What the care home does well

The home provides a safe, comfortable, clean environment for its residents. A resident states, "It is a 5 star home." Care practices are of a very high standard, as confirmed by residents, relatives and a healthcare professional. Documentation for all aspects of the home is excellently maintained. Each part of all documents is `tracked` and audited. This ensures that all staff know how to care, how to keep residents and themselves safe and what to do at all times. It also ensures that the provider and managers know that the service provided is always what the residents who live there want.Staff are well-supported, well-supervised, well trained and work together as a good team.

What has improved since the last inspection?

All new residents receive written confirmation that their assessed needs can be met following their initial pre-admission assessment. Medication administration is safe and all medicine records are well documented.

The home is registered to provide personal care, with nursing, for service users of both sexes whose primary needs fall within the following categories: Old age, not falling within any other category (OP) -32 Physical disability (PD) - 4 Service users in PD category must be 45 years of age or older. The maximum number of service users to be accommodated is 32. 2. 3. Date of last inspection 23rd February 2006 Brief Description of the Service: Harrowby Lodge is a privately-run care home, providing personal and nursing care for up to thirty-two older people of both sexes. It is housed within an attractive, late Victorian building with a purpose-built extension. It has been owned and run by the same person since 1987. The home is located in a quiet residential area of Grantham within half a mile of the town centre. Grantham is situated on a main trunk road and main train route. The local amenities include theatre, cinemas, pubs, leisure centre, schools and shopping centres. The home consists of fifteen single bedrooms downstairs, thirteen of which are ensuite, four upstairs and six double rooms. Communally, there are three lounges, including a conservatory and a dining room. As well as the separate ensuite facilities, there are five toilets and three bathrooms. Inside, it is pleasantly decorated throughout and residents’ bedrooms have individual styles to suit each person. A passenger lift gives access to the upper floor. The gardens are beautifully maintained to provide a secure, safe, tranquil outdoor area. There is car parking for approximately eight cars. The fees range from £425 to £525 per week. An unannounced visit was made to the home as part of a key inspection. It started at 10.15 and lasted 4½ hours. Information already held on file was used to plan the visit. The main method of inspection used is called ‘case-tracking’, which involves selecting a proportion of residents, and tracking the care they receive through the checking of records, discussion with them, the care staff and observation of care practices. Nineteen survey forms were received: eighteen from residents, some with assistance to complete them, and one from a relative. They will be analysed under each section as relevant. All were positive about the level and quality of care given. The site visit focused on whether key standards and requirements from previous inspections had been met and how the residents feel about the service provided. Four residents’ assessments and care plans were examined to ensure the health, safety and welfare of the residents is checked and that residents are allowed dignity, autonomy and choice. A partial tour of the home was made and a sample of other records examined. The inspector spoke with ten of the thirty-one residents, including those being case-tracked, a healthcare professional and four staff. The owner and deputy manager were present and assisted throughout this inspection. What the service does well: The home provides a safe, comfortable, clean environment for its residents. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. Residents receive sufficient information, prior to and after moving into the home, for them to be confident that the home can meet their needs. EVIDENCE: During the tour of the home, it was evident that each bedroom has a container on the wall in which is kept a copy of the statement of purpose, the service users guide and the resident’s own care plans. Residents’ personal files contain a contract of terms, re-enforcing what the letter says that is sent to confirm the home can meet the needs of each new resident. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Residents are well cared for and their needs are met by thorough care plan documents, good liaison with other professionals and safe medication practices. The respect and dignity afforded to all residents helps to create a happy atmosphere where residents can feel at ease and comfortable. EVIDENCE: The care plans seen are excellent, comprehensive, easy to follow documents. They provide clear instructions to ensure that all staff know how to care efficiently and effectively for the residents and keep them up-to-date with the residents’ wants and needs. They contain all the assessments, risk assessments, care plan issues, and other relevant records needed to show Harrowby Lodge Nursing Home Ltd DS0000065654.V308941.R01.S.doc Version 5.2 Page 11 what care is required and how it is to be delivered. The information and instructions to care staff part of the care plans is kept in the resident’s bedroom. The confidential part of the care plans is kept in the office and in the main contains information for nursing staff. The care plans are reviewed monthly or as often as necessary to ensure that the care given is always relevant to the residents’ needs. The ‘trail’ of how they monitor, audit and update the care plans issues and needs is exceptionally efficient. The inspector was shown how, the moment something occurs that could show a problem or concern, a separate sheet is inserted into the daily records. This is then closely monitored and if the issue becomes a regular ‘event’ or situation, a care plan issue record is instigated. This provides excellent checks of the residents’ altering or developing lives. The care plans also show clear evidence of resident or relative involvement. Three residents who spoke with the inspector confirmed this. When asked if they know what care plans are, they all gave a resounding “Yes!” One added, “I checked and signed mine only yesterday!” The care plans and resident comments demonstrate that there is good contact with community healthcare professionals. A visiting healthcare professional said, “Referrals from the home are brilliant: very speedy. They pick up on problems straight away and always contact me. There’s good liaison; they really make the effort.” In the residents’ surveys, seventeen residents agreed that they get good medical care when they need it. One added, “I’ve had good nursing care.” Another resident stated, “If we’re not well, they respond very quickly. They call the doctor straight away.” One of the nurse staff attends the MacMillan nurses meetings and study days and cascades the information gleaned to the other staff. The staff liaise with the MacMillan nurses for end of life care for residents. Because of the requirement at the previous inspection, the medication administration records were checked and a medication round observed. Each dose of medicine given was signed for after it was administered and all medication practices are now in order. Residents say they are treated with respect and dignity. Staff were heard to speak politely with residents and residents confirmed this happens at all times. The atmosphere in the home was one of cheerful calm and there was happy chatter between staff and residents, and smiles from the residents. When asked if they get the support they require, responses included, “Never had any problems.” “It’s a home, not an institution. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Sufficient autonomy and choice in all aspects of their lives is given to the residents to ensure they are comfortable and feel their wishes are respected and catered for. EVIDENCE: Activities are provided and staff spend extra time with those residents who need or prefer one-to-one contact but these could be better documented as not all social activities and contact is recorded when undertaken. Residents confirm that staff have time to spend with them and that the activities available and undertaken are to their wishes and tastes. One resident said, “Yes, but I don’t always take part. I like to sit by myself.” Others commented, “I like the activities person very much” and “I enjoy the activities very much.” The activities organiser is enthusiastic and keen to do training to enable her to provide a greater variety of activities and engage more residents in them. A large proportion of residents have regular visitors. Some of these take active part in the home’s communal activities and entertainments. Visitors say they are encouraged and welcomed into the home. “We find this is a very friendly, helpful and caring home, and that my father is very well cared for.” “The manager, qualified staff and care and support staff have always made us welcome and have supported us.” Care plans show, and residents confirm that their wishes and choices are known and catered for at all times. Residents say they are given choice in every aspect of their lives in the home. Residents say that the food at the home is good, with choice and variety. “There are always fresh vegetables and fruit available.” Comments from residents include, “Lovely”; “Not too bad”; “Always a choice, meals are home cooked and hot. Good selection of main courses and desserts”; “Nice and just the right amount.” Residents order their choice for the following day. The lunch meal observed was of two main choices with some residents having other alternatives. Residents who need assistance with their food are served in their own rooms to preserve their dignity. Staff assisting residents were seated comfortably, chatting and engaging with the residents they were helping. Food is kept in a heated container so that it is still hot when taken to each resident. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff training and the caring attitude of the provider and all the staff to the residents ensures that the residents are protected from harm or abuse and are kept safe at all times. EVIDENCE: No complaints have been received either at the home or at the commission office since the last inspection. Residents’ comments in the surveys received include, “ [I know how to make a complaint] but I don’t need to.” “No need to.” “Never had one [a complaint].” “I am very satisfied and happy here.” “I have no criticisms to make. I cannot praise the home enough. The staff are kind and caring and do what they can to make life easier for me.” “It is a 5 star home.” “I definitely feel safe here.” One relative reported that they “sometimes feel staff do not heed hoisting instructions from care plans.” However, a resident in a wheelchair who has very limited mobility and is hoisted regularly said she feels safe and comfortable when hoisted and that staff do it correctly at all times. “The manual handling is safe.” Harrowby Lodge Nursing Home Ltd DS0000065654.V308941.R01.S.doc Version 5.2 Page 15 Staff say and their records confirm that they have all received training in how to safeguard the people who live in the home and prevent the risk of harm or abuse. A healthcare professional said, “They really care about their clientele. They do their very best”. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25, 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides an attractive, comfortable, clean and safe environment for the residents who live there. EVIDENCE: At the visit to the home, all areas of the home were clean and tidy. As one resident put it, “It was a beautiful house and all the features remain: chandeliers, grandfather clock, oil painting, fireplaces. The atmosphere still remains.” Other residents’ comments in the surveys returned include, “It’s a first class hotel with room service.” “Everything is pretty well alright.” “You could not get better service at the Savoy.” Harrowby Lodge Nursing Home Ltd DS0000065654.V308941.R01.S.doc Version 5.2 Page 17 The garden is well-maintained and provides a secure, safe, pleasant area for residents to sit out in or to look at from the house. A resident said that the home is always warm: sometimes even a little too warm. Another said that the water in her room is always hot; not scalding. A relative stated, “There is a good standard of hygiene and cleanliness.” “Absolutely everything is snowy white; beds, tablecloths, pillows. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is a high level of qualified and capable staff to meet the needs of service users. Service users are protected by a robust recruitment process. EVIDENCE: All the staff agree that the staffing levels are very good at the home. On all morning shifts, there are two trained nurses as well as an “ample quota” of care staff to cater for all the residents needs. Staff say they do not feel unduly rushed in their work and have time to chat and give one-to-one attention to the residents as needed. The staff duty rotas, seen against the dependency levels of the residents, show that there are sufficient staff on duty at any time to ensure that the residents are cared for adequately. Residents say, “They answer the bells straight away.” “The staff are very good. They work equally hard: the care staff and the nurses.” “It’s very good here. We can’t complain. You wouldn’t get another home like this one.” “Staff help to reach things I can’t manage. They help whenever we need.” “They sit and chat to us and do our nails and beauty things. We are all very happy here.” Harrowby Lodge Nursing Home Ltd DS0000065654.V308941.R01.S.doc Version 5.2 Page 19 Staff say they enjoy working at the home and work as a great team, both in their groups and with one another. They say that they feel well-supported, by the provider and each other. Recruitment and induction for new staff is thorough, as is all aspects of training, both in their national vocational qualifications (NVQs) and in individual training courses. Staff say they feel valued, respected and assisted to do a good job of work. Their records contain certificates of courses undertaken and the provider has a staff matrix record to monitor where staff training needs are. Staff say that they are encouraged to take lots of training. The three care staff who spoke with the inspector have all achieved NVQs at levels 2 or 3. The nurse manager said there is a very stable workforce, most of whom have been there for years. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 36, 38 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The good maintenance and excellent monitoring of the service ensures that the home is safe and comfortable for the residents. EVIDENCE: The home is run since 1987 by an efficient and caring provider/manager who is also a trained nurse. She supports and encourages her staff, all of whom say they are happy at the home and feel she values them. The staff workforce is stable; some have worked there for as many years as the provider has owned the home and others have been there at least four years or more. Through these, she and the staff monitor and audit the service provided and put improvements into practice for the benefit of the residents and staff. All the staff, as well as residents, relatives and other stakeholders are involved and help to monitor the service. Surveys are distributed, completed and analysed. Resident meetings are held regularly. Relatives are encouraged to be involved whenever possible. All the residents spoken with and those completing the survey forms said, and relatives and others agreed, that they couldn’t think of a better care home to be in and that “everything is par-excellence”. Staff meetings are held, within their groups, which, staff say, encourages more to attend. They told the inspector that virtually all staff attend every meeting for their group and find them helpful and supportive. It was found, at the previous inspection, that the home keeps residents’ monies safely and maintains accurate records to confirm this. The provider, in the pre-inspection questionnaire, states that the home does not handle any residents’ financial matters. Staff say and their records confirm that they receive jointly clinical and personal supervision every two months. This is documented, signed for and dated on each occasion. They say they find these supportive and helpful. Maintenance records show that maintenance is regularly carried out and well documented. The inspection reports from the fire and environmental health officers show that the home is maintained safely and hygienically for the benefit of both residents and staff. The pre-inspection questionnaire completed prior to the inspector’s visit states that all policies and procedures and records are in place and up-to-date.
