What the care home does well

The home has maintained a long standing permanent staff group and those who were on duty at the time of the inspection were observed being attentive and competent in their responses to the needs of the service users. Members of the care support staff team expressed satisfaction with the level of training and support they received at the home. The service users were appropriately dressed and appeared relaxed and comfortable within their environment. They were observed to relate in a friendly and spontaneous manner with the various members of staff and service users, who spoke to the Inspector, indicated that they were happily settled at the home. The records, policies and procedures were satisfactory and indicated that the health, welfare and safety of the service users were being protected. Overall, the home was found to be clean and hygienic. The environment was homely, lively and safe.

Rainbow Lodge is a Nursing Home for service users with mental health problems and accommodates twenty people. Mr and Mrs Gopaul own the home and they are the Registered Providers. Mrs Gopaul is the Registered Manager. The accommodation consists of four single bedrooms and eight double bedrooms. There are bedrooms on each floor. There is a dining area and a few stairs lead to the lounge area on the ground floor. The dining room is comfortably furnished and is a designated smoking area. There is also a second quiet room on the first floor. The kitchen and laundry are on the ground floor. There are toilets and bathrooms on each floor. There are no lifts in the building so it is suitable only for service users with good mobility. There is a large secluded garden at the rear of the building and parking spaces for several cars in front of the home. The local community services are easily accessed. Staff supports the service users to undertake some small tasks, such as looking after their own laundry. Registered Mental Health Nurses, care staff, domestic staff and a cook are employed to provide care and practical support to the service users. This unannounced was carried out between 11.30 am and 4.20 pm on Monday 7th November 2005. The Registered Provider, Registered Manager, Nurse in Charge, four members of the care support staff group and eighteen service users were present. During the course of the inspection, discussions were held with three care support staff members and five service users. A tour of the building was undertaken. Records, policies and procedures were viewed and observations were made. Requirements that were made at the last inspection and Standards that were not covered were examined at this inspection. The Registered Provider, Registered Manager and Nurse in Charge, were cooperative and provided appropriate assistance throughout the inspection. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3,4 and 5. EVIDENCE: It was reflected on records viewed that a letter confirming that the home will meet the assessed needs of individual service users was sent to prospective service users and their relatives prior to admission. The Inspector was advised that prospective service users and their relatives were invited to the home and were required to participate in assessing the home’s suitability and capacity to meet specific needs. Contracts/statement of terms and conditions that had been signed by service users or their relatives were viewed and found to be satisfactory. Details relating to accommodation were seen to be included within the Service Users Guide. This complied with the requirement made at the last inspection. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 and 10. Consultations are held with service users in relation to their daily routines at the home and their rights to privacy and confidentiality are being upheld. EVIDENCE: The Inspector was informed that service users at the home were given choice in relation to meals, indoor and outdoor activities, what they wore, when they got up in the mornings or retired at night. Service users also handled their own money and were free to make purchases of their own choosing. It was evidenced on minutes viewed that regular service users meetings were being held at which decisions such as the menu and shared activities were made. A service user meeting occurred on the day of the inspection and it was decided that a number of service users would attend a specific shared outdoor activity. The home’s policy and procedures relating to confidentiality were in place. The Registered Provider confirmed that service users were assured that their privacy and confidentiality would be upheld at the home and that individual sessions with service users and their key workers were regularly held. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16. Service users are encouraged to be independent at the home and their rights are being respected. EVIDENCE: Service users were observed to move freely and comfortably around the home. They related in a relaxed and friendly manner with other service users and also with members of the care support staff team who were on duty during the inspection. The Inspector was informed that service users handled their own money and were free to go and come as they wished during waking hours. Many service users were observed independently leaving and returning to the home at the time of the inspection. Others sat in the smoking area, read the newspapers or participated in board games. Individual locks had been fitted on all bedroom doors and staff members were observed knocking on individual doors prior to entering. It was reflected on records viewed and in conversation with staff that service users assisted with doing their own laundry. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 and 21 The home’s policies and procedures in relation to medication and ageing, illness and death, were satisfactory. EVIDENCE: The home’s policy and procedures relating to medication were in place and the storage and administration of medication kept at the home were satisfactory. It was confirmed by the Nurse in Charge that none of the service users were administering their own medication at the time of the inspection. The Inspector was informed that the Registered Mental Health Nurses employed at the home were responsible for administering medication to the service users. The home’s policy and procedures on ageing, illness and death were in place. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22. The home policy and procedures in relation to complaints were satisfactory. EVIDENCE: The home’s policy and procedures in relation to complaints were clearly stated and accessible to the service users and their relatives. It was indicated on records seen that training on the Protection of Vulnerable Adults had been delivered to all members of the care support staff team. The complaints book was viewed and no complaints had been made to the home following the last inspection. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25, 26, 27, 28, 29 and 30. Service users bedrooms are personalised and are suitably furnished and fitted. However, carpets and items of furniture were worn and needed replacing. None of the service users are currently in need of specialist equipment. The communal areas within the home are spacious and appropriate for shared or individual activity. The home is clean, hygienic and well maintained. A requirement made under Standard 30 at the last inspection had been complied with. EVIDENCE: The service users bedrooms were viewed during the inspection. They were found to be suitably furnished and fitted and reflected individual choices and interests. However, chairs and carpets in several bedrooms were worn and needed to be replaced. The Inspector was shown a written plan that had been put into place for the refurbishment of service users bedrooms. The home has a spacious lounge, dining area and smoking room that are appropriate for shared and individual activity. There is also a large garden which is accessible from the lounge. The service users are able-bodied and no specialist equipment is currently required. The home was essentially clean and hygienic throughout. The requirement made under Standard 30 at the last inspection had been met. This related to the locking of a cupboard in which detergents were being stored. Service users benefit from clarity of staff roles and responsibilities. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35 and 36. The staff members that are employed at the home have varied roles and responsibilities of which the service users are aware. Appropriate training for meeting the needs of the service users have been delivered to the members of the care support staff team and they are regularly supervised. The home’s policy, procedures and practices in relation to recruitment are satisfactory. A requirement made under Standard 35 at the last inspection, had been met. EVIDENCE: The Staff group at the home includes Registered Mental Health Nurses, care support workers, domestic staff and a cook. A key worker system is in operation at the home. The Inspector was informed that the service users were aware of their individual key worker and also of the various roles within the care support staff team. Rainbow Lodge Nursing Home DS0000010960.V265964.R01.S.doc Version 5.0 Page 17 It was evidenced on records seen that staff training delivered included: Protection of Vulnerable Adults, Mental Health, Challenging Behaviour, Health and Safety, Fire Safety and Customer Care. Nine care support staff members had obtained NVQ qualification in levels 2 or 3. The Registered Provider confirmed that two members of staff were scheduled to commence training very soon. Members of the care support staff team who were on duty at the time of the inspection were observed to respond appropriately to the needs of the service users. A number of personnel files were examined at random and all were found to contain the required documents. Supervision records viewed were satisfactory and indicated that they occurred at least six times annually. The Requirement made under Standard 35 at the last inspection in relation to the updating of training programmes, had been complied with. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 38, 40, 41, 42, and 43. The home is organised and well run and the Registered Manager is experienced and appropriately qualified. A friendly and inclusive approach is being operated at the home. The records, policies and procedures are satisfactory and the rights, best interests and welfare of the service users are being safeguarded. A requirement made under Standard 42 at the last inspection had been complied with. EVIDENCE: The Registered Manager is a Registered Mental Health Nurse and has obtained a degree in Psychology. She confirmed that a friendly, caring, inclusive and respectful approach was in operation at the home. The Registered Provider and Registered Manager attended a service users meeting on the day of the inspection and service users were observed being relaxed and open in their interactions with them. The home’s records, policies and procedures were up to date, securely kept and indicated that the best interests of the service users were being safeguarded. Overall, the home was found to be organised and well run. The requirement made under Standard 42 had been met. This related to containers being placed in the garden for the disposal of cigarette ends.
