This inspection was carried out on 29th July 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Kimblesworth Care Home provides a homely and caring environment for the people who live there. The home has a settled and committed staff team who have developed close relationships with the people living in the home and know them well. Service users commented that they liked living there and that the staff are kind, treat them well and work hard.

What has improved since the last inspection?

As required by CSCI in the last inspection report, service users have now been provided with a lockable facility within their bedrooms where they can keep their valuables.

What the care home could do better:

The flooring in some of the bathrooms and toilet areas were showing signs of wear and tear. These should be reviewed and replacement considered. The manager needs to continue with his management qualification to ensure that it is completed within the timescale.

The home provides 24- hour personal care (with nursing) for twenty service users with mental health needs who are over sixtyfive years of age. Accommodation is provided on two levels, with a choice of lounges and dining room and other quiet areas for use by the service users. The staff team within the home comprises registered nurses (RNM) and care staff and supported by a team of ancillary staff i.e. domestic staff, handyman and kitchen staff. CSCI has a statutory duty to inspect all care homes at least twice a year. This unannounced inspection was carried out in accordance with this duty. The inspection took place on the 29th July 2005. Records were examined and a tour of the building took place. Time was spent talking to 6 service users and 4 staff. No relatives visited during the course of this inspection. In line with current CSCI policy on Proportionality, the inspection focused upon a number of key standard outcomes for service users. The key standard outcomes not inspected on this occasion will be raised during the next inspection of the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4&5 Admissions to the home are well managed. Service users receive appropriate information about the home and their needs are assessed prior to admission. EVIDENCE: The home has an up to date statement of Purpose and Service User Guide. Copies of these were seen, together with a copy of the CSCI’s most recent inspection report in the foyer of the home. These documents are made available to prospective service users to ensure that they have the information required to make a decision about going to live there. Statements of Terms and Conditions were seen on service user files. Assessment information was examined for three people. This information contained care manager assessments and the home’s own pre admission assessment document, which is completed by the manager when he visits the prospective service user prior to admission to the home, to ensure all of the Kimblesworth Care Home B54 S43604 Kimblesworth V219850 290605 Stage 4.doc Version 1.30 Page 9 care needs can be met. One service user spoken to confirmed that he had visited the home prior to admission and had chosen his room. The home does not provide intermediate care and therefore assessment of Standard 6 is not required. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) These standards/outcomes area were not assessed during the course of this inspection. They will be examined at the next inspection EVIDENCE: This standard area was not fully assessed on this occasion. However a check was made of the medication administration records to ensure that the requirement highlighted in the previous inspection report had been met. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17&18 The home has a satisfactory complaints and protection of vulnerable adult procedure in place. Service users have information on how to access advocacy services. EVIDENCE: A suitable complaints procedure was in place, with copies displayed in the foyer and in the service user guide. Service users confirmed during discussion that the manager is approachable and that any concerns raised with him are dealt with promptly. There were no complaints recorded since the last inspection. Information was available within the home of how service users could access advocacy services if they needed independent support or someone to speak on their behalf. Staff confirmed during discussion that they had received training in the Protection of Vulnerable Adult Procedures and would know the action to take should any form of abuse be reported or suspected. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25 &26 The home is a converted school and as such does not meet all of the National Minimum Standards with regard to environment. However it is safe and well maintained and meets the needs of the people who currently live there. EVIDENCE: Kimblesworth Care Home meets the standards for homes registered before 1st April 2002. The Registered Manager advised the inspector that he was aware of the areas where the home does not meet the National Minimum Standards in particular with regard to some corridor widths and some upstairs rooms. When assessing a service user prior to admission and only admit the person if the room available was considered suitable. During the tour of the building and whilst talking to people in their bedrooms, the inspector saw that service users could bring in their own furniture and belongings should they wish to do so. Bedrooms have now been provided with a lockable facility where service users can keep their valuables. The building was found to be clean, tidy and free from offensive odours. However some of the flooring in bathrooms and toilets were showing signs of wear and tear and should be considered for replacement . A number of health & safety and maintenance files were examined and found to be in order. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29&30 The home is staffed with adequate numbers of staff who are trained and competent to do their job. The procedures for the recruitment of staff are robust and offer protection for the people who live there. EVIDENCE: The home is staffed with a qualified nurse on 24 hours per day. There are also 3 or 4 carers on duty throughout the waking day and one carer at night. Additional staff include an activities organiser and catering, domestic and laundry staff. The staff files reviewed indicated that all of the required recruitment checks are made before a new member of staff starts to work in the home. After talking to staff and reviewing records it was confirmed that recent training has taken place in the areas of care planning, nutrition, food hygiene, fire safety, first aid, moving and handling and ‘Protection of Vulnerable Adults. The manger advised that two care staff have recently completed a TOPPS course. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. They will be examined at the next inspection EVIDENCE: This standard area was not assessed during the course of this inspection, however the Registered manager advised that he has commenced but not yet completed an appropriate management course. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP 31 Regulation 8&9 Requirement The manager must complete a management-training course at NVQ level 4 or equivalent. Timescale for action 31/12/05 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. No. 1.
