Harvey House also offers a rehabilitation service for upto eight individuals who need to re-gain skills before returning home. The forty single bedrooms are without en-suite facilities. The home has a enclosed garden, with seating and mature plants and trees, an aviary is the centre piece of the garden. The premise is owned by Leicestershire County Council Social Services Department and is situated in the village of Barwell which offers local shops, Churches and cafeterias. The home is easily accessible by private or public transport. Accommodation is provided over two floors with access between floors being via stairs or a passenger lift. This is an overview of what the inspector found during the inspection. This Inspection took place between 9.00am and 2.00pm. The opportunity was taken to look around the home, spend time with residents, view records and policies which, included viewing the care plans of three residents residing at the home, and one individual accessing the rehabilitation unit. Information recorded on the pre-inspection questionnaire and resident and relative Comment Cards has been incorporated into the Inspection report. The Registered Manager facilitated the Inspection, spending time with the Inspector; the Inspector also interviewed two members of care staff. What the service does well: What has improved since the last inspection?
The Statement of Purpose has been revised since the last Inspection; ensuring information about care provision is up to date. An additional booklet is available to those individuals accessing the rehabilitation unit. Staff have recently received training in the handling of medication, and its administration by a Pharmacist. Care staff have been allocated dedicated areas within the home to which they are responsible for the answering of calls bells. An Activity Organiser has been recruited, who has consulted with residents and their relatives so as to gain information as to residents interests and hobbies. Consultation has taken place through letters incorporating questionnaires and through meetings with residents and their relatives.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3 and 6. The admission process is well managed with residents needs being assessed ensuring staff have the appropriate information to meet individual needs. EVIDENCE: Harvey House has a Statement of Purpose, which outlines the care and accommodation offered to residents, arrangements for staffing including staff training, the daily living arrangements offered to residents including information on mealtimes. A separate booklet is available for those individuals accessing the rehabilitation unit. As part of the assessment process residents and relatives are asked to provide a history of their life prior to admission, this provides additional information to help in the delivery of care and activities provided. Information supplied could be used to a greater advantage in the developing of resident care plans. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9 and 10. Residents are looked after well in respect of their health and personal care needs. EVIDENCE: Care plans of three residents’ were viewed, which contain an outline of resident needs. Care plans could be further developed to encompass a comprehensive record of resident needs, with consideration being given to physical health, mental health and socialisation including hobbies and interests, thus providing a holistic approach to care. The records of one individual accessing rehabilitation was viewed, these contained assessments and an individual plan of care, which detailed goals, needed to be achieved, consistent with independent living. The medication and administration records of those residents whose records were viewed were inspected and found to be in good order. Staff were aware of the promotion of residents privacy and dignity, this was demonstrated through comments made to the Inspector and the Inspectors observation of staff interacting with residents. The Inspector received nine completed comment cards from residents, all indicated they were well cared for and that their privacy was respected and that they were treated well be staff. The Inspector received nine completed comment cards from relatives/visitors, eight confirmed they were happy with the overall care provided. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 14 and 15. Residents experience a homely life style and visitors are encouraged to visit. Various formal and informal activities are made available meeting residents’ recreational needs. EVIDENCE: An Activity Organiser has been appointed, since her appointment she has consulted with residents and written to their relatives to identify hobbies and interests of residents. Consultation has also taken place with residents General Practitioners, consistent with health promotion and safety. A meeting has been organised for residents and relatives to meet the Activity Organiser and discuss the provision of activities. Residents of Harvey House already have the opportunity to participate on a monthly basis, with entertainers visiting the home, who provide a variety of entertainment including keyboard playing and singing. Church and Communion services are also held within the home. A group of residents in the home attend local Churches. On a monthly basis residents also have the opportunity for an outing, visits have included Woodlands Garden Centre, picnics in Cadeby, Bosworth Water Park and Riversley Park in Nuneaton. The Registered Manager felt that the restrictions placed by external transport provision impacted on resident’s enjoyment of outings. It is recommended that the use of Social Services transport be explored for the benefit of residents. The Inspector received nine completed comment cards from residents, of which seven indicated they were happy with the food provided. The Inspector spoke with three residents, all of which confirmed their satisfaction with meals, one gentleman in particular was very happy with the choice and variety offered at mealtimes. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16. Complaints are handled objectively and residents are confident that their concerns would be listened to, taken seriously and acted upon. EVIDENCE: Harvey House has received one minor complaint since the last Inspection, which took place in April 2005, the complaint and the outcome of the investigation was documented. The Commission of Social Care Inspection has not received any complaints in relation to Harvey House. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24 and 26. A comfortable and safe standard of accommodation is provided for the resident’s including garden areas, which individually and collectively meet the resident’s needs. EVIDENCE: Harvey House is well maintained and is suitable to the needs of residents; it is decorated and furnished to a standard that creates a comfortable and homely environment. The garden is well maintained which benefits from mature trees and plants, the garden provides garden furniture, in order that residents can relax and look at the birds in the aviary, this being the centre piece of the garden. Harvey House has spacious sitting rooms on both the ground and first floor, which offers residents a variety of choices as to where to sit, one sitting room is provided for those individuals who wish to smoke.
Harvey House C51 C01 S32572 Harvey House V242480 150805 Stage 4.doc Version 1.40 Page 16 The Inspector viewed the bedrooms of individuals whose records and care plans had been read, all were furnished and decorated to a good standard, one bedroom had an odour of urine, the Registered Manager confirmed that conversations were already taking place with the resident and their relatives about the replacement of the carpet. The Registered Manager outlined environmental improvements, which would be taking place during this financial year; these improvements include the purchasing of new dining tables and chairs, decoration to the main stairs and hallway and the decoration of the ground floor. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30. Staff at the home are well trained and supported, and employed in sufficient numbers to meet the needs of residents. EVIDENCE: The pre-inspection questionnaire indicates that Harvey House employs twenty one members of care staff, of which thirteen have attained a level 2 National Vocational Qualification; this level of achievement is to be commended. The Inspector spoke with two members of staff, both of which have attained a level 2 National Vocational Qualification. Staff spoken with had a clear understanding of resident needs, and confirmed they were responsible for the devising and reviewing of care plans. Staff had not received formalised training in care plan writing. Staff have received training in a variety of topics, including fire awareness, moving and handling, first aid to name some. The Departmental Training Plan details training available to care staff in care plan and record writing and medical conditions including physical and mental health. Care staff should therefore be encouraged to access these courses to increase their awareness and develop service user care. Harvey House C51 C01 S32572 Harvey House V242480 150805 Stage 4.doc Version 1.40 Page 18 The Inspector discussed with the two care staff interviewed, the use of care plans and daily records and how these could be developed to encompass all aspects of care, and contain a greater level of detail and information. Staff interviewed confirmed the staffing levels, indicating that there were generally five care staff on duty in the morning, with two or three for the afternoon and three for the evening. Care staff hours were consistent for all days of the week, the one difference being a laundry assistant was not on duty at weekends, and therefore this became a care staff duty. The records of two members of staff recruited since the last Inspection were viewed and were found to contain all relevant documentation including a Criminal Record Bureau check. The Inspector received nine completed comment cards from relatives/visitors, of which five indicated that in there opinion there were not always sufficient staff on duty, of which two comment cards made an additional comment. ‘The main concern of this home is the shortage of staff sometimes when you need help you can wait up to 15 minutes. The Registered Manager and care staff interviewed confirmed that staff have now been allocated specific areas within the home for which they are responsible, this has improved the response to resident call bells. ‘On a number of occasions the doorbell is not answered, I can only assume the staff are too busy to be able to operate the entrance lock’. The Registered Manager advised that the current pagers allocated to staff do not indicate that the front door bell has been rung; staff pagers are to be altered to indicate the ringing of the front door bell. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 35, 36 and 38. The Registered Manager offers a clear sense of leadership, which reflects on the day-to-day delivery of care practices of residents and running of the home. EVIDENCE: The pre-inspection questionnaire completed by the Registered Manager detailed the regular maintenance and checks pertaining to the environment, which includes fire equipment and fire drills. The two members of staff spoken with confirmed that they receive regular one to one supervisions with a member of the management team, supervisions have not been as frequent as usual due to management availability, care staff confirmed that any issue of concern they had could be raised with the person in charge on a day to day basis.

Records indicated that financial arrangements were managed in a variety of ways, by the resident themselves or by their relatives. On the day of the Inspection a resident was observed being supported in raising concerns as to the fees charged for his residential placement.
