The home is registered for a maximum of 50 service users to include: up to 50 service users in the category of OP (older people not falling within any other category) up to 50 service users in the category of PD (younger adults with a physical disability) up to 6 service users within the category of DE (Dementia over 65 years of age) The service should at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 10th August 2005 2. Date of last inspection Brief Description of the Service: Harbour View is a purpose built care home registered to provide nursing care for up to 50 residents from the age of 18 years. The home was built on Bransty hill overlooking Whitehaven harbour. The residents said that they had many things to see from the home including small children in the play area, dogs being walked, boats out at sea and the comings and goings around the harbour. There is a good-sized car park in front of the home. Accommodation is provided in single en suite bedrooms on two floors. The décor was in very good condition and residents had been involved in choosing the colour schemes for their bedrooms. The facilities are accessible and meet the needs of residents and aids and equipment are in place to assist where necessary. The inspection commenced at 09:00 and concluded at 14:30. The inspection included talking with residents, visitors, and staff. Records including care plans, staff records and recruitment files and other relevant documents were looked at. A tour of all the building was also conducted and the manager was present throughout the day. What the service does well: What has improved since the last inspection? The décor within the home continues to be up dated and provides a bright, homely, clean and fresh environment for the residents. The lounge areas are newly decorated with comfortable seating areas and bright new carpets. The dining rooms have had new laminate floors, which the staff say are easy to keep clean. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, and 5 Prospective residents are given extensive information about the home enabling them to make informed decisions. Residents are confident that the home will meet their needs. Each resident has an individual written contract stating terms and conditions with the home. EVIDENCE: Residents are given information about the home before they move in. The information describes the accommodation and facilities available. It also gives information about the staff and the daily activities within the home. This enables residents to make informed decisions as to whether the home would suit their needs. Residents spoken to said that they looked around before they moved in and also they had the opportunity to talk to the staff. The staff assessed the residents prior to moving into the home and the residents said they felt the home would be able to meet their needs and gave them the chance to ask any questions they had. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 Where appropriate residents were consulted in all aspects of there lives to help them remain as independent as possible. EVIDENCE: Service users within the home have the opportunity for a full and varied life. Some attended day services and college. They are given choices and consulted on as to their daily routines and asked to choose what they wish to eat. Residents spoken to said that there was plenty for them to do and the home organised outings for those who wished to join in. Some residents spoken to preferred not to take advantage of the activities and felt that the staff did not mind them ‘ sitting and watching’ rather than joining in. Others spoken to preferred to spend the day in there own room. One resident found it more comfortable in bed, so spent most of her time there but in the daytime the staff moved the bed so she had a view out of the window. Documentation on the care plans also highlighted residents personal preferences. There was a daily activities programme on display in the reception area for the weeks’ activities. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11, 12, 13, 15, and 16 Residents are given the opportunity for personal development, and mix with their own peer groups. The home is very much part of the local community and residents are able to make friends and develop relationships. EVIDENCE: Residents are given the opportunity to extend their personal development. Activities within the home are varied and some residents are encouraged to attend day centres. Some residents were just getting on the bus as the inspector arrived and they were looking forward to their day at work. Other residents visited the Disability Day Services and one attended the Multiple Sclerosis Centre. Other residents have visits to the home from the Rehabilitation team or the physiotherapist from the stroke unit, to help with the residents’ progress. Families are actively encouraged to visit and they can bring in their pets if the residents wish. Residents lead their lives based on individual choice and having a chance to have a say into how they lead their lives. All choices made are documented within the care plans for all staff to respect and follow. Residents have the benefit of a separate therapy room and hair salon within the home for one to one sessions, which they enjoy. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21 Residents were confident that the staff within the home would respect their end of life wishes. EVIDENCE: Residents’ wishes during illness or end of life were identified as important by the home. Any individual wishes were documented within the care plan. Residents spoken to said they felt it was a comfort to them to have been asked about this issue by the staff. They felt that the staff would respect their wishes and treat them with dignity. The documentation within the care plans confirmed that the staff had discussed this issue sensitively with the residents. JUDGEMENT – we looked at outcomes for the following standard(s): 23 The residents’ are protected and kept safe by robust policies, procedures and appropriate staff training. EVIDENCE: Staff spoken to where able to explain the homes policy on abuse and what they would do to protect the residents in their care. They also spoke about the training they had undertaken. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26, 27, 28, and 29 Residents’ bedrooms promote each individual’s independence. The communal and private areas of the home are well maintained, spacious, and clean and provide comfortable accommodation for the residents. There is a range of equipment available to assist people in their daily life. EVIDENCE: The home is maintained to a high standard and residents found their surrounding pleasant and comfortable. All bedrooms are single with en-suite toilet facilities and plenty of communal toilets and bathrooms close to the private and communal accommodation. Bedrooms have been personalised by the residents and their families and contain the persons own furniture, pictures, ornaments and photos, which made them more homely. Special equipment was seen in use throughout the home to provide assistance to the residents and to maximise their independence. Staff had received moving and handling training and safe practices were seen during the inspection. Since the last inspection 3 lounges had been decorated and new carpets and new furniture had been purchased. The dining room had had a new laminate floor laid. The manager was waiting for new equipment in the residents IT room, which included new computer equipment and the Internet was going to be installed. The residents were looking forward to this. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 34, and 36 Staff are aware of their roles and responsibilities and are able to support the residents. The home has robust recruitment policies in place to ensure the residents are protected. Staff are well supported and have appropriate supervision to enable them to provide quality care for the residents within the home. EVIDENCE: Staff have clear direction of their roles within the home, they have job descriptions, which sets out their responsibilities. The home has a low staff turn over which enables the residents to build up relationships with the staff team. The residents spoken to spoke very highly of the staff and felt they provide a good quality of care, and are kind and caring. The residents are protected by robust recruitment policies within the home and records of new staff appointed confirm this. Staff said they are well supported by the management within the home and they have regular supervision, the staff files confirmed this. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38, 39, 41, 42, and 43 The manager provided leadership and direction to staff to ensure residents received consistent, good quality care. Documentation and records are kept secure to retain the residents’ privacy and confidentiality. Health and safety polices are in place to protect the residents. EVIDENCE: The manager has completed the registered managers award and gives direct leadership to the staff. The staff said that the manager operates an open door policy and was available at any time to provide guidance and support. The manager conducts regular audits within the home including seeking residents’ opinions to improve the quality of care within the home. Records within the home are stored securely and residents felt that all information kept about them was kept confidential and their privacy maintained. The manager and maintenance person are responsible for ensuring that any hazards are identified and dealt with promptly. There were no hazards seen during this visit.
