CSCI found this care home to be providing an Poor service.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Records showed that the hoist used to transfer one person had been serviced on 6th October. All substances which could have been potentially dangerous to people living in the home had either been disposed of or appropriately placed in the medication cabinet. Some fire records were available for inspection showing the alarm systems had been tested regularly. The registered provider was still unable to produce a fire risk assessment for the home. However improvements in staffing levels during the day and at night mean that the immediate risk identified for one person living at the home (who requires two people to transfer them) is no longer a factor. En-suite areas in bedrooms were checked and these were now clean but one person`s toilet was leaking. This was reported to the acting manager and the registered person.

What the care home could do better:

The improvements in staffing levels must be maintained and will be monitored during subsequent random inspections. Provision must also be made within the rota to ensure that staff do not work excessive hours, thereby compromising the quality of service they can provide to the people who live in the home. No additional requirements or recommendations were made as a result of this inspection.

Date of last inspection Brief description of the care home Onkar Care Home is registered to provide care for 10 adults with learning disabilities, all of whom may have additional mental health problems and three of whom may have a physical disability. The home is located in a residential area with easy access to public transport and the city of Leicester. The property consists of two floors and has ten single bedrooms all with en-suite facilities. There are a variety of aids and adaptations throughout the home based on residents assessed care needs. At the time of the inspection, fees ranged from approximately #329 to #751 per week. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: We made an unannounced visit to Onkar Care Home on Wednesday 8th October between 10:30 and 13:00. This was to monitor the response to requirements made at the key inspection on Thursday 2nd October, in relation to staffing and aspects of health and safety. We also looked at the general cleanliness of the home. The Draft Key Inspection Report has been issued and the quality rating of the home has been changed from good to poor. At the beginning of the inspection the new staff rota was still incomplete. The registered person has asked the registered provider from another home for advice (on an informal basis) on how to staff the home and structure the rota. After some discussion and guidance from the inspector the registered provided and acting manager produced a suitable rota. The rota showed three people on duty during the day until 10pm. At night there was one person sleeping in and one waking staff member. The rota showed that the night staff member had still not had a day off despite this being raised at the key inspection; this meant that they had worked at least 34 days without a break. Discussion with the acting manager and the registered provider appeared to resolve the situation. We were informed that an agency worker would cover the waking night duty and either the registered provider or manager would cover the sleep in duty at the home. It was emphasised that, given the personal care required by one female resident, that there should always be a female member of staff on duty at night. Two agency staff were on duty at the time of the inspection and confirmed that they had also worked at the home the previous day and were due to work at the home on subsequent days. Discussion was also held about the recruitment of new staff and the recruitment records for a new staff member were seen. This person was due to start work in the home when their name had been checked against the Protection of Vulnerable Adults Register.
