It is owned and managed by Mrs Rajeswaree Jeeawon. The home is located approximately one mile from the centre of Bexhill, with access to shops, pubs, churches and the post office. The accommodation comprises of three floors, and includes communal space, plus thirteen (13) single bedrooms, and three (3) shared bedrooms. Five (5) of the bedrooms provide en-suite facilities. We undertook an unannounced random inspection of this service on 22nd December 2008 following receipt of some concerns regarding medication arrangements in the home and the overall standard of cleanliness. We undertook a tour of the premises, spoke with staff on duty, met three residents and looked at medication storage, spoke with the provider/manager and another staff member regarding medication administration to residents and looked at recording of medication and individual medication arrangements as specified in support plans. We found the home to be clean and tidy with no unpleasant odours, the staff we spoke with confirmed there is a regular cleaning routine for resident bedrooms, these are cleaned every morning. The home will benefit from investment in an upgrading programme particularly communal washing/toilet areas and communal hallways which are showing signs of wear and tear with bathrooms in particular in need of refurbishment. We looked at medication arrangements within the home and found that all administering staff are trained to a good standard and receive regular updates, only one resident self administers, although most other residents have medication administered to them but are not observed by staff always taking their medication and medication sheets record this activity although we noted staff routinely using the wrong code to denote this. We found that whilst generally recording of medication is satisfactory for the period 20th/21st a number of recording omissions were visible on Medication administration sheets. We checked medication stocks with staff to ensure medications had in fact been given and found this to be the case, We have issued a requirement to the home to maintain accurate medication records at all times. We noted that the home has established general PRN medication guidelines to staff and we would recommend these are individualized for each resident. The home has established details of medications and side effects within individual resident files. The facility exists for residents to confirm their consent to medication administration by staff but this is routinely not completed in support plans and should be. We found no mechanism for recording the medications received into the home for each resident other than the receipt that is signed for the pharmacy and we would recommend a system is developed. We noted that risk information makes clear those residents whose compliance may pose a risk but this is not reflected in the support provided to them around medication and we consider an element of complacency has developed in the general administration to residents without the service being able to justify its rationale for present practices. Care Homes for Adults (18-65 years) Page 4 of 9 We consider that the medication storage is not compliant with pharmaceutical society guidelines and the medicines Act 1968 we have discussed this with the provider and consideration should be given to the upgrading of storage facilities as part of a general upgrade of the premises.
