The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This is a well managed home, run in the best interests of service users. Services users spoken with indicated they were happy with the care and support they were receiving. They appeared happy and content in their surroundings. The Manager and staff are to be congratulated on their recent safe evacuation of service users following a fire at the home. Efforts put into fire training and fire drills paid off and the arrangements made by management and staff for the care of service users following the fire are commendable.

What has improved since the last inspection?

The requirement made following the previous inspection for staff to undertake adult protection training has been addressed. These staff were able to demonstrate the action, which needs to be taken in respect of allegation or suspicion of abuse. The Manager has been assessed as `fit` to be registered with the Commission.

What the care home could do better:

The requirement made in respect of the need to have a central record of all complaints received, with details of the investigation, actions taken and outcomes, has yet to be addressed.

Admittance of a person under 65 is for Mrs TT only, DOB 8/10/1948. This condition will apply until the named person leaves the home or turns 65. 12th July 2005 Date of last inspection Brief Description of the Service: Muriel Street is a newly purpose built 60 bedded care home, registered with the Commission for the provision of care for older people suffering primarily with dementia and mental disorder. The home is owned and managed by Care UK Ltd and service provision is in partnership with Islington Council. It is situated in a residential area within walking distance of Caledonia Road, where there are various shops and methods of public transport. The home is adjacent to Regents Canal. The accommodation provided is as follows: Ground Floor – 20 beds for nursing care of people with mental disorder First Floor - 20 beds for nursing care of people with dementia Second Floor – 20 beds for people requiring personal care of people with dementia. The home consists of all single bedrooms with ensuite toilets, showers and wash basins. In addition there are assisted bathrooms and toilets on the two nursing floors. Each floor is divided into two units, accommodating 10 service users in each. This includes separate dining rooms and lounges. The home is pleasantly furnished throughout. Direct care of service users is provided by qualified nursing staff, both Registered General Nurses and Registered Mental Nurses and Care Workers. This was the second unannounced statutory inspection for 2005/2006. The emphasis on this inspection was to assess the key standards, which had not been assessed at the last inspection, and to follow up requirements made in respect of recording complaints and arrangements for handling adult protection issues. In addition, a key element of this inspection was to assess how service users needs were being met following a fire at the home, the damage of which resulted in the closure of 13 beds. This matter will be referred to throughout this report, as the issues arising relate to a number of the key standards. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The Manager and staff had worked very hard in ensuring the continued well being of service users, following the recent fire. It seemed clear that service users were happy with the arrangements in place in the interim and were very appreciative of all the support they were receiving. The matter will need to be kept under review, and the emphasis will be on ensuring each individual service user is settled into their own designated bedroom as soon as possible. EVIDENCE: This standard was re-assessed in view of the recent fire at the home. Service users were safely evacuated to another area within the home. This meant there were nine service users without their own bedrooms. At this point the management of the home, following consultation with service users, relatives, and staff, decided it would be in the best interest of service users to try and accommodate them at the home in the interim, rather than transfer them out of the home. Service users were accommodated in communal areas, which had been turned into temporary bedrooms. Service users spoken with confirmed they were happy with the current arrangements and that they wished to remain at the home. The Inspector spoke with the Contracts Manager from Islington Local Authority, who was present, and it was agreed Muriel Street Resource Centre DS0000054457.V258864.R01.S.doc Version 5.0 Page 9 that service users without their own designated bedrooms would need to have their needs re-assessed by their relevant care managers. In the meantime all new admissions have been put on hold until such time as all existing service users can be accommodated in a single designated bedroom. It is anticipated that the bedrooms which are currently out of use will be fit to be occupied by mid January 2006 but this matter will need to be kept under review as the current arrangements would only be acceptable as an interim measure during this period. Robust risk assessments had been developed for each service user, who had to be evacuated. These were evident in care plans and highlighted the orientation needs of service users. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 & 10 The risk assessments developed for service users who needed transfer to other areas of the home following the fire were sufficiently robust to highlight their needs. Such information should be reflected on the relevant care plans. Staff are demonstrating the capacity to uphold service users; rights, dignity and privacy. EVIDENCE: The standard in respect of care planning was assessed as met at the previous inspection at the previous. The Inspector advised that individual care plans should be developed to reflect the information from the risk assessments, which were formulated in respect of the recent moving of service users. Staff have worked very hard to ensure service users’ rights, dignity and privacy are maintained and arrangements are being made, as an interim measure, to further enhance this by arranging for appropriate mobile screens to be delivered. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Service users are assisted in exercising choice and control over their lives. This is particularly pertinent to those service users who had to transfer to other areas within the home recently. EVIDENCE: The recent event demonstrated how service users and their relatives /carers are assisted to exercise choice and personal autonomy. This included evidence of ongoing communication with service users and relatives in addition to arrangements for personal possessions. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The manager has yet to demonstrate that robust records of complaints received are held centrally and includes actions taken and outcomes. The knowledge, which staff demonstrated, on adult protection issues demonstrates the effectiveness of the adult protection training, which they undertook. There is a need to ensure all new members of staff can demonstrate the action they need to take in response to allegations or suspicions of abuse, pending attendance at appropriate training. EVIDENCE: A requirement was made at the previous inspection in respect of the need to keep appropriate records centrally on complaints received, including actions taken and outcomes. The manager confirmed there was a complaints log on each floor but there needs to be a central record. The requirement for staff to receive adult protection training has been addressed. Staff were able to demonstrate they would take appropriate action in the event of an allegation of abuse. There is also a need for newly appointed staff to demonstrate this, pending attendance at adult protection training. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 Until the recent fire the home has continued to meet all the environmental standards and this is still the case, with the exception of the 13 bedrooms, which are currently out of use. Staff have demonstrate thought and sensitivity in the arrangements they made for the service users who were affected. This matter will need to be reviewed again in January 2006. EVIDENCE: This is a new purpose built home, which up to the point the fire occurred, met all the environmental standards. Until the fire damage has been repaired some service users are sharing rooms within communal areas. These areas have been personalised and have been assessed as meeting the service users’ needs in the interim. The matter will need to be kept under review and a further evaluation carried out in January 2006. JUDGEMENT – we looked at outcomes for the following standard(s): 3&8 The staffing arrangements in place meet the needs of service users. EVIDENCE: These standards were assessed as met at the previous inspection. In view of the recent fire and the need to accommodate service users in communal areas extra staff have been rostered for day and night duty to ensure these service users’ needs are met. Staff demonstrated their competence in respect of the safe evacuation of service users during the recent fire. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 & 38 The home is run in the best interest, safety and well-being of service users. EVIDENCE: The Manager has been registered with the Commission since the last inspection. Arrangements in place demonstrate the home is being managed effectively. This was particularly highlighted in the co-ordinated effort recently required of staff and management to ensure the safety and well-being of service users. The safe evacuation of service users demonstrated the effectiveness of fire training undertaken by staff in addition to the fire evacuation procedures in place. The Fire Authority’s investigation into the cause of the recent fire is awaited.
