V34916 dated 7th September 2006 To provide care for one named service user under the age of 65 years Date of last inspection Brief description of the care home Polebrook Residential and Nursing Home is a fifty one bedded purpose built care home located in the village of Polebrook, which is three miles from the town of Oundle. Birchester Medicare Ltd own the home. The home provides for older people requiring

The accommodation is all located on the ground floor, with a mixture of single and double rooms , the majority of which have en-suite facilities. The home has several communal rooms and a large conservatory area. The following fees were provided by the registered manager as being current on 20 April 2009. Privately funded people are charged between £575 and £625 dependent on assessed need. Local Authorities who are funding residents are charged at the rate which is set in their contract and again this depends on peoples assessed needs. For people funded by a Local Authority a `top up fee is usually required to be payable by the person using the service. The fees include personal care and where applicable nursing care, meals and accommodation. Chiropody, hairdressing services, and newspapers can be arranged and are charged separately. Other costs would include clothing and toiletries. Care Homes for Older People Page 3 of 9 What we found:
On arrival to the home the inspector found liquid medication for a person who use the services unattended in a communal area. On return to the medication tray, the nurse on duty was able to confirm to the inspector what was in each pot and that they were intended for one person. The home have a secure, designated treatment room to store all medication and a separate internal room to this to house all external creams and bulky items. The Controlled Drugs in the home are stored in a designated cabinet but this did not conform to The Misuse of Drugs Act 1973 ( safe custody). Apart from one of the Controlled Drugs kept at the home, the audits carried out tallied with the records made in the Controlled Drugs Register. The staff would manage most peoples medicines and would obtain these supplies mainly in a monitored dosage system. The administration to each person is documented on a record sheet. To ensure the right medicine is given to the right person a photograph is retained with the administration record sheets. For one person it was noted that a family member had given the home a cream to use. It had been documented in the daily notes when this had been applied to the bottom area but there was no supporting information on the administration sheets or in the care plans. A sample sheet of signatures to assist in noting which staff member gave out a particular medicines was not seen. To support the carers and nurses on individuals needs, medication profiles are kept within the care plans but it was evidence that these are not always updated. To ensure that all medicines supplied in via the supplying pharmacy are accounted for the nurses and carers were checking in medicines on the recording sheets. The homes recording systems were shown not to be robust as three medication audits carried out did show anomalies with quantities. It was evidenced that stock supplied in one month was not being accounted for in subsequent months even though it may have been given in those months. What the care home does well:
There is a system in place at the home to ensure that when a person needs to see a GP, that this is prompted quickly and all the records are to hand. Medication is stored securely at the home. The staff ensure continual supplies of prescribed medication are available to the people who use this facility. The staff have received relevant training in handling medicines in a care home setting and and follow the homes policy on handling medication. Medication is administered as the Prescriber intended and also any additional actions that requires to be followed is being acted up on too.
