What the care home does well

What has improved since the last inspection?

All care plans had been reviewed and updated and are now of good quality, setting out clearly how residents` needs are to be met. One care plan for a resident with dementia was particularly good as it showed staff working closely with a resident`s consultant and family to produce written guidelines for staff to follow in caring for the resident.

What the care home could do better:

At present the Acting Manager works as a full-time carer in the home while simultaneously fulfilling her role as a manager. She has responsibility for managing the staff team and overseeing all residents` records. It is recommended that she is given some designated non-caring hours each week to concentrate on these areas. Arrangements for staff supervision are currently informal. It is recommended that the Acting Manager is given the opportunity to carry out some formal staff supervision. This will give staff the opportunity to focus on their work performance and training needs.

Date of last inspection 4.02.05 Brief Description of the Service: Pine View is a 15-bedded residential care home situated on the Hinckley Road close to a range of local amenities. The home caters for older people, some of whom have dementia. There are nine single and three double bedrooms. The first floor is accessed by a passenger lift. On the ground floor there are two lounges, a dining room, and a conservatory. To the rear of the home is a large terraced garden with patios and seating areas. This was an unannounced inspection that took place on a weekday. The inspector talked to six residents, the Owner, the Acting Manager, and two care staff. The premises were toured and communal areas and bedrooms inspected. Care and medical records were examined. Two Recommendations were made. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 Care plans are up to date and of good quality. They clearly set out residents’ needs and explain how staff are to meet them. Staff are persistent in ensuring residents’ health needs are met. EVIDENCE: Four care plans were inspected in detail. All had been reviewed and updated in April 2005. Those sampled were of good quality, being comprehensive and containing appropriate risk assessments. One care plan, for a resident with dementia, was particularly good. Records showed staff had worked closely with the resident’s consultant and family when planning their care. The Acting Manager had drawn up written guidelines for staff to follow to ensure consistency. In addition, relaxation techniques were being to prevent the resident becoming agitated. At present a District Nurse visits the home every day to provide health care. The Acting Manager said relationships with the District Nurses are good, and staff can ask them for advice at any time. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 Structured and individual activities are provided to suit residents’ needs. Visitors are made welcome at the home and have a choice as to where they see residents. The full-time cook provides a varied and wholesome diet, which was praised by all the residents interviewed. EVIDENCE: The present resident group is lively and active. They enjoy socialising with the staff in the lounges, and there is a friendly banter and humour when staff and residents get together. One resident commented ‘We have such fun with the staff and they make us laugh all the time.’ A few structured activities are offered including visiting entertainers and singa-longs. On the day of inspection a party was being planned for a resident’s birthday. One resident looks after pot plants in the garden. Visitors are made welcome at the home and see residents in their own rooms or in one of the lounges. Menus showed that a varied and wholesome diet is provided with plenty of fruit and vegetables. There is a cooked breakfast every Thursday and Sunday. All residents interviewed praised the food comments included ‘The food’s lovely and there’s lots of different things to choose from’, and ‘I’ve no complaints about the food – it’s very tasty.’ Pine View Care Home C51 S64266 Pine View V226722 130505.doc Version 1.30 Page 13 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17, 18 Complaints are taken seriously and responded to within five days. There is a user-friendly complaints procedure for residents and their relatives/friends to follow. Good systems are in place to protect residents from abuse. EVIDENCE: The home has a simple, clear, and accessible complaints procedure and records showed that complaints are listened to, taken seriously and acted upon. The complaints procedure states that all complaints will be responded to within five days. Complaints received are recorded on a complaint pro forma and listed in a complaint log. Staff attempt to resolve all complaints informally, but if that is not successful the complainant is advised to use the home’s formal complaints procedure. The home has a policy on abuse, which explains the different forms abuse might take, and advises staff on how to respond to allegations of abuse. Staff are trained in-house to protect service users from abuse. The Owner who keeps the financial records, which are countersigned by the Acting Manager, oversees these arrangements. If there are concerns that a service user is not getting the money they are entitled to the home follows this up and involves a social worker if necessary. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 23, 26 The premises are well maintained and in good decorative order. Residents’ bedrooms are homely and individually decorated. The home is clean, fresh and tidy throughout. EVIDENCE: Communal rooms and five bedrooms were inspected. All were decorated and maintained to a good standard. The Owner oversees the care of the premises and is quick to act should any area need attention. The residents’ bedrooms are homely and individual to their occupants. A cleaner is employed four days per week and follows a cleaning schedule supplied by the Acting Manager. All residents’ comments about the environment were positive and included: ‘This home is cosy and comfortable.’ ‘All the rooms look nice.’ ‘I think the decoration is good.’ ‘The staff keep the home very clean.’ ‘The bedrooms are lovely. The staff do them every day.’ ‘The night staff last night hoovered and worked so hard – the home was beautiful this morning.’ Pine View Care Home C51 S64266 Pine View V226722 130505.doc Version 1.30 Page 17 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28 The staff team work hard and the residents are impressed with them. Relationships between the residents, the Owner, and the staff are excellent. EVIDENCE: The home currently has 15 residents. Of these four need the assistance of two staff to transfer (although not at night), and the rest transfer independently. There are two carers on duty during the day and one waking carer, plus another on call, at night. Residents made many positive comments about the staff and the Owner including: ‘The staff are excellent ‘All the staff are fabulous’ ‘Sue (the Acting Manager) works very hard. All the staff do.’ ‘They’re lovely the staff. They help us a lot.’ ‘We just ask them for anything and they get it.’ ‘We’re so lucky with the staff – they’re marvellous.’ ‘Dinesh (the Owner) is very committed to the home.’ ‘Dinesh talks to us every morning.’ ‘The man who owns the home is lovely – he’s always talking to us.’ Pine View Care Home C51 S64266 Pine View V226722 130505.doc Version 1.30 Page 18 Excellent relationships were observed between the staff, the Owner, and the residents, and this contributes to the home’s happy and lively atmosphere. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 36 At present the Acting Manager does not have any designated time to attend to her management responsibilities. Formal staff supervision is not being provided at present so staff do not have the opportunity to consider their work performance and training needs EVIDENCE: The home has a new Acting Manager who has worked in care for 13 years. She has experience and training in working with older people with dementia and other mental health needs. The Acting Manager works as a full-time carer in the home. She is responsible for managing the staff team and overseeing all residents’ records. It is recommended that she is given some designated non-caring hours each week to concentrate on her management responsibilities. He meets with the Registered Manager once a week to discuss how well the home is running. At present arrangements for staff supervision are informal. The Acting Manager said this is because she works alongside her staff and is able to supervise them informally on a daily basis. She also carries out spot checks, including at nighttime, to ensure the home is running smoothly. The Owner carries out annual staff appraisals. It is recommended that the Acting Manager is given the opportunity to carry out some formal (ie recorded) staff supervision.
