What the care home does well

The service provides training and support for staff in the home. The acting manager is completing audits of medication. The food, fluid and turn charts are being completed by staff. People are referred to the appropriate health professionals when necessary.

The acting manager is aware that some care plans need to be improved so that people receive consistent care.

It is a two-storey building, surrounded by safe, well maintained enclosed gardens, and is situated in the suburbs of Cambridge close to local amenities and shops. Public transport to the city of Cambridge is readily available. Hinton Grange Care Home provides 52 single and 4 double occupancy bedrooms some of which have en-suite facilities. There are 4 separate bathrooms, 4 separate shower facilities and ten toilets. The home is on two floors. The ground floor has the extra care unit, which caters for residents with dementia or other mental health diagnoses. The first floor can be accessed by lift or stairs and provides residential and nursing care. Carers and nurses provide 24hr care in the home. Copies of inspection reports are kept in the foyer at Hinton Grange. The cost of care at Hinton Grange is available from the office on request. Care Homes for Older People Page 3 of 8 What we found: We, the Care Quality Commission (CQC) carried out a random inspection of Hinton Grange on Monday 21st December 2009 at 10:40 hrs. This inspection was completed by two inspectors to check that requirements made during the key inspection on 4th August 2009 and compliance visit on 17th August 2009 had been met. We looked at the care plans for people living in the home and found that the information provided did not always give a clear picture of their needs. One person had information in a paper file that was incorrect about pain they was supposed to be suffering from. When we spoke to the person they stated they did not have any pain just itching. The person said they saw the GP regularly and were very happy with the care they received in the home. There was no care plan relating to medication for another person where it would have been useful to link with evaluation of pain and the use of medication for this. The service improvement director showed us a care plan that had been updated and written in a very personalised way with much greater detail and said this was how all care plans were expected to be written but it was a work in progress. Staff were very aware of who was on a food, fluid or turn chart and the reasons for them. Eight food/fluid charts,4 turn charts and five dressing plans were seen and all had been completed as required. The service improvement director stated that the night nurse totals the fluid intake and records this on the computer system used in the home. Medication Administration Record (MAR) charts were seen. One had been handwritten because the person was a new admission and the local chemist had not yet provided blister packs and typed MAR charts. The handwritten prescription details had been signed by two staff and there were sample signatures at the front of the register. The acting manager had not provided a sample signature even though she had administered some medications. One person had paracetamol prescribed as two tablets, three times a day (soluble) maximum two at any one time to a maximum of eight in a day. The MAR chart showed the person had been written for it to be given four times a day although this had never happened as the person often refused paracetamol. There was inconsistent recording of the number of tablets when they were given as some staff had written 2 x 50 mg and others 1g. The acting manager said this would be discussed with staff and she would ensure staff recorded in a consistent way. Another person was prescribed one or two senna tablets but there was no information about when either dose should be given. The MAR chart showed that staff always administered two tablets and the nurse in charge was unable to say why, although they said that there was a bowel chart but not how this informed the administration of one or two tablets. The outcomes for people being administered the medications above was not compromised at this time, however the acting manager needs to ensure accurate Care Homes for Older People Page 4 of 8 recording of medication administration in the audits being undertaken. Staff administering medication locked trolleys appropriately and gave other medication as required. Other MAR charts were seen and had been completed well. This meets the requirements and means the service has complied with the statutory notice. When staff are writing in care plans and daily notes they need to understand the difference between the use of him and her, which when used inappropriately can confuse what is being said and of whom.
