What the care home does well

Residents are cared for in a safe, well-maintained, homely environment by staff who are aware of their needs. Care plans identify residents` needs in detail and this helps staff to provide consistent care. People who use the service are happy with the care they receive.

What has improved since the last inspection?

The home has redecorated a bathroom and fitted a new bath unit. Ongoing work is being carried out to fit water temperature control valves to basins in individual residents bedrooms. The corridor areas of the room had been redecorated. All requirements made at the last inspection have been addressed.

What the care home could do better:

The home is recommended to examine its procedure for the formal reviews of the residents to ensure the procedures and the standard of the recording meets the National Minimum Standards.

Within the total number registered - one service user can be accommodated within the OP category aged 62 years and over. Date of last inspection Brief Description of the Service: Homer Lodge is a care home, which provides both personal care and nursing care for older people. It is a purpose-built home situated in a residential area close to the centre of the City of Lincoln and is within walking distance of local shops and facilities. The home is situated off a main bus route and there is off street car parking to the front of the home. The homes own car park has limited space at the rear of the home and alternatively there is public car park nearby. Accommodation is provided in 15 single and 10 twin bedrooms on the ground, first and second floor levels. Rooms to the first and second floor are served by a lift. The home is in a residential part of the city, it does not have a garden area, but there is a block-paved terrace leading out of the ground floor lounge which is accessible by wheelchairs and service users are able to sit. This homes service users guide states that, “Homer Lodge aims to create and maintain a happy, safe and homely environment and to establish a partnership with service users in planning and implementing their care.” Homer Lodge Care Centre DS0000002609.V276785.R01.S.doc Version 5.1 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The inspection took place between 8.45am and 3.00pm. The main method of inspection used is called case tracking which involved selecting three residents and tracking the care they receive through the checking of their records, discussion with them and care staff and observation of care practices. A tour of the premises was conducted and care records were inspected. Two members of staff and three residents were interviewed. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3 & 6 There are satisfactory procedures for the introduction and assessment of people to the service, ensuring that care needs are met. Residents are happy with the care provided. EVIDENCE: The home has a statement of purpose and service user guide which enables residents to make an informed choice whether their needs can be met by the resources of the care home. The deputy manager stated that residents are given a copy of the terms and conditions for their stay at the home. Residents confirmed that they had a copy of this document. The inspection of the care records for residents being case tracked provided evidence that full assessments had been carried out prior to them coming to stay at the care home. Terms and conditions were found to be on individual residents files. The residents interviewed were able to discuss their terms and conditions and state the cost of their stay at the home. The deputy manager stated all new residents are encouraged to visit the home prior to making any decision to stay there. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 &11 Care plans identify all areas of need and provide detailed care instructions for staff; this enables staff to provide appropriate care. Residents’ health needs were being met. EVIDENCE: Each resident has an individual plan which contains information relating to his or her care needs. Care plans are reviewed daily and formerly reviewed by the key worker on a monthly basis. Plans included risk assessments to monitor areas such as nutritional needs, the risk of developing pressure damage and manual handling. Individual residents medical conditions and personal likes and dislikes were documented to ensure that staff were aware of individual care needs. Residents’ health needs were being met. Visits by doctors, chiropodists, dieticians and physiotherapists were recorded on their files. They confirmed that in their opinion they these were being met by the care home. Interviews with residents provided further evidence that community healthcare services visit the home on a regular basis. One resident returning home after intermediate care stated “district nurses have visited me to confirm the support they will be giving to me when I return home”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 &14 The home provides a range of leisure and social activities. Contact with relatives and friends is encouraged. EVIDENCE: A member of staff interviewed stated that she acted as the homes activity coordinator. She stated that residents are provided with a range of activity within the home and taken out into the community. One of the residents being case tracked stated that she had been taken to local restaurants for a meal. The activity coordinator stated “we organise bingo, pub lunches and entertainers who visit the home.” One residents stated “I love playing bingo and liked to read quietly in my room or in a lounge. A hairdresser visits the home every two weeks.” The home has a visiting policy which meets the National Minimum Standards. Residents stated friends and family are encouraged to visit the home and are made very welcome. The visiting policy of the home is displayed in the foyer. The religion of each resident is recorded in their individual file. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17,18 The home has robust procedures for handling allegations of adult abuse. Staff had received abuse training and were clear on the action to take in the event of this occurring. EVIDENCE: The home’s adult protection policy is in line with current local guidelines. A copy of the Lincolnshire County Council Adult Protection Procedures was included in the homes policy and procedures manual. Staff interviewed stated that they had read and understood both documents. Staff had a good knowledge of the types of abuse that could occur and the actions that they must take if they had any concerns. The deputy manager had a good understanding of procedures to follow regarding reporting any suspected abuse to the Commission for Social Care Inspection and Social Services. Staff comments and training records demonstrated that staff had received appropriate training in this subject which would help them to recognise and take appropriate action should the need arise. The homes complaints procedure was discussed in the formal interviews. Staff were familiar with this policy and stated it was displayed in the foyer of the care home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 &26 The home is well maintained and decorated. It is homely and comfortable, odour free, and was found to be cleaned to a high standard throughout. There are appropriate aids and adaptations provided in the home to maintain residents’ independence. EVIDENCE: A tour was made of the care home. All areas were clean and smelt fresh. Ongoing maintenance has been carried out in the care home since the last inspection. A number of bedrooms and corridors have been decorated. A bathroom has been fitted with a new bath, decorated and tiles replaced. There is ongoing work being carried out on the hot water supply to residents bedrooms. There is a rolling programme to fit temperature controlled valves to all basins in bedrooms. The deputy manager stated this process has almost been completed. Residence stated their satisfaction with the environment of the care home. Homer Lodge Care Centre DS0000002609.V276785.R01.S.doc Version 5.1 Page 15 A resident commented “the home itself is nice, I like my bedroom the lounges are comfortable and you can use a small lounge if you want some quiet time.” There were no health and safety issues identified at this inspection. Staff stated that they felt safe working in the care home. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29 & 30 There are sufficient staff on duty to meet the needs of the service users and staff have the skills and experience necessary to carry out their roles. The recruitment policy of the care home is being followed. EVIDENCE: Staff interviewed confirmed that they feel there is always sufficient staff on duty to meet the needs of residents. They stated that the staffing rota is always met and there is additional staff brought on duty if the need arises. A resident stated, “ staff here are top grade, you could not find better, you are looked after very well.” The recruitment records for two members of staff were examined. Their files contained all of the information set out in the Care Home Regulations, which must be obtained before a new member of staff commences employment. The home has a training plan in place for the next 12 months. Staff confirmed in their formal interviews that training is being offered which is linked into supervision and appraisals. One member of staff stated “ I feel that I have been provided with training to ensure I have the skills to meet the needs of the residents of the home.” Homer Lodge Care Centre DS0000002609.V276785.R01.S.doc Version 5.1 Page 17 Management and Administration The intended outcomes for Standards 31 – 38 are: 31. 32. 33. 34. 35. 36. 37. 38. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33 & 38 There is good leadership and guidance for staff, ensuring that service users receive consistent quality care. The environment is safe. The home is run in the best interest of residents. EVIDENCE: Staff stated the registered manager is very supportive. A staff member stated that “there is a very good understanding between the registered manager and all members of staff. Shes very fair but has very clear expectations of the standard of care practice which she expects to be carried out within the care home. If she is not happy with any aspect of the practice of the home she will bring it to the attention of the staff member immediately but in a fair and professional manner”. The care records, discussion with staff and residents provided evidence that the home is run in the best interest of the residents. Care plan instructions state that staff when providing care must always consider the privacy and dignity of the residents and take into account their wishes and choices.
