Conditions of registration: Two named individuals over the age of 65 Date of last inspection 5/01/2005 Brief Description of the Service: Jasmine House provides a long term home to six service users with a learning disability, and is owned and operated by the local authority. Some of the service users have associated physical disabilities, and mental health issues, and one service user is now over the age of 65.The home is situated close to the centre of Ely, and within walking distance of the city centre. Service users are encouraged to use the nearby local facilities.The accommodation is on two floors, with one ground floor bedroom, and communal facilities and the remaining five bedrooms on the first floor. This is an overview of what the inspector found during the inspection. This was the first inspection of the current year, and an unannounced visit. The service users in the home all lead active community based lives, and on the day of the inspection, were all out and about. As a result, a second visit was arranged, at which time it was possible to meet three of the service users, and discuss their views on the service. These views are incorporated in this report, along with observations made during a staff meeting, which was being held at the time of the first inspection. Cambridge County Council operates the home, and the home therefore follows all the county council policies and procedures. What the service does well: What has improved since the last inspection?
Since the last inspection, the statement of purpose and service user guide have been revised, and meet the requirements of the regulations. In addition further assessment work has been carried out, looking specifically at the daily activities undertaken by service users. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Policies and procedures are in place to ensure that any service user moving to the home has had a detailed assessment completed, and is clear that the home is able to meet the assessed needs. EVIDENCE: The home provides long-term accommodation for the service users living there, but following the death of a service user earlier this year, currently has a vacancy. A policy is in place to manage the admission of a new service user, and this was discussed with the manager. An initial referral from the local learning disability team would be followed by an assessment of the persons needs by the home manager, with a view also taken on the compatibility of a new admission with the other service users in the home. Initial visits would be arranged, followed by a longer visit before a final decision would be made by all parties involved in the process, including the service users and their family. A prospective new admission has been identified, and the home is in the early stages of assessing whether they are able to meet this persons needs, before looking at the wider issues of compatibility. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7 and 9 Detailed care plans and risk assessments are in place and regularly reviewed, ensuring that all staff are aware how to meet the individual needs of the service users, and able to offer appropriate support in making decisions affecting their everyday lives. EVIDENCE: Each of the service users in the home has a detailed care plan, ensuring that staff are aware of their needs, and how these needs should be met. These care plans were seen during the course of the inspection, and discussed with the manager. Service users are supported in making choices regarding all aspects of their lives, for example, discussions are being held with service users who currently attend a local day service in, and the intention is to provide activities during the day, which the service users have chosen. These discussions have also identified the need for one of the service users to retain the structure that attending the day centre gives, to try and change things completely, would be very difficult for the service user to cope with. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, and 16 Evidence clearly demonstrated that service users are able to use local community facilities as well as more specialist services, and the activities available are appropriate, and kept under review. EVIDENCE: As has been indicated above, service users are encouraged to take part in activities appropriate to their age and culture, as well as activities in their local community. Day service activities are being revised, to offer a more community-based programme. The manager and staff team are all working to ensure that service users are supported in choosing suitable activities, and still have the opportunity to look at new experiences. During the second part of the inspection, the inspector was able to talk to three of the service users who indicated that they are generally happy with the service and choice offered, although one of the service users would prefer to move back to the Huntingdon area, where she lived previously.

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 and 19 The initial assessments and care plans ensure that service users health, physical, emotional and personal support needs are identified and met in an appropriate manner. EVIDENCE: Service users health and personal care needs are identified through the assessment and review process, to ensure that needs can be met. The home offers support to the service users in an appropriate way, and the service users spoken to during the inspection were happy with the service they receive, and the way in which it is delivered, and have a good relationship with the staff team in the home. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23 Although service users felt happy their comments and concerns would be listened to, insufficient information is available within the policies to enable them to raise these concerns with the CSCI if they wished to. EVIDENCE: Service users spoken to during the inspection felt happy that their views are listened to, and any concerns they have taken seriously. One service user identified issues, and these were discussed with the manager who was able to clearly show that the concerns are being listened to, and will be addressed where possible. The complaints procedure in the home remains an issue, but is apparently being addressed, and a new procedure being written. The home operates under the county council policies and procedures, but needs to ensure these to not bring them into conflict with the CSCI or care standards regulations. Staff have all received adult protection training through the local authority, and are aware of procedures. Any new staff will receive training as part of their induction. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 and 30 The home offers a very suitable environment, in which service users contribute to the daily activities, such as cooking and cleaning. EVIDENCE: Jasmine house offers a clean and comfortable home, on a very domestic scale, which is very appropriate to the needs of the service users living there. There are sufficient communal areas, which allow service users he chance to spend time quietly if hey choose, or relax in the kitchen joining in or watching the preparation of meals. Individual rooms are decorated to individual choice, and service users have the opportunity to bring personal items with them. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 The home employs sufficient staff at present to meet the needs of the service users, with a variety of training available. EVIDENCE: Staff all receive training both internally and through county council courses, and a detailed induction programme is available for new starters. Statutory training requirements are updated annually. The home’s manager is currently completing an NVQ assessor’s course, which will make NVQ training easier to manage. Staff have also received training specific to individual service users needs, for example around dietary issues. Staffing levels are suited to the service users living in the home but should be reviewed before any new admission takes place. JUDGEMENT – we looked at outcomes for standard(s) 39 and 42 Policies and procedures are in place to ensure the health and safety of service users and staff is protected at all times. EVIDENCE: As has already been indicated, discussions were held with three service users as part of the inspection, and the views of staff members at a team meeting were also observed. It was clear from the discussion that the views and wishes of service users are listened to, and reflected in the support provided. The home is owned and operated by Cambridgeshire County Council, and has all the health and safety policies in place to ensure the protection of service users and staff.
