What the care home does well

The manager supports all the staff in the home to provide a continuing high standard of care in the home. Systems of care within the home are regularly reviewed and the staff are consulted about any changes to be made in the home. The general environment of the home is maintained to a high standard and all the staff, both nursing and ancillary appear to take pride in their work within the home. Residents in both wings of the home confirmed that are given freedom of choice within their daily lives and that communication between themselves and the care staff is good and that they are always aware of changes that may take place within the home.

What has improved since the last inspection?

Nutritional assessments are now in place for all residents in the home. Residents` records demonstrate that these assessments are regularly reviewed and updated.

What the care home could do better:

The home is constantly reviewing standards in the home and will look to maintain these standards.

No more than 23 beds may be used at any one time for service users who are in need of nursing care. 30th August 2005 Date of last inspection Brief Description of the Service: Little Haven is a registered care home that accommodates forty-three service users over the age of 65years. Twenty-three service users are in receipt of nursing care and the remaining twenty are in need of assistance with personal care only. The home is the sister home to Forest Oaks and is administrated by the Wilverley Association Ltd, which is a charitable trust, and as such, all members of the governing committee are volunteers. The home was purpose built some years ago and is housed on three floors in pleasant gardens and is situated in a semi-rural area close to the New Forest. The home is close to shops and other local amenities. Many of the residential service users are independent and are able to go out into the community independently. All 43 rooms are single occupancy and have en-suite facilities and have extra facilities within them for storage. The service users who receive personal care furnish those rooms and supply all soft furnishings making each room very individual. The home has two passenger lifts installed. The grounds surrounding the home are well maintained. This inspection was the second statutory unannounced inspection for 2005/2006. The inspection was conducted by one inspector and lasted four and a half hours. The manager was present throughout the inspection. A tour of the building was made and records were examined. The inspector met with residents and members of staff. Feedback from residents and staff was very positive. Staff and residents stated that communication with the manager and her deputy is very good. The residents spoken to confirmed that always felt comfortable in discussing any concerns they had with the manager or any other member of staff. Members of staff confirmed that the training they receive and support from the manager and her deputy enables them to maintain a high standard of care in the home. They stated that they found their job very rewarding. Residents told the inspector that the staff in the home were always kind and supportive towards them. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Arrangements for assessments are in place, prior to admission, which ensure that the needs of the resident can be met. Standard 6 is not applicable to this service. EVIDENCE: The manger or her deputy undertakes a full needs assessment for all service users who are to be admitted to the home to ensure that all their care needs will be met. The assessment includes all aspects of personal care and well-being, their physical and psychological needs and personal safety. The information for this assessment is obtained from the resident, their families and other health care professionals who may be currently caring for the prospective resident. This assessment identifies the health care needs of the resident and forms the basis for care plans that will be written to address the care needs of the resident. Residents for this area of the home are generally self-caring and independent, but may on occasions require some assistance with personal care and mobility. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 8 The care planning and communication systems in the home ensure that the health and care needs of the residents are met in a way that respects their privacy and dignity. Standards 9 and 10 were assessed during the inspection undertaken in August 2005. EVIDENCE: The inspector examined records for four residents. All the records examined contained a comprehensive pre-admission assessment. On admission to the home a further assessment is undertaken to provide additonal information allowing the staff to identify the individual care needs of each resident. Personal details documented, included the contact details for the service user’s next of kin, date of birth, the date that they were admitted to the home, and the doctor with whom they are registered. Care plans had been drawn up to address the care needs of the resident. The care plans identified the problem and clearly demonstrated the action plan and Little Haven Nursing & Residential Home DS0000011432.V272235.R01.S.doc Version 5.0 Page 11 the outcome that was to be achieved; ensuring that the health care needs of the resident is met. Residents are able to access dental and optical care as required. Podiatry care is provided every six weeks for the residents and they are able to choose whether they receive the service from a male or female podiatrist. All residents are registered with a local doctor. For residents who lived in the local area prior to admission to the home, they are able to retain their own doctor. The manager was able to confirm that no residents had a pressure sore. A tissue viability risk assessment is undertaken for all residents and this is reviewed and re-evaluated every month. Residents assessed as being at risk are nursed on a pressure-relieving mattress appropriate to their assessed needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Comprehensive training undertaken by the staff enables them to be competent to do their jobs. EVIDENCE: Health care assistance working in the home have the opportunity to undertake training for NVQ at level 2and 3. The training officer for the home reported that at this time six health care assistants have achieved NVQ at level 2 and an additional six of their colleagues are working toward their level 2. One health care assistant has achieved NVQ level 3 and a further four are working towards this qualification. Members of staff spoken to confirmed that the home provides a very comprehensive training programme and that they are encouraged and supported by the manager and the training officer. Those members of staff spoken to who have undertaken their NVQ level 2, stated how much they had enjoyed the training and that it had given them more understanding of their role within the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35 and 38 The manager of the home is able to demonstrate that she is suitably qualified to ensure that the home is well run and that the needs of the residents are met. Procedures are in place to ensure that residents’ financial interests are safeguarded Close monitoring of practices within the home safeguard the health, safety and welfare of residents, staff and visitors to the home. EVIDENCE: Mrs Adele Roberts, is a Registered General Nurse, Level 1and has been the registered manager for the home since 2002. Mrs Roberts has her Registered Managers award which she completed in 2004 and has also completed her Management in Care award. Further post registration qualifications include specialist training in coronary care, diabetes. Little Haven Nursing & Residential Home DS0000011432.V272235.R01.S.doc Version 5.0 Page 17 Mrs Roberts has also undertaken teaching qualifications from City and Guilds in Adult Education and a nurse specific teaching qualification, ENB 998. She will be undertaking a training course for managers in Abuse Awareness in January 2006. The home is responsible for the monies of one resident. Copies of all receipts of expenditures are obtained and are forwarded to the resident’s solicitor who is their Power of Attorney. Inspection of records indicated that regular tests are undertaken of all equipment and systems used in the home, including fire safety equipment, emergency lighting and extinguishers. The records also demonstrated that fire drills and instructions take place regularly and any faults noted in the system are rectified immediately. Staff members spoken to confirmed this. The weekly testing of the fire alarm system was undertaken during the inspection. All staff employed in the home undertakes regular training for: Fire safety Safe moving and handling Infection control First aid On the day of the inspection the kitchen was found to be very clean and tidy. Records examined by the inspector demonstrated that there is a regular programme of cleaning of all areas in the kitchen and that the temperatures of fridges and freezers are recorded daily.
