Conditions of registration: Date of last inspection 20th May 2005 Brief Description of the Service: Lobswood House is a care home that is registered to accommodate up to 26 residents in the category of dementia and mental disorder in older people. It is a large detached older property covering three floors, (with a passenger lift to all floors,) located in the seaside town of Littlehampton, opposite a wooded green, and near to the sea and the town centre. Twenty two of the bedrooms in the establishment are single, six en-suite, in a newly built extension. The home has a secure secluded garden to the rear of the property, which includes a patio and a water feature. Apex Healthcare privately owns the service, and the registered Manager is Mr. J. Muldoon. The responsible person operating on behalf of the company is Dr. G S Kalkat. The proprietor and Manager have both received satisfactory Criminal Record Bureau checks. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, the second of a minimum of two statutory visits that an inspector must make to each care home during the course of a year. The first inspection, which was announced, was undertaken earlier in the year. At this inspection the Inspector looked at standards that were not looked at during the previous inspection, as well as any outstanding issues from the last report. The reader is advised to look at the reports of both inspections for a fuller picture of the home. The Manager was not available on the day of inspection, but the Inspector was very ably assisted by the Deputy Manager, and after her shift finished, by a Senior Carer. During this inspection the Inspector examined records of care being provided to residents; as well as records of any accidents, issues, concerns or complaints, to make sure that the residents at Lobswood House were being taken care of. The inspector met many residents, but due to their mental frailty, they were not all able to tell the Inspector what it was like living at Lobswood House, they were, however, seen to be comfortable and well cared for, and appeared happy. A visitor spoken to on the day of inspection told the Inspector that she had no concerns at all about the care being provided by Lobswood House. At this inspection Lobswood House was audited against the National Minimum Standards for Older Persons. The vast majority of the elements in each of the standards assessed were met, however it was found that one of the concerns raised during the last inspection had not been fully addressed, and a requirement is made that all residents bedroom doors have locks fitted. The Inspector would like to thank everyone who cooperated with her on the day of this inspection. What the service does well:
All of the residents, who were spoken to on the day of inspection, spoke very highly of the staff members, the food and their bedrooms. The building is maintained safely, and it was seen that parts of the home had been recently decorated. All the records required by the Inspector on the day of inspection, were found to be in order. A visitor was very complementary with regard to the care being provided by the staff at Lobswood House. A visiting theatre group was coming to the home the day following the inspection, to entertain the residents with music, and songs. The Inspector that told that Lobswood House do not use agency workers, ensuring that the residents always know their carers. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 4 Each resident has a written contract/ statement of terms and conditions with Lobswood House, and residents and their representatives know that the home will meet their needs. EVIDENCE: The terms and conditions, for both private and funded residents, contain all the requirements of the National Minimum Standards. Evidence from care plans, and a discussion with the Deputy Manager and a resident’s daughter, confirmed that the home was able to meet the assessed needs of the residents accommodated there. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 The resident’s health, personal and social care needs are set out in an individual plan of care. EVIDENCE: Several care plans were inspected and were seen to be comprehensive and up to date. Monthly review are undertaken to ensure that the plan is constantly updated with the changing needs of the resident. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21 and 23 and 24 Residents live in a safe, well-maintained environment and have access to safe and comfortable indoor and outdoor communal facilities. Residents have sufficient and suitable lavatories and washing facilities. Residents have the specialist equipment they require to maximise their independence. EVIDENCE: Lobswood House was seen to provide level access to all areas used by residents, and maintenance records were available to show that the home is well maintained with periodic checks carried out by out side contractors. Lobswood House offers separate sitting and dining areas and provides sufficient communal space to meet the National Minimum Standards. There is outdoor space for residents, which is accessible to those residents with mobility problems. Lobswood House has a sufficient number of assisted baths or showers, and there are accessible toilets close to the communal sitting areas and the dining room.

A requirement is made in this report for this matter to be addressed. Two of the bedroom doors did not shut tightly when released. The Inspector was assured that the maintenance man would undertake a regular check to ensure that they always shut in the future. On the day of inspection a door was wedged open, the staff member accompanying the Inspector was made aware of the safety implications surrounding this. EVIDENCE: There was evidence that all staff members receive good training to enable them to care for the vulnerable people who live at Lobswood House. A visitor told the Inspector that she was very pleased with everything that she found at the home every time she visited, and that there always appeared to be enough people on duty to assist the residents when they need it, Lobswood House DS0000014615.V254169.R01.S.doc Version 5.0 Page 15 Management and Administration

Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 36 and 37 Residents are safeguarded by the accounting and financial procedures of the home, staff are appropriately supervised, and resident’s rights and interests are safeguarded by the home’s record keeping and policies and procedures. EVIDENCE: The inspector saw that insurance cover is in place, and there is a business and financial plan for the home, which would be available upon request. The staff members at Lobswood House receive formal bi-monthly supervision, and policies and procedures were in place with regard to Data Protection. All records seen kept up to date, and secure.
