Conditions of registration: NONE Date of last inspection First since registration Brief Description of the Service: Orchard Manor provides a service specifically aimed at Young adults leaving education. They are currently registered for 20 people, although plan to extend this at a later date. The service offers structured opportunities to learn and develop a variety of skills, as well as taking part in a range of social and leisure activities. The service provides support to a range of service users with complex health and physical care needs, and currently charges fees ranging between £1753 and £3108 per week, depending on the assessed needs of the individuals and the level of support required. The service is based in Meldreth, close to the Cambridgeshire/Hertfordshire border, but within easy reach of all amenities. This is an overview of what the inspector found during the inspection. The service was first registered in April 2006, and a variation increasing the number of service users approved in October 2006, and this was a key unannounced inspection, and the first undertaken. Although now registered for up to 20 service users, they are currently well under capacity. During the inspection a number of service users and staff were seen, and their views and comments are reflected in the report. What the service does well: What has improved since the last inspection?
This was the first inspection since the service was registered in April this year, and therefore there are no noted improvements. It was however pleasing to note that the comments of the inspectors during the registration process were noted, and acted on. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 2 Quality in this outcome area is Good This judgement has been made using available evidence including a visit to this service. Detailed information is obtained for anyone moving into the service to ensure their needs can be met. EVIDENCE: During the inspection, the service’s admission procedures were discussed in detail with the manager of the service. Before any new admission takes place, an initial visit usually takes place involving the service user/family/social worker, and this is followed by a detailed assessment visit, which often involves an overnight stay. Once a package of care and the necessary funding agreed, confirmation is sent to the service user in writing, and details of when the service user will move in agreed. On the day they move to the service, all the relevant paperwork, care plans etc are completed. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 6, 7, 8 and 9 Quality in this outcome area is Good This judgement has been made using available evidence including a visit to this service. Care plans provide detailed information to ensure staff are able to support service users in all areas of their lives. EVIDENCE: As indicated earlier, detailed assessments are prepared prior to the service user moving to the service, and on the day of admission, care plans are prepared, which are constantly evaluated over the first three months the service user is in the home. The service endeavours to include and involve service users in all aspects of the home and their care, this can present challenges with those who are unable to communicate verbally, but the staff team have a wide range of communication skills, which was evident when observing them with service users. As part of the care planning, detailed generic and individual risks assessments are undertaken, and again these were seen during the visit.

The service promotes personal and community development in all areas. EVIDENCE: The service provides facilities for young adults with profound levels of disability to continue to develop within a structured setting, as well as being able to take part in leisure and cultural pursuits. This is achieved through individually centred plans, looking specifically at the wishes of the service user, and using a variety of methods and settings. Service users are encouraged to learn and develop new skills through a daily programme, established as part of the package, and which offer both vocational and leisure activities, and in addition a range of leisure pursuits are available at weekend and during the evening.

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 18, 19 and 20 Quality in this outcome area is Good This judgement has been made using available evidence including a visit to this service. Care plans ensure personal support is provided in a way that suits the individual. EVIDENCE: Personal support needs, together with physical and emotional health needs are detailed in the care plans discussed earlier, and care staff all receive detailed training to ensure these needs are met. The service uses two local GP practices, and already has a good rapport with them, and the services of local district nurses are also used. Dieticians from Addenbrooks hospital are providing support and advice and providing staff training. The service users are all individually assessed to clarify their ability to self medicate, but at the present time none of the service users in the home are able to manage or administer their own medication and staff therefore support them with the medications they need to take. Procedures are in place to ensure service users are safe, and concerns listened to and acted upon. EVIDENCE: This is a new service, and their local complaints policy is based on Scope’s national policies on complaints and adult protection. There have been no complaints received by the service since it was registered in April, and none received by the commission. The service offers a clean and homely environment for service users, equipped to a high standard. EVIDENCE: The complex work undertaken on the premises has just been completed, and the environment offers a well-equipped and comfortable setting for the service users living there. Particular care has been taken to provide a setting that is as domestic as possible, and all the bedrooms have access to en suite facilities. The standard of decoration and equipment is high, and service users are able to personalise their rooms, and choose colour schemes. The environment was very clean, with no offensive odours, and service users spoken with on the day were very happy with their new home. Recruitment policies ensure staff are fit for their role and competent, and regular supervision maintains standards. EVIDENCE: The staff team assembles all have the required skills and competence to meet the needs of the service users in the home. During the inspection, several of the staff on duty demonstrated their high level of commitment to the service users, and were seen to interact in a very appropriate manner. The homes recruitment policy was discussed, and staff files examined indicated that all the required procedures are followed. A detailed training plan is in place and evidence of the training that has already taken place as well as that planned was discussed with the manager.

Policies and procedures ensure staff and service users are not placed at risk. EVIDENCE: The home is well managed, and the management team were all seen interacting with the service users already resident. It was also particularly good to see that senior management knew service users individually, and took time to stop and talk with them. Service users and families are involved in regular reviews, and an annual quality assurance survey undertaken, together with monthly management visits and reports.
