264.V351842.R01.S.doc Version 5.2 Page 15 Discussion with staff confirmed that they are aware of the individual health care needs of residents and support residents in acting on the advice from health professionals. For example each resident has their own daily routine, which takes account of any specific professional advice such as specific exercises. A Community Learning Disability Nurse has been working with Staff, advising staff on strategies for managing behaviours in order to improve the daily lives of all residents’ in the house. A sample check of the management of residents’ medication identified that it is generally well managed. The medication was securely stored. Advice was given to return some discontinued creams to the pharmacist for disposal. A staff member administering medication discussed the medication with the resident and ensured that it was checked against the prescription before administration reducing the risk of error. The prescribed medication was available and the staff member signed the record to confirm administration. Staff spoken with, were mindful of the need to ensure that the procedures in place for the safe administration of medication are followed and that any discrepancies are reported to the Manager. This is important to ensure that residents are not put at risk, however advice has been given to consider if a missed signature, where it is clear medication has been given, could be dealt with through the staff supervision and training process rather than safeguarding adults. This judgement has been made using available evidence including a visit to this service. There is a positive approach towards complaints, which helps residents to raise concerns and provides good safeguards. EVIDENCE: The Commission for Social Care Inspection have received no complaints about the service. Surveys received from two residents confirm that they know who to talk to if they are not happy and how to make a complaint. Records at Hanover Drive show that two complaints have been received from a resident. Review of the information confirms that residents are supported and feel able to raise concerns, which are taken seriously and acted on. Staff spoken with including agency staff were aware of their responsibilities for safeguarding the people in their care and reporting any concerns that they may have. Northamptonshire County Council safeguarding adults procedures were available for reference in the event of concerns being raised. It was identified that the organisations procedures require completion of a protection of vulnerable adults referral for all medication and medication errors regardless of risk. Advice has been given to consider whether it is necessary to make a safeguarding referral to social services and notify the Commission for Social Care Inspection in cases where it was clear that a resident had not been placed at risk. It is of concern that overuse of the procedures may lead to incidents not being taken sufficiently seriously.

This judgement has been made using available evidence including a visit to this service. Resident’s live in a home that is comfortable and homely. EVIDENCE: The home is of domestic layout situated in a residential area within easy travelling distance to the town centre and local community resources; all areas were clean, tidy and well maintained. Residents confirmed in the two surveys received that the home is always fresh and clean. The three resident all have single rooms, two are located on the first floor and the third is on the ground floor. Two residents spoken with were happy with their rooms, which were personalised, having TV, Video and personal entertainment systems within the rooms. Residents confirmed that staff respect their right to privacy. Staff were observed to knock and be invited in, before entering residents rooms.

JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Staffing arrangements are generally very good, however the recruitment process needs to be more thorough to ensure that residents are not put at risk. EVIDENCE: Observations and discussion with residents and staff indicated that there are sufficient staff to meet the needs of residents. Agency and permanent staff spoken with presented as caring and knowledgeable about the needs of individual residents. The agency staff used are all familiar to the residents having in some cases worked in the home for a few years. Residents spoken with were happy with the staff team and interactions between staff and residents indicated that good trusting relationships had been developed.
Hanover Drive DS0000070264.V351842.R01.S.doc Version 5.2 Page 21 A sample check of staff files to check the adequacy of the recruitment process identified some shortfalls in the information obtained. Criminal record bureau clearances had been obtained prior to staff working in the home. However shortfalls were identified in the process for a member of staff who had been recruited while the Registered Manager was away on sick leave. The application indicated that a full employment history had not been declared and there was no evidence of challenge or further discussion. Advice was also given about gathering references from people able to comment on a person’s suitability for working with vulnerable people. This is important in helping to safeguard residents. Induction training was being prepared for a member of staff due to start work. Discussion with staff, responses from two staff members in surveys and a sample check of the records confirmed that staff receive training appropriate to meeting the needs of residents. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 42 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is managed in a manner that promotes and safeguards the health, safety and welfare of residents’. EVIDENCE: The Registered Manager has several years experience of working with the client group and appears committed to providing good care and support for residents. Discussion with the Registered Manager identified that she had been away on sick leave for a period of seven weeks. CSCI received no notification of this or of the arrangements for managing the home in her absence. Under the Care Homes Regulations 2001 there is a requirement that CSCI are informed of any
Hanover Drive DS0000070264.V351842.R01.S.doc Version 5.2 Page 23 absence of a registered person including a registered manager over 28 days. The role of the registered people is considered to be particularly important in relation to the welfare of residents as they hold the legal responsibilities. The Commission for Social Care Inspection (CSCI) requested completion of an annual quality assurance self assessment (AQAA). It would appear that this was sent to Hanover Drive for completion during the period that the registered manager was on sick leave. In spite of a follow up letter reminding of the legal responsibilities for completing this document it still was not returned. This reenforces the need to keep CSCI informed and also having arrangements in place to deal with matters that arise during a manager’s absence. Following the inspection visit the Registered Manager has forwarded a copy of the AQAA completed by two staff members to CSCI. Review of the information indicates that that there is an awareness of the need to carry out an ongoing review of the support provided to residents to ensure that their needs and expectations are met. On the day of the inspection the Area Manager was carrying out a monthly visit to monitor the quality of care provided. Discussion identified that each month there is a different focus to the visit and that checks are made as to how well the home is meeting the National Minimum Standards. No health and safety concerns were identified during the inspection. Records and discussion with staff confirm that appropriate training in safe working practices is provided for staff such as first aid, movement and handling and fire safety. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. Information must include the reasons and proposed arrangements for management of the home during the absence. This is to confirm ensure that proper arrangements are in place to safeguard residents. An annual quality assurance 20/12/07 assessment must be supplied as requested and in future requests must be returned within one month of the request.
