Conditions of registration: None Date of last inspection 30th June 2005 Brief Description of the Service: Queensbury Road is a home for up to 5 service users with learning disabilities. The home is owned and managed by Royal Mencap Society who have many years experience of providing care in residential settings. 22 Queensbury Road is situated amongst other houses, similar in age and design, close to local shops and accessible by public transport. The premises offer single bedroom accommodation for all service users. Communal space provided includes, a lounge, conservatory, kitchen, bathroom and shower room. This is an overview of what the inspector found during the inspection. This unannounced inspection took place during an early-morning visit over a period of approximately 2 hours. The timing of the visit enabled the inspector to observe the breakfast and early-morning routines in preparation for service users going off to their different day-care settings. There were brief discussions with all service users, a member of staff on duty and more detailed discussions with the acting manager for the home. All parts of the home were seen during the visit. This is the second inspection visit of the year and the majority of the key standards have been assessed during the first visit, therefore the focus of this visit was on gaining the views of service users regarding the quality of care, reviewing previous requirements and recommendations from the last report and assessing the remaining key standards through a review of records and discussions with the acting manager. What the service does well: The home continues to provide a family style environment, service users were keen to stress their satisfaction with the arrangements for their care and they particularly enjoyed the Christmas and New Year period. Given that the group of service users tend to spend much of their social time together, they chose as a treat over the holiday period to have individual outings of their choice including a meal out with a member of staff rather than a whole group outing. Service users participate fully in the running of the home including cooking and other domestic duties in line with their level of interest and abilities. There is an individual focus of care, for example one service user has chosen to spend less time in day care placements and the home has arranged extra funding to allow the service user to remain in their home during the day or to have alternative outings. Service users are offered a healthy diet and enjoy their meals and mealtimes. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home respects and promotes service users rights within the context of group living. EVIDENCE: A review of records and discussions with the manager and with a service user showed the practical ways in which the home encourages service users to accept and develop areas of responsibility e.g. through the choices they make and the routines they decide to follow. This was particularly evident in the routines arrived at for one service user who now enjoys a much better quality of life, combining attendance at day care with simply enjoying her home life when other service users are out of the home. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 and 20 The home provides individual service users with appropriate levels of personal and health care support EVIDENCE: At the request of the service user the manager has made appropriate arrangements for the refurbishment of her bedroom which allows her to spend more time in her room engaged in solitary activities which she enjoys. Another resident has particular health-care needs, which dictates that there is close and active monitoring of food intake and general health care. Care Plans contains sufficient information to ensure all staff on duty are aware of the routines and potential risks to health and general well-being. Staff are able to manage and support the service user in a manner which is not oppressive. The home has reviewed its medication system and further developed arrangements with the medication provider to enable a service user to continue to self Medicate in a safe manner. EVIDENCE: There are no current concerns, complaints or protection issues in the home, in discussions the manager was able to recount how the home had dealt with a Protection Of Vulnerable Adults issue sometime ago. It was clear that all procedures had been carried out properly and that the manager had a thorough understanding of the processes. EVIDENCE: The manager is aware of the need to ensure there is regular review of Care Plans and risk assessments and is in the process of reviewing some service users care files. There are effective arrangements in place to allow one service user to continue to self Medicate. The expressed wishes of one service user have been responded to and changes made to staffing levels and the service users daily routines, the practical outcome of this has been an enhanced quality of life for the individual concerned.
