The inspector found no outstanding requirements from the previous inspection report, but made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The acting manager was co-operating with health and social care agencies to improve the service for people living at the home.

What the care home could do better:

The provider needs to review the duties of staff and maintain the required number of staff to meet the ongoing needs of residents at the home. A requirement is made for the provider to consider whether the medication administartion system in operation at the home is safe. The manager needs to keep families and staff informed about action taken to meet newly identified needs of people using the service.

Date of last inspection Brief description of the care home Kings Lodge provides personal care and support for up to 21 people over the age of 65 years who have a dementia related condition. The home is situated on the outskirts of Higham Ferrers, with public transport links to neighbouring towns. The home is a conversion of a large residential property, with 2 larger communal lounges and 2 smaller sitting rooms. Bedrooms are located on the ground and first floors and a shaft lift provides access to the first floor. There is a garden at the rear of the property, Care Homes for Older People Page 2 of 8 which contains an aviary. The home has been in the hands of the current owners since 2004. Care Homes for Older People Page 3 of 8 What we found: The reason for this inspection was a concern raised by an anonymous complainant as follows: That a resident is at risk of falls. There are no risk assessments completed in respect of this resident. The resident requires 1-1 support but there are only three staff. They consider that management are not doing enough to support. Carers have been advised to lock the resident in their room. That medication is not being signed for and that medication is missing. Cook has no qualifications and the food is not edible. People who use the service are not happy. People using the service are covered in bruises. Not enough staff. We met with the acting Manager who was willing and co-operative with the investigation. Three staff and the acting manager were on duty for sixteen resident. One carer was in the kitchen cooking the mid-day meal. One resident was case tracked to review the care plan, supporting risk assessments and medication administration records. In reviewing the care plan and in discussion with staff it was evident that the needs of the person being case tracked were being met by the staff team. The individual was however in need of a healthcare assessment to investigate if there was an underlying physical need for the falls and to review their present medication. The doctor was addressing this but there was a delay in obtaining specialist healthcare support. In reviewing the medication records it was evident that there had been many changes to the medication for the resident being case tracked. Not all of these changes had written confirmation from the doctor, with staff relying verbal directions. This practice is unsafe. In discussion with the acting manager they informed the inspector that they were reviewing how the medication was ordered, stored and administered. They were considering a move from a monitored dosage system to having individual medication in their original containers. One carer was undertaking the role of cook at the time of the inspection and this was reducing the number of staff available for direct care. In discussion with staff it was evident that there had been issues with an untrained member of staff at weekends but this was being addressed. In reviewing healthcare needs it was evident that the resident was being referred appropriately for healthcare support, however this was not always conveyed adequately to families and staff. This was creating poor communication and raising anxieties. Risk assessments had been completed for the individual being case tracked and additional staff had been brought in to maintain observation during the day. It was observed during the inspection that even this did not prevent all falls. The relative of the resident was distressed about the falls and felt that the system was failing their relative. Care Homes for Older People Page 4 of 8 The position of cook is vacant at present and temporary cover was in operation. This was short term and a new member of staff was due to commence their duties. In the meantime carers are performing these duties. The meal provided during the inspection was presented well and looked appetising. Residents had no complaints. The complainant was concerned that the resident being case tracked was being restrained by locking their bedroom door at night. The complainant was not willing to make a formal witness statement. In discussion with staff on duty during the inspection no further evidence was found to support this part of the complaint.
