This inspection was carried out on 15th November 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The staff team are clearly motivated. They recognise the importance of making sure residents have a good level of care delivered by a trained staff team. The homes environment is maintained to a high standard for the comfort of people living there, with a consistent maintenance programme in place to make sure the homes environment is always kept to a high standard.

What has improved since the last inspection?

The home has reviewed and changed its assessment records, medication records and procedures, complaints recordings and recruitment checks so that they cover all areas of need and protect residents living in the home.

The service should, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection 5th July 2005 Date of last inspection Brief Description of the Service: North Shore Nursing Home is an establishment providing residential and nursing care for up to 25 residents, who suffer from dementia. The home is situated in a residential area of Blackpool. There are three floors and a rear extension. There are twenty-three single rooms of which sixteen are en-suite. Security pads are in place at the front door and between the ground floor corridor and the extension area, this is for the safety of residents. The home is equipped with an appropriate range of aids and adaptations suitable to meet the needs of residents living at the care home. This was the second statutory inspection for 2005-06. It was unannounced and undertaken during a weekday period. The inspection examined a number of standards, including Nutrition, Privacy and Choice, Financial management, Environmental standards and health and safety issues which will be focused upon in the main body of this report. During the inspection the manager and individual staff members assisted the inspector with the process. Due to the high level of dementia suffered by residents it was not possible to gather comments, however the report will reflect the observations made during the inspection. What the service does well: What has improved since the last inspection? What they could do better: Currently there is no registered manager in post. The homeowners have communicated with the Commission in respect of this but a decision must now be made and application submitted. This will also ensure the conditions are registration are met. This is also important to ensure that the home is managed effectively at all times. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 4 The assessment records provide a complete insight into the needs of residents and describe how their needs will be met. EVIDENCE: A review of the homes assessment records has been completed and now the records provide a clear picture of the individual needs of residents and how there needs will be met, so that they receive the appropriate level of care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9, & 10 Medication administration and recording has been reviewed for the protection of residents and staff. The staff team protects residents Privacy sensitively, so that their respect is upheld. EVIDENCE: A full review of the medication system has taken place so that procedures now in place protect residents and staff. The staff team, who are aware of the need to be sensitive in situations, when, due to the level of confusion experienced by residents living there, there are occasions when individual residents may demonstrate behaviour, which would cause their level of Privacy to be displaced, upholds residents Privacy. Observations made and general discussion with staff confirmed staff know how to deal with situations when they arise. There are no locks on bedroom doors due to the levels of confusion of residents, which may cause additional risk to themselves or others. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Choice is limited due to the level of confusion experienced by residents living there, however there are systems in place to make sure choice is not totally restricted. Resident’s nutritional needs are met by a balanced diet. EVIDENCE: Due to the high level of confusion residents suffer due to various levels of dementia, choice is limited. This was not seen to be restrictive, due to the level of knowledge of the staff team, who said they get to know individual residents likes and dislikes and so can offer choice in their individual daily lives. Resident’s nutritional needs are met by providing a balanced diet. The chef said “we buy all the food as fresh as possible and use local suppliers”. Special diets are catered for and staff spoken to said, “we know their likes and dislikes”, so that they get what they like. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, & 26 The homes environment is well maintained, with good adaptations in place for the comfort of people who live there. EVIDENCE: The homes environment is maintained to a high standard, so that people who live there have a comfortable environment in which to live. The home is clean and hygienic with no evidence of offensive odours. All rooms are decorated individually, with most residents having personal items of furniture or possessions in their rooms. There are a number of bathrooms with a variety of bathing adaptations, so that residents with mobility problems can bathe in comfort. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 Recruitment procedures have been reviewed and revised for the protection of residents. EVIDENCE: Residents are protected through the revised recruitment procedures now in place, which makes sure all applicants have undergone ‘fitness’ checks prior to commencing work at the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 35,& 38 The home has systems in place to make sure residents are protected and safe at all times. EVIDENCE: There was evidence of the homes management team having systems in place to make sure residents and staff are protected by those systems. However there is no registered manager in place at the home. This must now be given consideration and an application must be made. There is a good system in place for the management of residents monies and personal allowance, so that residents money is kept safe and is accessible at all times by designated persons for the protection of residents.
