The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Residents said that the food had improved and most felt that the staff treated them well and provided the care that was needed. Residents are able to make their rooms homely with their own possessions. The manager has identified any shortfalls in the service provided. She has prioritised what needs to be done and has already accomplished much, for example in securing funding for specialist equipment, developing care plans and advertising for domestic and maintenance staff.

What has improved since the last inspection?

Several acting managers had been in the home prior to the present manager taking up the position. The manager is now registered and has established clear leadership of the home. The manager has the support of the home`s owners to make changes to improve the service.

What the care home could do better:

Several requirements and recommendations were made at this inspection, but it must be acknowledged that the manager had already identified most of the things which need to be improved and has already accomplished much. The manager needs further time to make necessary changes. Residents would like to be further consulted about all aspects of life in the home. The home could organise resident`s meetings where people could give their views and help to develop the home.

Date of last inspection 14th December 2004 Brief Description of the Service: Oakdale is a large detached private home which offers accommodation with personal care for twenty-seven older people. The home is situated in a residential area of Thundersley. It is relatively close to the main shopping areas of Southend, Rayleigh and Benfleet. Bus routes are a short walk from the home. The homes facilities are situated on two floors and include two lounges, one with a dining area, twenty-three single bedrooms and two shared bedrooms. A shaft lift provides access to all areas used by service users. Some of the bedrooms have en suite facilities. The home has off road parking facilities to the rear of the building. There is a well maintained rear garden. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 5 and 6 For those residents newly admitted to the home, assessments are detailed. EVIDENCE: Residents said they were able to visit the home and move in on a trial basis. The manager feels this is a good opportunity to naturally observe people and further assess their needs. The manager has carried out detailed assessments. Some of those assessments carried out before her appointment did not include enough information. The home has been asked to try to gain further details that are missing on such assessments. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9, 10, 11 Care plans have improved but need to be reviewed. Some staff are caring and respectful towards the residents. Medication administration was not robust. Recording of the last wishes of residents was not consistent although the manager has tried to obtain this information. EVIDENCE: Care plans for people recently admitted clearly detailed their care needs. Daily recordings were clear for staff to use. Not all care plans had been reviewed often enough. Some comments may be insensitive to residents who are entitled to read them and contribute to them. Residents’ preferred term of address should be recorded and used by staff. The home should ensure that people wear their own clothes at all times. The administration and recording of medication observed was not robust. The system must be reviewed, as there are risks to residents at present. Generally, residents praised the staff for their kindness and friendliness but the respect shown to residents was not consistent amongst staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 and 15 Visitors are made welcome in the home. Limited activities are available to residents; some people would like to have more to do. There is choice of food. Menus are kept but not displayed in the home. EVIDENCE: Some residents read newspapers, played cards, chatted and later in the day, played a game with a carer. One resident helped by wiping tables after lunch. A hairdresser was working in the home. Some residents said they get bored and some felt isolated. Opportunities to interact with residents were not always taken by staff. Ways of occupying and engaging with people who have dementia were limited. Visitors said they were welcome in the home. Those residents spoken to say they have enough to eat and they like the food. There is a choice of food and it would help if the menus were displayed. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a satisfactory system for responding to complaints. EVIDENCE: One complaint was made at the home that was upheld. None have been made to CSCI. The complaints policy was displayed in the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,22,23 and 26 The home was generally comfortable and the garden well maintained. The manager had identified specific areas of the building and garden that could be improved. The providers were supporting these proposed changes. The home has funded specialist equipment, such as mattresses and wheelchairs that resident’s need. EVIDENCE: Fans were being used to air communal rooms and the home was very warm. Blinds have been ordered for windows in the dining room to shade this area. The manager plans to adapt a bathroom that can be used for hairdressing, as present arrangements are not acceptable. There are plans to alter the garden to isolate the car parking areas and make the area secure for residents. Standards of cleanliness were not sufficient in places and there was an unpleasant smell in two areas. Several small maintenance jobs were needed; the home planned to advertise for maintenance staff. Grab rails have been provided in corridors. Further adaptations to the premises, for example, additional grab rails and an accessible telephone, to be used in private, would improve people’s use of the building. A call bell was not accessible to residents in the small lounge. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30 Sufficient care staff are employed at the home. Agency staff were not being used. A vacancy for maintenance staff is being advertised. Further training to help staff meet the varied needs of residents would improve the care given. EVIDENCE: The home has recruited domestic staff that will start soon. The home will advertise for maintenance staff. The manager has put a thorough training package into place for new staff. Training for established staff is being developed to ensure that all staff have the same approach to residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 33 and 38 The manager has prioritised areas requiring improvement in the home and is committed to improving standards of care. Reports of monthly visits by the registered providers have not been received at CSCI recently. EVIDENCE: The manager is very experienced in the care of elderly people. She has been in place for approximately six months and has made many improvements and changes to the home. She is working to achieve the Registered Managers’ Award. The manager ensures staff have plans of the building and understand fire instructions. Some equipment used for hairdressing was old and did not appear to have been electrically tested, therefore being a potential risk to residents. Chemicals which are potentially dangerous to health must be stored appropriately. 1.6.05 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. Weight of residents on admission and susequently should be recorded. Care plans should be reviewed at least once a month and updated to reflect changing needs. Care should be taken that comments on care plans are sensitively written as residents/their representatives are entitled top read them. Staff should record and use the term of address preferred by residents and should be instructed on how to treat residents with respect at all times. People should wear their own clothes at all times. Arrangements regarding terminal care and arrangements after death are recorded. That forums such as residents meetings are held to ask residents their views on all aspects of life in the home. That menus are displayed so that visitors, residents and staff know what is being served. A minimum ratio of 50 of care staff should achieve NVQ Level 2 by 2005.
