Date of last inspection 21/10/2004 Brief Description of the Service: 1a Harborough Road provides twenty-four hour residential care for four adults with a learning disability. The home is run by Mencap and benefits from the support of a wider organisation.The home is situated in a residential street within walking distance of Rushden town centre and local amenities. All residents are accommodated in single bedrooms with one located on the ground floor. There is a comfortably furnished lounge and a large kitchen where residents share their meals with the staff. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over the morning of a weekday. Residents were in the process of getting up and ready for their daytime activities when the Inspector arrived. As none of the Residents have any verbal communication and communicate their needs to Staff mainly through gesture it was not possible to ascertain their views on the care provided. Judgements regarding the standards of care have been formed from observations of the interactions between Residents and Staff, discussion with a member of Staff about a Residents care needs and a sample check of one Residents care records to see how the care is planned and supported. What the service does well: What has improved since the last inspection?
Staff has received training in the administration of a particular medication, which may be required for a Resident with epilepsy. Speech therapy has been arranged for a Resident via the day centre and the home are going to be liaising with them regarding any recommendations. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 and 9 Residents are receiving good support in their everyday lives and have the opportunity to make some choices however care planning processes do not identify and fully support personal goals. EVIDENCE: From discussions with Staff and observations they have developed a good understanding of Residents and their needs. Residents have their own particular routines and given the complex needs of Residents and the fact that they have no verbal communication it is particularly important that Staff is aware of these. Due to Residents having restricted communication abilities it was not possible to ascertain their views on how they are provided with choices. Staff was seen to be providing choices where possible for example several choices of breakfast were offered to a Resident. Staff showed pictures of foods and meals, which are being collated to try to increase choices for Residents. Risk assessments and plans of care are in place to guide Staff as to the actions they need to take to meet each Residents care needs. A sample check confirmed that relevant areas are included however in some cases more specific information is required, for example a care plan for seizures stated

It is important that more specific guidance is given particularly as there is some fairly new Staff. Although it was apparent that care plans and risk assessments had been added as new risks and care needs are identified care plans were not consistently dated and signed. The Inspector was told that the existing care plans are going to be reviewed and a more person centred approach to care planning is going to be adopted. Currently there is no evidence of planning to meet particular goals in the care plans however this will be part of the new format. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 Residents are involved in daytime occupations and activities and are supported in maintaining links with families. EVIDENCE: All of the Residents attend day centres and their programmes are logged in the Staff handover book. On the day of the inspection three Residents were attending day centre and one Resident was staying at home for the day. Residents appeared to be happy to be going to the various centres. Reviews are held with the day centres and Staff confirmed that they try to liaise with them to ensure consistency of approach. Conversations with Staff confirmed that Residents are supported and encouraged to maintain links with families. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 and 20 The overall standard of care and personal support is good and health care services are being accessed. EVIDENCE: On arrival at the home Staff were assisting Residents with bathing and dressing. Staff was mindful of Residents privacy and dignity and all personal care was provided in the privacy of bedrooms and bathrooms. Residents were all assisted to be appropriately dressed and well presented prior to going to the day centres. There is appropriate equipment to assist with bathing and changing a Resident who has a physical disability in addition to a Learning disability, which has been installed following an occupational health assessment. Staff were observed administering medication to Residents. Medication is checked by two Staff, administered and signed as given to reduce the risk of errors. Staff confirmed that they have received training in the administration of a rectal medication which although hasn’t been required for many years is prescribed for a Resident. It was confirmed that particular Staff have been authorised by a Health Professional to administer this medication.

The Registered Manager is also going to contact the Community Nurse to arrange for a full health check to be carried out and health action plan developed. It is recommended that this be pursued to ensure that all relevant health care advice and support is being accessed. Follow up checks and records following accidents were discussed and following the inspection the Registered Manager has advised that more detailed checks and records following accidents or identification of bruising have been introduced. These more detailed records have had the benefit of identifying that the way a Resident has been using the garden swing has been the cause of some bruising. JUDGEMENT – we looked at outcomes for standard(s) 24 and 28 Residents are provided with a clean, homely and comfortable place to live. EVIDENCE: The home is situated in a residential area on the outskirts of the town of Rushden. The home is not identifiable as a Residential Care home. The premises are clean, bright, homely and comfortable in style. The furnishings, fittings and equipment appear to be of good quality. Shared space in the home consists of a dining/kitchen, a lounge and an enclosed garden. Residents were having breakfast in the kitchen during the inspection. Breakfast is served individually as and when Residents are washed and dressed however on previous visits in the evening the Inspector has seen Staff and Residents eating an evening meal together in the kitchen. The lounge is furnished with comfortable chairs and a settee and there is a television. The enclosed garden was well maintained a task, which is carried out by Staff in the home.
