What the care home does well

The home provides a domestic and comfortable environment, which is very well maintained. The manager and staff are very committed to providing good standards of care for the residents. The food is well thought of and residents feel respected and enabled to socialise in a manner that is comfortable for them. The gardens are well maintained. The home`s consistently provides good standards of care and continues to meet a majority of the National Minimum Standards.

What has improved since the last inspection?

Nothing in particular was noted to have improved, residents and staff continue to state that they feel the home is consistent in maintaining good standards.

What the care home could do better:

Nothing was identified in terms of outcomes for residents, during this inspection, though not all of the standards were assessed. It was noted that some hot water outlets continue to be over the accepted level of 43 degrees. It is required that until temperatures are adequately controlled, thorough risk assessments are put in place to safeguard residents. Radiator guards, which were required to be installed at the previous inspection, are largely out standing, though the manager stated that these were being installed that day.

There is a good level of adaptation to meet the needs of residents with a physical disability. The furnishings and lighting in communal areas are domestic in character. Residents are encouraged to bring in items of furniture from home to personalise their rooms. There are ample communal, bathing facilities and areas for social recreation. The grounds are tidy, secure and accessible to wheelchair users. Sales on 5 December 2005. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for residents and their views of the service provided. This process considers the home’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provisions that need further development. The primary method of inspection used was ‘case tracking’ which involved selecting two residents and tracking the care they receive through review of their records and either discussion with them, the care staff and observation of care practices. The inspector also spent time talking to other residents in the home and two members of staff. Overall the feedback was very good. Residents were happy to express their views about the home, they were positive in terms of the skills and attitude of the staff and of the overall standards of care, food, social recreation and the environment. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3.6. Residents are fully assessed before moving in to the home. The home does not provide intermediate care. EVIDENCE: The records in two resident’s files were checked as part of this inspection. One person had an extended social work assessment, which had been obtained prior to their admission. All files contained assessments conducted by the manager, or deputy manager. All of the assessments were comprehensive and contained sufficient information to enable staff to ensure that they could meet the residents assessed needs. There were detailed action plans for care workers. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Residents receive a comprehensive assessment. Assessments are updated following a review. The home is able to meet the healthcare needs of it’s residents. Medication issues are managed appropriately. Residents are treated appropriately. EVIDENCE: Two care plans observed, were well set out and detail each area of need and an action plan is drawn up to meet this need. Risk assessments were well documented in each of the resident’s plans that were inspected. Particular attention is placed in the need to prevent pressure sores, falls and safe working practices. Daily records are well maintained by care staff and professional input from district nurses and GP’s is well documented. Evidence gained from speaking to residents and staff suggested the care planning process was accurate and outcomes satisfactory. Red Roofs Care Home DS0000008740.V270798.R01.S.doc Version 5.0 Page 10 Resident’s plans contain details of each individual’s health care needs, including tissue viability and continence risk assessments. There is evidence that people have been appropriately referred to health care professionals. Care plans viewed contained records of visits by district nurses, General Practitioners and other professionals. Healthcare professionals were observed visiting on the day. The inspector was informed that a number of doctors surgeries are used to ensure that the residents can register with a GP of their choice. Staff training records evidenced that medication training was provided for staff responsible for the administration of medication. The homes medication administration systems have been well maintained. There is a policy and procedures for receiving, recording, storing, handling, administering and disposing of medicines. The home is registered with the local pharmacist and support and advice obtained as and when needed. The pharmacist visits twice a year and conducts and audit of the homes medicines. Staff were observed during the visit interacting positively with individuals, a resident spoken with, reported staff provide a good standard of care and areas of concern would be discussed with the registered manager. All residents who spoke with the Inspector commented very positively on the conduct and attitude of the staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Residents are supported to access daily activities and access the local community. Residents feel they retain much of their independence after moving into the home Resident’s maintain contact with family and friends. Food provided at the home is considered to be very good by the residents. EVIDENCE: Residents spoken with felt they were happy with the level of activities within the home and outside. Residents commented that the philosophy of the home and the attitude of the staff enabled them to make choices and felt they were generally well respected. The staff reported that they encourage residents to participate in events and outings. Planned trips and events are organised within the home. Residents also spoke highly of visiting entertainment arranged by the home. Although there is no formal activities program, the manager arranges for local artists to visit the home, who provide mainly musical entertainment. These are particularly popular with those with cognitive impairments. Residents spoken with commented positively on the standards and quality of food. Staff spoken with, were well aware of residents individual preferences. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18. The home operates suitable complaints and adult protection procedures. EVIDENCE: The inspector observed a satisfactory complaints policy and procedure, on display and contained in the Residents guide. Records are well maintained and very few complaints are received. Residents spoken with, stated they would raise concerns with the registered manager if they felt the need to. The home has an appropriate Whistle Blowing Policy and a policy detailing Adult Protection Procedures. The homes policies and procedures for responding to suspicion or evidence of abuse, or neglect, are all satisfactory. The home has comprehensive policies regarding resident’s money and financial affairs. Training certificates in respect of Adult Protection were observed on staff files, this is also covered during the induction process. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,25,26. The home was clean and tidy at the time of this inspection. The home is well maintained. Radiator guards still require fitting. The hot water supply is not regulated appropriately. EVIDENCE: The home is accessible, safe and well maintained. Residents spoken with feel it meets their individual and collective needs and is comfortable and homely. Rooms observed, are centrally heated and heating may be controlled in the resident’s own room. Lighting is domestic in character and includes a bedside light. Emergency lighting, which is an integral part of the fire alarm system, is provided throughout the building. Water outlets are temperature recorded, but it was noted that some exceed the regulation 43 degrees. These are required to be regulated and risk assessed. Radiators are still exposed and require guards, though the manager stated that this was in the process of being done. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. Staff are recruited and employed in suitable numbers. Staff are trained in mandatory subjects required by this standard and for care specific support. EVIDENCE: On the day of inspection there were two care staff, one senior care staff, the registered manager, deputy manager, one cook and two domestic staff on duty. The staffing levels meet requirements, with the current resident occupancy level. There are suitable recruitment policies and procedures that have been observed at previous inspections. Two staff files were sampled at random. These contained evidence of applications, interviews, pre-employment checks, criminal declarations, confidentiality agreements, supervision and training plans. Both files contained a CRB with disclosures. Of the staff files examined, they all contained evidence of training in a number of health and safety subjects, dementia awareness, NVQ level 2, staff supervision and appraisal plans. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,36,38 Residents are consulted over issues in the care home. Staff are appropriately supervised. Health and Safety is generally well managed. EVIDENCE: Residents stated that they felt they were consulted about day to day issues. The inspector observed a standard induction procedure. The manager reported that she supervises and completes appraisal for all the senior and care staff. Records of appraisal were viewed. All staff files contained records of supervision and appraisal. Staff spoken with also supported this process. Staff that were spoken with were aware of health and safety procedures and commented positively on the training provided. Risk assessments were observed on individual files and are in place for the building and individual residents. Records for Health and Safety monitoring and the servicing of systems and appliances were inspected on this occasion and were found in general, to be up to date.
