This inspection was carried out on 11th November 2008.

CSCI found this care home to be providing an Adequate service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 6 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The registered manager has made alot of improvements since she has been in post. People who use the service appear to have their personal care given to a good standard. Staff appear caring, and appear to do their best to ensure the people using the service are cared for. The service caters for some people who are difficult to place in other services, and people`s needs appear to be met to a good standard. In regard to the statutory requirements issued at the previous key inspection, the following progress has been made: (1) Care plans- these appear to be completed and reviewed to a satisfactory standard, for the people we case tracked on this inspection. (2)Medication- The registered manager said she has set up a system of regular audit to ensure medication is managed to a good standard. On this inspection we did not audit the system (due to time constraints), but will do this at the next key inspection. (3) Social activities- An activities co-ordinator is now in post and organises activities each week day afternoon. (4) Statutory notifications- The registered persons are notifying CSCI as necessary, according to the regulations. (5) Training and development- The registered manager said staff are receiving the training required according to regulation (for example fire safety, food hygiene, first aid, infection control, manual handling) and according to the needs of the people using the service (e.g. dementia, epilepsy). The registered manager said nurses are receiving specialist training, and have developed appropriate links to enable nursing staff to update their knowledge and skills (e.g. tissue viability). A schedule of what training people have received is maintained, and certificates are maintained on individual staff files. Due to time constraints we did not complete a full audit of training, but will complete this on the next key inspection. (6) The home`s environment- there has been an improvement in the decorations inside and outside the home. For example the outside of the home has been painted. Some work still needs to be completed for example decoration and upgrading of some of the communal rooms in the downstairs of the home (e.g. chapel, hair salon, smoking room, downstairs toilet). (7)There is a satisfactory system to ensure the prevention of legionella. There is an up to date gas certificate, and there are suitable records that fire equipment is being tested.

What the care home could do better:

We still think some further improvements are required. These are primarily in relation to the fabric of the building: For example: (1) Bathroom facilities on the first floor are inadequate. There is one communal bathroom at one end of the building, but there are no specialist facilities at the other end of the building. This results in people who use the service needing to travel an extensive distance to use a specialist bath. There is a plan to have an additional shower facility, but this has not been implemented, and is not in the area where facilities are currently lacking. Many bedrooms have an `ensuite` facility but these facilities are not really suitable for the frail-elderly. The previous requirement is therefore renotified. (2) Further work still needs to occur to upgrade parts of the building. For example the roof leaks intermittently, and may need to be replaced. One area of communal rooms on the ground floor still needs to be redecorated (e.g. smoking room / chapel/ hair salon) (3) It is not clear whether thermostatic valves are fitted on all baths (in line with Health and Safety Executive guidelines).

The registered provider is Care Homes for Older People Page 2 of 12 Newport Care Limited. The registered manager is Ms Sherran Thompson. Holywell Bay care home is situated in the village of Holywell Bay, which is on the north coast of Cornwall near the town of Newquay. The home has pleasant views of the sand dunes, golf course and sea. The home is a large two-storey property which has been extended. There is a choice of communal areas for example a large dining area, which also has a seating area, a large lounge, a TV lounge, and a smoking room. There are also other rooms which the registered manager said the provider is planning to redecorate so they can be used. The home also has an enclosed garden which residents can use. At the time of the inspection fees were between #550 and #2000 per week. Additional charges are made for hairdressing, newspapers and personal items. Care Homes for Older People Page 3 of 12 What we found: We completed this inspection to check progress regarding the statutory requirements issued at the key inspection which took place on the 24th April 2008. We met with the registered manager, and also inspected the premises. We discussed with the registered manager what action had taken place regarding the statutory requirements. We looked at some records to verify what was discussed. When we looked around the care home the premises was clean, warm and appeared generally well maintained. People using the service appeared clean, happy and well cared for. There appeared to be suitable numbers of staff on duty. The home must have suitable 01/10/2008 bathing facilities, in sufficient numbers to meet the needs of people accommodated in the home. Bathing facilities must be suitably adapted to meet the needs of the elderly and disabled. Bathing facilities must be within a suitable distance to the bedrooms of people who use the service. (b) Training regarding people with challenging / aggressive behaviour, dementia and mental health needs. (c)Suitable records of training e.g. NVQ, induction training and other training certificates need to be maintained and available for inspection. The registered persons must submit to the commission: (a)A training policy which outlines what training staff will receive, and when. This should be forwarded to CSCI as soon as possible. (b) A plan outlining how the registered persons will ensure all staff receive training required in the report. This must include a current `audit? of what training people have received, and when the required training will be delivered. (c) An update of the delivery of staff training on 1st October 2008 Care Homes for Older People Page 7 of 12 regarding all staff. 6 38 13, 23 The registered persons must 01/10/2008 improve health and safety standards in the following areas: Ensure a risk assessment is completed regarding the prevention of legionella, and any necessary preventative measures are taken to prevent Legionnaires? disease. Emergency lighting and fire doors must be tested at intervals prescribed by the fire authority. Gas appliances must be serviced. Thermostatic valves must be fitted, where appropriate, to wash hand basins and bathsin line with HSE guidelines. Appropriate evidence must be forwarded to the Commission within the timescale set.
