The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The standard of care provided is much appreciated by the relatives and carers, and comments given to the commission include "I always feel she is being well cared for" " Nothing seems to be too much trouble and all the staff are very co-operative, my grateful thanks goes to them all" "I am consistently impressed by being made welcome by the staff." The residents are well looked after and treated as individuals. Given the size of the premises and the client group, the home is cleaned and maintained to a very high standard, and staff should be commended for this. The staff training and development programme is excellent, and has provided a trained and motivated team, to provide the best care for the residents. The provision of staff rotas` three months in advance is very good practice. A quarterly newsletter is provided that is both interesting and informative for staff relatives and visitors. The management of financial and accounting systems is very clear, and there are clear audit trails.

Two unnamed residents under the age of 65 may be admitted to the home at any one time. The home has four named individuals who, on admission, were under the age of 65. The total number of service users to be accommodated at any one time must not exceed 102. 8th February 2005 Date of last inspection Brief Description of the Service: The Manor House Nursing Home is situated in a quiet rural location, within visiting distance of Oxford, Aylesbury and Banbury, and close to the market town of Bicester. It is easily accessible from the M40. It is set in over four acres of gardens and lawns, overlooking open countryside, with a small lake, pond and fountains. A landscaped enclosed courtyard has been created with a water feature. The original house is a 16th century manor house that has been considerably extended. The accommodation is provided on two floors and most rooms are single, en-suite. There are a few shared rooms. The home is divided up into four lodges, North, West, East, and Garden, each with its own complement of communal areas and staff. The Manor House Nursing Home is home to 102 frail older people, who require nursing care, and some require specialist care for various forms of dementia Manor House Nursing Home DS0000042340.V262591.R01.S.doc Version 5.0 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This was an unannounced inspection and not all of the key standards were inspected on this occasion. The remainder will be inspected later in the inspection year. The inspection focused on the administrative and support systems within the home. Records required by regulation were examined, and discussion with the registered manager, and key members of staff took place. A number of Commission for Social Care Inspection comments cards were received and have been reflected in this report. The inspector received a warm welcome from the staff, and every co-operation was given. The home was in the process of being decorated for the festive season and was looking most attractive. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3,6. People are only admitted to the home having had a comprehensive assessment of their care needs and confirmation that the home can meet those needs. The home does not provide an intermediate care service. EVIDENCE: The registered manager told the inspector that she carries out all preadmission assessments to ensure that the home can meet the assessed needs. The gathering of information from family members, carers, any other professional, or voluntary body involved in the care provision prior to applying to reside at the home, supports this. The home has made a decision to defer the provision of an intermediate care unit for the present time. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10. All the residents’ are treated with dignity and respect. EVIDENCE: The inspector observed staff assisting the residents’ and it was apparent to the inspector that the staff understood issues of privacy and dignity by the manner in which they interacted with the residents’, and by knocking on doors before entering private rooms, bathrooms or toilets. The rooms, which are shared, have portable screening available to provide privacy when carrying out personal care. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26. The home is clean well maintained and there were no unhygienic odours at the time of the inspection. EVIDENCE: The inspector toured the building and found that all areas were cleaned to a high standard, and that the building and equipment were maintained in good order. There were no unhygienic odours observed and this is commendable given the client group and the difficulty in managing continence. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30. The home is well staffed at all times to provide for the care needs of the residents’. Recruitment polices and practices are robust. The registered manager is committed to staff training and development and the staff are well trained and competent to do their jobs. EVIDENCE: Staff rotas were examined and showed that sufficient numbers and skill mix of staff are on duty at all times. The registered manager arranges the rotas three months in advance which is commendable, as the staff arrange their social life around the service rather than the other way round. The registered manager explained that at the present time a number of the care staff are nurses who have completed their adaptation course but prefer to stay at Manor House as a carer, rather than moving to another home in the group to take up a nursing post. Staff spoken to expressed how they enjoyed working at Manor House, and felt supported in their work Manor House Nursing Home DS0000042340.V262591.R01.S.doc Version 5.0 Page 14 The domestic staff compliment is complete at the present time. A sample of staff files were examined and showed that the home’s recruitment policies and practices are robust. Evidence of training and development was seen on staff files and show that the staff are well trained in all aspects of care relevant to the care group who live at Manor House. Over 50 of the care staff have achieved NVQ 2 in care, and a number have exceeded this. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34,35,38. The home’s accounting and financial systems are in good order. The residents’ financial interests are safeguarded. There are excellent policies, procedures, practices in place in all matters concerning Health and safety, and this is supported by the training and development programme. EVIDENCE: The inspector examined the home’s financial and accounting system with the administrator and found that it was all in good order and that there is a very clear audit trail. The financial system is managed from head office and all accounts are generated from this office. Manor House Nursing Home DS0000042340.V262591.R01.S.doc Version 5.0 Page 16 The residents’ are able to hold small amounts of cash at the home and the keeping of clear records of all transactions safeguards this, and receipts kept of any purchases on the residents’ behalf. The inspector examined service records for hoist, passenger lift, boilers, fire fighting equipment, and founds these to be up to date and complete. Records of regular monthly checks of all maintenance systems were seen and found to be up to date. The laundry was found to be in clean and orderly and staff spoken to clearly take pride in their work. There is concern that one of the three tumble dryers is not in operation, and it creates more work for the staff in dealing with the laundry for 101 residents. It is recommended that the registered manager should look into the possibility of replacing the condemned tumble dryer.
