The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Staff deliver good personal care to residents and treat residents with dignity and respect. Residents are well cared for and happy living in the home. The home employs stable core staff that understands residents needs and communicate with them very well. Residents attend day centres` and go out each week. The deputy manager has made great progress in the home since being in charge and has a good knowledge of residents needs. The deputy manager has developed practices to improve outcomes for the residents and service. The home seeks views and opinions from residents and staff in regular meetings.

What has improved since the last inspection?

The deputy manager has introduced the following changes since the last inspection: Thorough recruitments checks addressing existing shortfalls in previous staff recruitment. A new quality assurance system seeking views of placing social workers residents and staff. Regular resident meetings, staff supervision and induction training for staff. Monthly management meetings with the registered provider. One Regulation 26 visits to the home from the registered provider. Recruitment of 1.5 extra carer assistants, an assistant manager and a cleaner for the home. The deputy manager had enrolled in NVQ level 4 in care management training. New updated care plans for all residents. Redecoration of some parts of the home including treatment for dampness. A new walk in shower for residents in wheel chairs. Staff training in dementia. Updated duty rota to include staff surnames and hours worked. Some updated policies and procedures and new statement of purpose. Regular fire drills. A new printer for the homes office and a new mini bus to take residents out. Appointments for residents to visit the hospital, audiologist, dentist and opticians. Regular trips out for residents to visit weekly clubs and outings out in the community. The home staff records were available for inspection.

What the care home could do better:

It would be good for residents if the homes stability in respect of the future of the home was finalised. The home could recruit a new registered manager who is able to lead and continue developing the service. The home must ensure all residents are pre assessed and visited prior to admission to ensure the home can meet their needs. The home must appoint a new trustee for residents as the previous registered manager has now left the home.

Date of last inspection 20/4/05 Brief Description of the Service: Maplin House is a large detached house set in a sought after residential area in Shoeburyness/Thorpe Bay. Maplin providing accommodation for up to sixteen older people with some degree of learning disabilities, including one with dementia. Accommodation is provided on two floors within single and double bedrooms. There is a kitchen and bedroom on the third floor for staff use.The home has adequate communal areas on both floors but has limited outdoor space. The home is in close proximity to the beach, shops, parks and local bus routes. The home provides transport facilities to take residents out. The inspection took place on 9/6/05 in response to the home receiving an immediate notification from the CSCI on 20/4/05 in respect of poor recruitments checks. The inspector returned to review care and found the deputy manager requested another inspection as the home had made progress within the past month. Eight residents, two staff members and the deputy manager were all spoken to at inspection. Feedback regarding the inspection was given to the deputy manager and a follow up letter of acknowledging improvements, following the inspection sent to the registered provider. The inspector would like to take this opportunity to thank residents, staff and the deputy manager for their time and cooperation during the inspection. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users have an opportunity to “test drive” the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. 6. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5,6. Prospective residents individual aspirations and needs are not always assessed prior to them being admitted to the home and residents do not always have the opportunity to visit and test-drive the home before admission. Residents do however know their changing needs and personal goals are reflected in their individual care plans. The home is not registered to provide respite or intermediate care. The home had not visited the resident prior to admission to complete any assessment therefore was unable to identify if the admission was suitable for either the resident or the home. Fortunately the admission was found to be suitable but a minor variation to registration was required by the home, as the home was not registered to accommodate the resident’s medical condition. The inspector advised the deputy manager regarding the seriousness and risk of not making adequate assessments prior to admission and spoke to all the relevant parties regarding the admission including the resident and local placing authority. The home has recently updated the homes statement of purpose. Including their physical and emotional health needs. Service users’ health care needs are fully met. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users know their assessed and changing needs and personal goals are reflected in their Individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. Service users receive personal support in the way they prefer and require. The Commission considers standards 7, 8, 9 and 10 (Older People) and Standards 6, 7, 9, 18, 19 and 20 (Adults 18-65) are the key standards to be inspected at least once during a 12 month period JUDGEMENT – we looked at outcomes for standard(s) 7,8,10, Residents continue to be well looked after in respect of personal care and now have relevant health professional appointments to ensure their health needs are met. Care plans are updated and reviewed monthly and contain all relevant information to reflect residents changing needs. Maplin House I56 S15450 Maplin House V232591 090605 Stage 4.doc Version 1.30 Page 12 EVIDENCE: The inspector noted how happy and well looked after residents were during inspection. All residents were clean and very well presented. Residents confirmed they were assisted well with personal care and able to help choose their clothes. The communication between staff and residents was very positive and mutually respectful and staff treated residents with dignity. Care plans now include a profile’, a photo of each resident, health professional appointment list and contained updated risk assessments. All care plans are to be reviewed monthly by the deputy manager. The deputy manager was currently contacting all placing social workers to make arrangements to review all residents living in the home as some residents had not had a review from placing authorities for years. The care plans gave staff instruction regarding care and daily care plans reflected this was carried forward in day-to-day care. The home ensures all residents have access to a general practitioner. The inspector identified since the last inspection residents had attended appropriate health professional appointments. Residents confirmed this at inspection stating they had been to the dentist for new dentures, visited the opticians for new glasses and audiologist for new hearing aids. One resident had visited the hospital for a new surgical shoe. The manager was praised for improved outcomes for residents regarding health and personal care. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Including opportunities for personal development. Service users engage in appropriate leisure activities. Service users maintain contact with family/ friends/ representatives and the local community as they wish. And have appropriate personal, family and sexual relationships. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. 13. 14. 15. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14. Residents now take part in age, peer and culturally appropriate activities inside and outside the home and enjoy being part of the community engaging in appropriate leisure activities. Residents have appropriate personal relationships with family and friends and exercise choice being consulted in all aspects of life in the home. EVIDENCE: During inspection residents were very happy with the changes since the last inspection because they were going out in the new mini bus purchased by the registered provider for the home. Residents went out for lunch at inspection while another resident was to go to the pub. Other residents commented they are now are able to go to their clubs and day centres and staff take them out more. A new resident who had just moved into the home had an interest in aircraft, was taken with other residents to an air show and all residents appeared much more motivated and happier living in the home. This is good practice. One resident had asked for perfume so the deputy manager had devised a bingo game as a prize of perfume to meet the resident’s needs in a fun way. Other residents had started to participate in daily living activities in the home including cooking and gardening. One resident had produced some beautiful tubs of flowers for the home. Residents were seen to be much more motivated and active. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. Including neglect and selfharm. The Commission considers standards 16 and 18 (Older People) and Standards 22 and 23 (Adults 18-65) the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18. Residents’ legal rights are protected and they are protected from abuse by good recruitment practices and appropriate staff training. EVIDENCE: No complaints had been made in the home since the last inspection. Residents have now started to exercise more independence but the home must appoint a new appointee for residents’ personal money, as the previous appointee no longer works in the home. Since the last inspection the deputy manager has reviewed staff recruitment checks and requested staff without appropriate checks produce relevant documentation missing from staff files. The home has employed only staff that produced appropriate checks to ensure residents are protected and had produced all relevant references and checks missing at the last inspection for staff employed in the home. All staff are now undertaking a new induction programme which includes adult protection. Staff at inspection was aware of the signs of abuse and the procedures for reporting abuse. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers standards 19 and 26 (Older People) and Standards 24 and 30 (Adults 18-65) the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26. The home was comfortable clean and free from offensive odours decorated since the last inspection to meet the needs of residents accommodated. The home now provides adequate bathing facilities for disabled residents. EVIDENCE: The home was welcoming, clean and comfortable. The registered provider had given funding for a new walk in shower for disabled residents, treatment of dampness in one residents bedroom and decoration within the home. The new decorated bedrooms made the home look more bright and welcoming and residents appeared very happy with the changes. Residents’ bedrooms are all to be redecorated and at inspection one residents was very happy with her new decorated bedroom which was bright, furnished to a good standard. Service users are supported by an effective staff team. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. Service users benefit from clarity of staff roles and responsibilities. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers standards 27, 29 and 30 (Older People) and Standards 34 and 35 (Adults 18-65) the key standards to be inspected at leat once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30. Staff recruitment checks are now adequate to protect residents and staff and management help to support staff to meet the needs of residents living in the home. Staff were positive and happy about changes, staff numbers and the staff rota met the needs of this standard. EVIDENCE: Staff files were available and examined and records confirmed the deputy manager had sought appropriate CRB checks, references, and identification/immigration papers. References now included appropriate authenticity checks to confirm their validity. The deputy manager had joined a recognised umbrella body to carry out the home’s CRB checks and introduced new induction programmes, declaration of fitness to work record, operational policies, aim and objectives, new contracts of terms and conditions for staff and appropriate staff training and supervision. This is good practice. Staff without appropriate adult protection training had attended training since the last inspection. The home had recruited extra care and cleaning staff for the home. Staff numbers appeared to match the number of staff on duty. Staff meeting records were examined and evidenced staff were able to view their opinions in meetings. Staff files confirmed supervision had commenced for all staff to identify future training needs. This is good practice. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from a well run home and from competent and accountable management of the service. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31,32,3336,38. Residents benefit from the ethos, leadership and management approach of the deputy manager working in the home who is of good character and able to run the home in a competent manner to benefit residents. The homes is run in the best interest of residents regarding health and safety promoting protection. EVIDENCE: The new deputy manager has made vast improvements since the last inspection and demonstrated that the home has the potential to fulfil the homes aims and objectives. Residents and staff appeared to be very happy with the changes and improvements and staff appeared to be more settled in their roles, which benefit residents. The CSCI has not received an application for a new registered manager at the time of inspection, but the registered provider has written to the CSCI to advise this is to be addressed in the near future. The deputy manager has arranged to commence NVQ level 4 in Care management and was advised at inspection regarding pre assessment of residents before admission. The inspector was delighted with the progress and improved outcomes for all residents at Maplin House and hopes this will continue. The deputy manager has commenced a quality assurance and monitoring system for the home, but this is was not yet complete.
