Conditions of registration: Date of last inspection 22nd June 2005 Brief Description of the Service: Harbledown Lodge is an elegant Georgian Country House. It is set within 50 Acres of grounds. The Home is approached by its own private drive. Also within the grounds is a day centre. The Home has access to the centres facilities. The Home has parking spaces for visitors. Harbledown Lodge offers nursing care for up to 56 Service Users. It has forty-two single rooms and seven double rooms. Many rooms have en-suite facilities. This is an overview of what the inspector found during the inspection. This was an announced inspection. It was conducted with the assistance of the Registered Manager Mrs. Julie Caine over a one day period. The inspector spoke with people that live in the home and staff. The inspector received positive feedback in the form of comment cards from visitors and people that live in the home. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Prior to each person moving into the home, their needs assessed this is to ensure the home can meet individuals needs. EVIDENCE: People only move into the home after having a comprehensive, holistic assessment, conducted by the Manager or another nurse trained to do so. Pre-admission assessments viewed were in-depth, exploring physical, psychological, and social needs. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9 People that live at the home have an individual plan of care. The homes policies and procedures for administering medication safeguard the people that live there. EVIDENCE: A number of care plans were viewed. Care plans are developed from the information that is gathered at the initial pre-admission assessment and additionally for some people, the care manager’s assessment of needs. Care plans were well written, and well reviewed. They held appropriate information to enable staff to care for the people who live in the Home. Suitable risk assessments are in place to ensure as far as reasonably practicable the safety of people living in the home. Records for the receipt, administration and disposal of medicines were viewed. They are well documented and appropriately recorded. The home has a suitable medication room, with a sink, and controlled medications cupboard. Medication amounts were cross-referenced to the controlled medicines book and found to tally. It was noted that the sharps box in the medical room was not dated. Recommendation made regarding this matter. (Recommendation 1 Part A).

Suitable procedures, in line with the homes only policy allows for this to be monitored. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 People that live in the home can exercise choice and control over their lives. EVIDENCE: People in the home are encouraged to make their own choices. They can make choices such as: when they get up and when they go to bed. There are meetings for the people that live in the home, so that they can influence the way in which they live. Meetings are recorded and actioned. People are encouraged to look after their own finances, but the home does hold small amounts of pocket money for people who cannot manage their own money. People that live in the home have their preferences regarding the way they live recorded in their care plan. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 People that live in the home are protected from abuse. EVIDENCE: All nurses and carers at the home have had training in the prevention of adult abuse. Carers cover this subject in their induction training. Carers that the inspector spoke with demonstrated a good knowledge of abuse issues. The home has a suitable whistle blowing policy. The home has had no allegations of abuse and no POVA referrals. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,21,26 The home is safe and well maintained. The home has a sufficient amount of bathrooms suitable to the needs of people that live in the home. The home is clean and hygienic throughout. EVIDENCE: The lay out and location of the home is suitable for its stated purpose. The Home is accessible and well maintained; the grounds are accessible to wheelchair users and are kept tidy, attractive and safe. The building complies with the requirements of the local fire service and environmental health department. The home has automatic door closures fitted on all doors. Bathroom and toilets were clean and uncluttered. All bathrooms have suitable locks. The home has a selection of assisted bathrooms. People that live in the home have the option of having either a bath or a shower. Bathrooms, toilets and sluices have appropriate hand washing facilities i.e. paper towels and liquid soap, for the control of infection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35 The Manager is trained and competent to be in charge of the Home. Service users finances are safe guarded. EVIDENCE: The Registered manger is a trained nurse. She has recently been registered by the Commission to be the Manager of Harbledown Lodge. She was previously registered as the Manager of another Home within the group. Staff and people who live at the home agreed that the Manager was approachable. The Home has meetings for night staff, day staff, maintenance, the people who live in the Home, and the groups Managers. These are all minuted and actioned. The home holds pocket money for some people. The home provides secure facilities for the safe keeping of people’s money. Money is held individually, and not pooled. Receipts of any transactions are retained. Three peoples money was cross-referenced to their accounts and were found to be accurate.
