Conditions of registration: 1. Redcliffe House Residential Care Home is registered to provide accommodation and personal care to a maximum of 8 men and women whose primary care needs fall within the following category: Learning Disability (LD) NA Date of last inspection Brief Description of the Service: Redcliffe House Residential Care Home is situated in Mansfield and is close to local amenities. It is a small converted family home offering accommodation and personal care to a maximum of eight service users whose primary care needs fall within the category of learning disability. Accommodation is provided within eight single bedrooms, one bedroom offering ensuite facilities, comprising of a toilet and sink. There are two bathrooms and three shower rooms for service users use. The service is not wheelchair accessible, which the provider discussed with the Disability Rights Commission, and the statement of purpose was amended to reflect this. There is adequate communal space comprising of a living room, dining room, kitchen and training kitchen. There is access to an enclosed medium sized garden. The manager said that the current weekly fees are £1200 as a basic payment then additional payments based upon the needs assessment may be required which is discussed at the time of the initial assessment. This is an overview of what the inspector found during the inspection. This unannounced inspection took place four and three quarter hours and was performed by one inspector. The main method of inspection was case tracking, this is a method of sampling the records of three randomly selected service users to ascertain if the needs of service users are appropriately assessed and identified needs are being catered for by the home to maintain optimum health and wellbeing of the service user. There are currently only two service users residing in the home each with varying needs. Therefore only one service user was spoken with during the inspection so as to give the inspector an insight into the conditions and standards within the home. The service user spoken with was happy with the care received and life within the home. The manager assisted in the inspection process and two members of staff were spoken with. Staff were able to discuss service users needs and how service users are supported and facilitated to live an independent life as able. The service has worked towards establishing the services and facilities on offer and ensuring a quality service is offered to service users. Time has been spent on researching services and facilities within the community to ensure these may be accessed as required. The service has implemented various policies and procedures to ensure a quality service is offered and service users are safe. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Prospective service users individual aspirations and needs are assessed. EVIDENCE: Prior to admission prospective service users undergo an in depth preadmission assessments. Those assessments seen were extremely in-depth and informative ensuring individual needs and expectations were fully identified. Prospective service users are visited in the community and may also visit the home prior to making a decision to live there. Where one service user had moved into the home from a different town, time was spent prior to them making a decision getting to know the area and the facilities and services on offer. Programmes of support and recreational activities were seen to be arranged prior to a decision being made. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7 and 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The manager said that service users are supported to maintain independence as they wish and staff aim to facilitate this. Both staff and the service user spoken with confirmed this. Key workers are designated to service users to ensure that continuity of support is offered. Support plans are designed for each individual and a section is dedicated to ‘All about me.’ Service users are supported to fill in this section themselves. Those seen were completed in varying degrees, as this is an ongoing process. These were extremely personalised and reflected service users individual needs and aspirations. One service user spoken with said that staff were nice and their needs were met.

There was evidence seen within the support plans to confirm this takes place. Staff spoken with discussed how service users are facilitated to lead a life of their choosing with support. The service user spoken with discussed their trip to town that had taken place that morning. Appropriate risk assessments were in place within service users support plans. The manager said that these are discussed and negotiated with service users, and a balance is reached so as to ensure service users rights are maintained. Facilities to hold personal finances are available should a service user wish to use this service. If a service user wishes to hold their own finances a lockable facility is available within service users rooms. Personal money held by this facility was checked and was correct. Currently social workers are used as advocate, however the manager said that should the need arise an independent advocacy service would be accessed. This judgement has been made using available evidence including a visit to this service. The manager said that service users are supported to access facilities in the local community and this has helped service users to build up their confidence. Service users have an individual structured programme in place to ensure personal likes, hobbies and aspirations are continued. There is a training kitchen in the home and several days a week service users are supported to purchase, cook and eat the meals they have chosen to prepare. The manager said should a service user wish to attend college they would be facilitated and supported in doing so. One service

Staff support service users to access facilities and services within the community. Service users are also supported to carry out household tasks following agreements made. On the day of the visit one service user had been to the local town with two members of staff. On speaking with the staff and service user it was confirmed that this was a regular occurrence. There are no restrictions placed upon visiting and visitors may be received in private should they wish. There was evidence seen within training files to demonstrate that staff undergo an induction which is based upon a recognised structure, included within this is instruction on maintaining service users rights. Service users receive their mail unopened. A wholesome and appealing diet was on offer and service users and staff plan the menus on offer jointly, this was seen to take place during the visit. The kitchen was clean and tidy. Relevant records such as food, fridge and freezer temperatures were seen, cleaning records are maintained within the diary to state when this has taken place. Following discussions the manager is aiming to view the safer food better business documentation and adapt it to the service accordingly. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 and 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users receive personal support in the way they prefer and require. Service users physical and emotional health needs are met. Service users are protected by the homes medication policies and procedures. EVIDENCE: Support plans are in place for personal support as required, those seen demonstrated that service users are supported to be as independent as able. Staff spoken with discussed how personal care is offered. The multidisciplinary team and specialist services are accessed as required. The manager said that as many as possible are accessed within the community and service users are supported in accessing these. There was evidence seen to confirm this within service users case files. Appropriate medication polices and procedures were in place. Staff who administer medication have undergone training via the supplying pharmacy, staff spoken with confirmed this. Medication records seen demonstrated that currently handwritten entries are only signed by one member of staff, following discussions of good practice recommendations with the manager she intends to implement that two staff sign these.

The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. EVIDENCE: Relevant policies and procedures are in place for dealing with complaints and concerns. One complaint had been received from a service user since the opening of the service. This had been recorded and was seen to have been dealt with appropriately. One service user spoken with said they were happy with life within the home and they had no problems. The manager said that she has an open door policy so that anyone can speak with her at any time. Relevant polices and procedures were in place in regards to adult protection. The manager said that training for this had been arranged, staff spoken with confirmed this. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users live in a homely, comfortable and safe environment. The home is clean and hygienic. EVIDENCE: A tour of the home was undertaken and showed that suitable furnishing was available. The manager said that service users may bring in their own furniture if desired. One service users room observed was seen to be personalised. All areas of the home were clean, pleasant and hygienic. Service users where able are responsible for the cleaning of their own rooms and carrying out their own laundry with support from staff. Relevant maintenance is carried out on a routine basis and on request; evidence of this was seen within records held. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34 and 35. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are supported by competent and qualified staff. Service users are supported and protected by the homes recruitment policies and practices. Service users individual and joint needs are met by appropriately trained staff. EVIDENCE: The duty rotas were seen which showed that sufficient staff were on duty to meet service users needs. The manager said that staff levels were dependant upon the number of service users residing in the home and their needs. Both staff members and the service user spoken with confirmed this. Four staff files were seen and contained all the required documentation. Staff spoken with confirm that has undergone a Criminal Record Bureau Check. Staff said that they felt supported in their personal development and that sufficient staff training was available. Records seen showed that staff are working towards completing all mandatory training. This judgement has been made using available evidence including a visit to this service. Service users benefit from a well run home. Quality assurance systems are being developed to ensure the home is run in the best interests of service users. The health, safety and welfare of service users are promoted and protected. EVIDENCE: The manager is registered with the Commission for Social Care Inspection. She has 16years experience in working with people experiencing learning difficulties and has previously managed a care home for the older person. She is in the process of completing the registered managers award and remains up to date with mandatory training. She has attained the City and Guilds Advance Management in care level 4. She said that she felt supported in her job role. Staff spoken with spoken highly of the manager and confirmed she was always approachable. Redcliffe House Residential Care Home DS0000067779.V319101.R01.S.doc Version 5.2 Page 18 The manager said that quality assurance systems are currently being developed and the service is working towards the Investors in People Award and the National Autistic Society – Autism Accreditation Award which reflects that the service is up to date and able to offer appropriate care to service users. Relevant maintenance servicing certificates were seen which were satisfactory. Accident records observed contained significant information and demonstrated that appropriate action had taken place. The fire logbook showed that the fire alarm system was tested on a weekly basis, however although a member of staff and the manager said that emergency lights were tested on a monthly basis this had not been recorded. Also it was stated that staff have undertaken fire drills however there were no records available to confirm this.
